For apoplexy (stroke) resulting in aphonia, mental confusion, and deviation of the mouth and eyes: The *Neijing* states that pathogenic wind is characterized by its mobility and rapid changes. Upon invasion, it harms the human body, causing sudden alterations; thus, all diseases originate from wind. San Sheng San (Three Holy Powders) may be used, instilled into the nasal cavity to induce expectoration of phlegm-fluids, after which the mouth will naturally open. In cases of unconsciousness with locked jaws where gruel or medicine cannot be swallowed, this formula is indicated. If these severe symptoms are absent, San Sheng San may still be used for emesis; subsequently, Tong Sheng San (Unsurpassed Holy Powder), Liang Ge San (Cooling Diaphragm Powder), Ren Shen Ban Xia Wan (Ginseng and Pinellia Pill), or Gui Ling Gan Lu San (Cinnamon Twig, Poria, and Sweet Dew Powder) may be administered. One must strictly avoid foods that stir wind or generate phlegm, such as chicken, pork, fish, rabbit, alcohol, vinegar, and buckwheat.
The method of inducing emesis to expel phlegm applies to conditions such as head wind with vertigo, numbness of the extremities, epigastric pain, fullness in the heart and abdomen with fluid sounds upon palpation. Du Sheng San (Single Holy Powder) may be used for emesis; afterward, acrid-cool herbs for clearing the upper jiao should be taken. Zhang Zhongjing stated: "This is caused by cold phlegm knotting in the chest."
For epilepsy progressing to a dazed state, use San Sheng San for emesis. Administer in a warm room protected from drafts, allowing diaphoresis, purgation, and emesis methods to be employed together. Subsequently, administer Tong Sheng San; recovery occurs within a hundred days.
"Lei Tou Feng Lan Gan" (Thunder Head Wind Laziness/Dryness) is a colloquial misnomer. This condition involves cold phlegm in the chest, often caused by frequent hair washing. Cha Tiao San (Tea Regulation Powder) may be used for emesis to expel two to three sheng of phlegm, followed by Shen Xiong Wan (God Xiong Pill) for purgation (three to five bowel movements). Finally, Yu Feng Bing Zi (Wind-Healing Biscuits) restores health. "Lei Tou" refers to red, swollen nodules on the head, resembling sour jujubes; pricking them to bleed cures the condition.
Chronic red eyes lasting years are termed "Head Wind Invasion," accompanied by headache. Use Du Sheng San for emesis, followed by Xi Xin San (Washing Heart Powder) or Ba Zheng San (Eight Righteousness Powder). Acute onset of red, swollen, and painful eyes indicates heat in the Foot Jueyin Liver Meridian; treating with cold herbs that promote urination, clear liver heat, and dispel wind-heat leads to recovery.
Wind-induced tearing, colloquially called "Cold Tears," is a misconception. The *Neijing* states: "When the Taiyang cannot secure fluids, external wind strikes while internal fire burns; heat clashes, causing tears." The *Neijing* also says: "Heat causes the five fluids to exit." Excessive heat leads to tearing. Treatment uses one Beimu (Fritillaria Bulb), preferably white and fine-grained, and seven Hu Jiao (Pepper grains), ground finely without copper or iron contact, applied to the eyes before bed. Therapeutic principles state: "Wind should be dispersed with acrid flavors; cold should be released with sweet flavors." Qi congeals in cold; blood disperses in heat.
Phlegm located above the diaphragm obstructs the clarity of the head and eyes, causing thick sputum, cough, wheezing, and tidal fever. Use Du Sheng San for emesis, followed by Sou Feng Wan (Wind-Searching Pill). The *Neijing* describes this as the meaning of "moistening flow and drying dampness."
For wind-strike, seasonal qi disorders, warm diseases, or shaoyin cold damage within three days, presenting with headache, fever, and aversion to cold: Tong Sheng San and Yi Yuan San (Benefit Primordial Powder), 5–7 qian each, decocted in one large bowl of water with over ten slices of fresh ginger, ten stems of scallion whites (with roots), and a pinch of fermented soybeans. Boil three to five times, strain, and drink half first. After a while, induce vomiting by probing the throat with a hairpin; do not rinse the mouth. Then drink the remaining half. Follow with an acid-acrid soup of scallions and vinegar. Cover with blankets until sweating occurs, resolving the disease. In turbulent times, this aligns with the *Neijing*'s concept of "Excessive Fire Year," where intense heat affects the Metal/Lung organ system. Acrid-cool formulas ensure success. In calm times, similar to water transformation, acrid-warm formulas like Sheng Ma Tang (Ephedra Decoction), Ge Gen Tang (Kudzu Decoction), or Bai Du San (Eliminate Toxins Powder) resolve the exterior. Even with tidal fever, there is no harm. Adding fermented soybeans and scallions promotes upward expulsion and spontaneous sweating. The *Neijing* states: "Because it is light, scatter it." Scattering means uplifting. Emesis and diaphoresis disperse cold and heat pathogens. Post-treatment requires careful recuperation; after ten days, the pathogen does not recur.
