What is the name of the disease characterized by both vomiting and diarrhea? The *Neijing·Re Lun* (Treatise on Heat Diseases in the Yellow Emperor’s Inner Canon) states: This is a syndrome of tetany and cholera-like disorder. What qi causes this? It occurs because pathogenic qi resides in the middle jiao, preventing the ascending and descending of yin and yang qi. The symptoms include heart pain and vomiting; before vomiting, there is abdominal pain followed by diarrhea. If both the heart and abdomen are painful, vomiting and diarrhea occur simultaneously, causing sudden chaos and confusion within an instant. Patients with this syndrome prefer cold substances. It can be cured by administering Wuling San (Five-Ingredient Poria Pill) and Yiyuan San (Benefit Origin Powder) mixed with ice water. Hot substances are strictly contraindicated. How should tetany be treated? The Classics state: "For exhaustion, use warmth." Here, "warmth" implies gentle nourishment and preservation.
Another method: Administer Shengjiang Tang (Fresh Ginger Decoction), Yiyuan San (Benefit Origin Powder), Baizhu San (Atractylodes Macrocephala Powder), or Yugong San (Yu Gong Powder), chilled with ice, and sip slowly. If thirst persists, drink frequently. If ice is unavailable, fresh well water is acceptable. When using these remedies, white rice porridge or rice soup is strictly forbidden. Dry and hot herbs such as Guizhi (Cinnamon Twig) and Fuzi (Prepared Aconite Root) must not be taken; ingestion will lead to death. In areas without medicine, one may consume Dijiang (Earth Slurry). To prepare Dijiang, dig a pit, fill it with fresh water, stir until muddy, then immediately collect the clarified portion. Drink three to five cups for immediate recovery.
For adults and children suffering from sudden, incessant watery diarrhea, the *Neijing* states: "This is a rapid-onset pouring diarrhea." If prolonged and unhealed, it is termed Yongxie Zhuxia (Gushing Diarrhea). This is a disease caused by excessive Fire movement due to its swift and violent nature. Urgently administer Ganlu Yinzi (Sweet Dew Elixir), Wuling San (Five-Ingredient Poria Pill), and Tianshui San (Heavenly Water Powder) mixed with fresh well water; or decoct these herbs in Jinghua Shui (First-drawn Well Water), cool them, and take orally. The illness will then subside. Do not use Yu Mi Qiao (Pericarpium Papaveris Rubrum / Poppy Shell), Ganjiang (Dried Ginger), Doukou (Cardamom Fructus), or Sheng Sanzi (Holy Mixture Powder). Even if diarrhea stops, these drugs cause stagnation of intestinal and stomach qi, leading to abdominal distension. This approach emphasizes distinguishing yin and yang and promoting urinary flow, which constitutes the subtlety of treatment.
The *Shanggu Tianzhen Lun* (Discourse on the Heavenly Truth of Antiquity) states: "The interaction of one yin and one yang is called the Dao." Therefore, males and females possess different yin-yang constitutions, resulting in distinct manifestations of Tiangui (Heavenly Nectar / Reproductive Essence) and blood. Females, being yin and static, experience regular menstrual fullness and overflow; males, being yang and active, release essence during sexual intercourse. Through their harmonious union, procreation becomes possible. The *Yijing·Xici* (Great Commentary of the Book of Changes) states: "When male and female unite their essences, all things are transformed." Humans acquire form by inheriting the qi of heaven and earth.