In adults and children, combined wind, dampness, and cold cause bi-syndrome and numbness/paralysis of extremities. The *Neijing* states: "Ying (Nutrient) deficiency and Wei (Defensive) excess." Skin insensitivity and bi-syndrome without itch or pain are treated with Yu Jin San (Curcuma Powder) for emesis, followed by Dao Shui Wan (Water-Guiding Pill) using mild-cold herbs for purgation. After purgation, acrid-warm formulas induce sweating. Long-term administration of Dang Gui (Angelica Sinensis), Shao Yao (Peony Root), Wu Tou (Aconite Root), and Fu Zi (Prepared Aconite Child Root) invigorates channels and activates blood for cure.
Chronic toothache due to wind-boring teeth is treated by piercing one Ba Dou (Croton Seed) with a needle, burning it until smoke ceases but retaining its nature, and fumigating the gum base.
Diarrhea with undigested food, day and night, borborygmus, and complete undigested stool is treated with Dao Shui Wan or Yu Gong San (Yu Merit Powder) for purgation. For elderly or weak patients with qi deficiency, Wu You San (Worry-Free Powder) is used. Observe the strength of the pathogen; after one or two days, Wei Feng Tang (Stomach Wind Decoction) treats the wind. If unresolved, Gui Zhi Ma Huang Tang (Cinnamon Twig and Ephedra Decoction) induces sweating for cure. The *Neijing* states: "Wind invading the intestines causes intestinal wind and unsifted grain diarrhea." Qixuanzi said: "Wind enters the stomach, steams upward, causing undigested food to pass down." Also: "Evening meals undigested constitute unsifted grain diarrhea." Another classic states: "Wind injury in spring leads to unsifted grain diarrhea in summer." Thus, wind requires sweating. Intestinal rumbling is wind agitation producing sound. Strictly avoid overly drying herbs like Poppy Shell, Cardamom, and Dried Ginger. For gradual cases, dry dampness; for severe cases, attack the pathogen. The classic states: "Decrease implies gradual improvement; increase implies severity." Use Wu Ling San (Five-Ingredient Powder with Poria) minus Zhu Ling (Poria) plus Ren Shen (Ginseng) powder.
Malaria in wealthy families consuming rich foods, whether intermittent or frequent, with varying heat/cold ratios, is treated with Da Chai Hu Tang (Greater Bupleurum Decoction) for purgation (3–5 movements), followed by Bai Hu Tang (White Tiger Decoction), Yu Lu San (Jade Dew Powder), or Gui Ling Gan Lu San. If unresolved, excessive accumulated heat requires Shen Xiong San Hua Shen You Wan (God Xiong Three-Flower Divine Protection Pill) or Tiao Wei Cheng Qi Tang (Regulate the Stomach and Direct Downward Decoction) in large doses for strong purgation. Post-purgation, take Wu Ling San decocted in long-flowing water. Xiao Chai Hu Tang (Lesser Bupleurum Decoction) is also acceptable. Alternatively, start with Chang Shan San (Changshan Powder) for emesis, then Liang Ge San or Bai Hu Tang. Strictly avoid heating foods and five pungent meats (pork, chicken, fish, rabbit); recurrence follows violation.
Malaria in poor rural families, caused by coarse diet, thin clothing, and labor, differs from the wealthy approach. On the day of onset, Wen Pi San (Warm Spleen Powder) from Ye Fu Duo Xiao Fang is used. If unresolved, Chen Sha Dan (Cinnabar Pill) ensures cure. Afterwards, take Bai Hu Tang decocted in long-flowing water; avoid hot foods, as malaria stems from summerheat and latent heat. The *Neijing* states: "Summerheat injury in autumn inevitably causes malaria."
Bone-steaming fever, dry skin, thick sputum, limb pain, flushed face, scorched lips, night sweats, irritability, restlessness, occasional cough/wheezing, loss of appetite, weakness, spontaneous sweating, and emaciation in men and women: The *Neijing* attributes this to essence deficiency in men and blood stasis in women. Use Cha Tiao San for mild emesis, followed by Dao Shui Wan and Yu Gong San for mild purgation (3–5 movements). Then take Chai Hu Yin Zi (Bupleurum Drink), Gui Ling Gan Lu San, or Xi Jiao San (Rhinoceros Horn Powder). Select Da Sou Feng Wan, Bai Zhu Wan, Tiao Zhong Tang, Mu Xiang Bing Lang Wan, or Ren Shen San based on deficiency/excess. Hemoptysis or hematochezia (blood loss conditions) contraindicate emesis. Avoid harsh hot herbs like Ginger and Aconite; ingestion worsens condition, reduces muscle mass, and increases danger. Modern misunderstanding of releasing the exterior and attacking the interior causes this error. Moxibustion on wrists and ankles during summer is strictly forbidden, as these are superficial yang surfaces. The *Neijing* states: "All yang meridians manifest in the four limbs." These points are shallow; moxa sores heal poorly. Moxibustion on chest, Zhongwan, below navel, back shu points, and Zusanli, especially with burning needles, yields no effect and causes suffering. Consider this! Consumption diseases involve cravings; edible desired foods should follow *Shi Liao Ben Cao*. Spinach, mallow soup, and ice-cold items should not be banned. Water and grain enter the stomach to nourish vessels. Prohibition closes the stomach, wastes the body, enlarges the pulse, and signals death. Apply this to all consumption diseases.
For deficiency-consumption with prolonged weakness and no heat signs, tonification is needed. Wu Bi Shan Yao Wan (Peerless Yam Pill) cures.
Hemorrhoids with swelling and pain: The *Neijing* states: "Overeating causes sinews and vessels to relax; intestinal dysentery becomes hemorrhoids and persists, transforming into fistulas." Hemorrhoids and fistulas share treatment. *Zhen Yao Da Lun* notes: "Sun Yang victory brings condensation and cold; ice forms out of season; hemorrhoids and malaria erupt." Commentary: Excess water qi blocks yang fire; yang fire fears water stagnation, causing hemorrhoids. Shao Yin reversal causes febrile rashes, abscesses, and hemorrhoids. Commentary: Internal fire steaming, metal qi repelling externally, yang heat stagnating internally causes rashes and ulcers. Severe rashes become ulcers. Mild heat causes external febrile rashes; severe heat causes internal abscesses. Small intestine heat causes internal and external hemorrhoids. Reversal heat changes affect the posterior and lateral body. *Lingshu* states: "Taiyang deficiency causes hemorrhoidal malaria and mania." Water deficiency allows fire to dominate. Start with Dao Shui Wan and Yu Gong San for purgation, followed by Zhi Ke Wan (Citrus Aurantium Pill) and Mu Xiang Bing Lang Wan. Promote bowel movement with mallow soup and spinach. Strictly avoid sexual activity and heating foods (chicken, fish, alcohol, vinegar, pungents).
Phlegm-cough in the wealthy results from rich flavors. The *Neijing* states: "Rich flavors generate heat." Take Tong Sheng San plus Ban Xia (Pinellia Tuber) to stop cough; follow with Ren Shen Ban Xia Wan to transform phlegm, descend fluids, stop cough, and calm wheezing. The poor often contract wind-cold-dampness. The *Neijing* states: "Dampness injury in autumn generates cough in winter." Take Ning Shen San/Ning Fei San (Calming Spirit/Lung Powder) plus Bai Zhu (Atractylodes Macrocephala Rhizome). Extreme cough with facial flushing and distress indicates fire transforming and attacking the lung; distinguish carefully.
Sand-stone lin syndrome and five types of difficult/dysfunctional urination with umbilical abdominal pain in adults and children are primarily treated with Yi Yuan San, mixed with long-flowing water. Heat in the bladder dries fluids; colloquially called "cold lin," it is a common global error. Wu Ling San minus Gui Zhi (Cinnamon Twig) plus Yi Yuan San is named Dan Shen San (Light Permeation Powder).
Sudden acute red, painful eyes with unceasing swelling, bulging eyeballs, and membrane formation require immediate treatment. Rub stalk grass against both nostrils to induce bleeding for instant cure. For severe cases, prick Baihui (GV20), Cuanzhu (BL2), or between eyebrows to bleed. Hold water in mouth, tighten collar, do not spray water directly. Spit out water once bleeding stops. Acute red swelling is "Dragon Fire" disease, accumulated heat. *Nanjing* states: "Eyes see when nourished by blood." Used reluctantly. Blood transforms into tears; pain causes discharge. Classic states: "Transmitted root disease, treat qi first. In urgency, treat the branch; in slowness, treat the root." Another method: Boil salt in long-flowing water for emesis; follow with Shen Xiong Wan or Si Wu Tang (Four Substance Decoction). Classic states: "Acute illness and sudden death belong to fire." Again: "Treat urgency by addressing the branch, slowness by addressing the root." Branch is red/swelling; root is fire/heat. Salt soup is salty-cold to control fire. For inability to open eyes due to pain, sniff Qing Jin San (Green Gold Powder) into nose to induce sneezing; true qi rises, evil exits.
Mouth ulcers and tight lips in adults and children: Wash away white crusts with Suan Jiang Shui (Vinegar Plant Water); apply Fu Yan San (Banquet Attendee Powder) before bed. If unresolved, apply Qian Bai Shuang San (Lead White Frost Powder) for cure.
Throat closure with swelling/pain preventing speech in men and women: Prick Shaoshang (LU11) well point at the corner of the thumbnail on both hands. Superficial pricking to bleed cures instantly. If unresolved, gargle with Wen Xue Tang (Warm Blood Decoction) to guide heat out with heat.
Head swelling/pain, scrofula, and chest/hypochondriac lumps/scabs/nodules not resolving, or continuous pus: Fry Cang Yan (Blue Salt) 1–2 liang, decoct in one large bowl of long-flowing water, cool slightly, drink in 3–5 doses. After a while, probe throat with hairpin to induce emesis, expelling 2–3 sheng of cold phlegm. Follow with blood-invigorating and channel-unblocking herbs. The *Neijing* states: "Salty flavor induces upward expulsion and downward drainage, belonging to yin." *Tongren* records: Shaoyang starts at outer canthus, passes behind ear, descends hypochondrium, passes Guomen. Scrofula, tubercles, and horse-knife goiter are diseases of the Foot Shaoyang Gallbladder Meridian, characterized by abundant qi and scarce blood.
Goiter bag distension/fullness: *Yangsheng Lun* states: "Between two mountains near water, people often have goiter; deep soil and water cause goiter." Terrain dictates this. Ren Shen Hua Ying Dan (Ginseng Goiter-Resolving Pill) resolves it. Goiter herbs mostly use Hai Zao (Seaweed) and Dai Zhe (Kelp), salty-cold in flavor.
Initial back carbuncle: Use Cang Yong Wan and Yu Zhu San in large doses to purge bowels 10–20 times. Then, use needle to densely puncture three layers within the red halo around the swelling until blood drains. Wipe with soft cloth. In severe cases, bleed Baihui (GV20) and Weizhong (BL40). Apply Yang Qi San (Yang Qi Powder). Do not immediately take Shi Wei Nei Tuo San (Ten-Ingredient Interior-Supporting Powder) containing Guan Gui (Cassia Bark) decocted in wine. Men's back is yang; adding heat to heat is too hot.
"Penile Abscess" is a misnomer; it is actually Male Blood Hernia, unrecorded in *Nanjing* and *Suwen*. It involves the Jueyin Liver Meridian, the pathway of blood flow. Chong, Ren, and Du meridians are collateral branches of the Liver. *Nanjing* states: "Seven hernias in men." Blood hernia is one. Treat with Dao Shui Wan, Tao Ren Cheng Qi Tang (Peach Kernel Direct Downward Decoction), or Di Dang Tang (Resist It Decoction) as for blood stasis. Accumulation without dispersion requires large-dose purgation (10–20 movements), followed by Yu Zhu San to harmonize blood and unblock channels. People often use Da Huang (Rhubarb) and Mu Li (Oyster Shell); failure indicates ignorance of blood-harmonizing methods.
Chronic genital ulcer ("Sao Gang") treated with Dao Shui Wan and Yu Gong San to purge liver meridian first; apply Mu Xiang San (Costus Powder) topically 2–3 times daily; follow with light rice porridge for 1–2 days.
All chronic malignant sores: Apply Mu Xiang Bing Lang San (Costus and Betel Nut Powder) for cure. Hiccups in men and women, colloquially "Chi Te," result from yin-yang disharmony. Fire desires to ascend but is suppressed by cold; cold cannot overcome fire, causing stagnant sound. Shaoyin cold damage also presents this; Ji Ji San (Already Intersecting Powder) treats all.
