Pediatric cases may involve fright induced by visual exposure to strange objects or auditory perception of unusual sounds, causing the spirit to scatter and qi to become chaotic. The ancient method primarily prescribes Qingre Zhenjing Tang (Clearing Heat and Calming Fright Decoction) [containing Chai Hu (Bupleuri Radix), Mai Dong (Ophiopogonis Tuber), Bo He (Menthae Haplocalycis Herba), Zhi Zi (Gardeniae Fructus), Chuan Huang Lian (Coptidis Rhizoma), Long Dan Cao (Gentianae Radix), Fu Shen (Poria with Hostwood), Gou Teng Gou (Uncariae Ramulus cum Uncis), Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma), Mu Tong (Akebiae Caulis), using Deng Xin Cao (Junci Medullia) and Zhu Ye (Bambusae Folium) as guides], mixed with Cinnabar powder for oral administration. Alternatively, Anshen Zhenjing Wan (Calming Spirit and Pacifying Fright Pill) [containing Tian Zhu Huang (Calamushi Hirtae Spadix), Fu Shen (Poria with Hostwood), Dan Xing (Arisaematis Cum Bile), Zao Ren (Ziziphi Spinosae Semen), Mai Dong (Ophiopogonis Tuber), Chi Shao (Paeoniae Radix Rubra), Dang Gui (Angelicae Sinensis Radix), Bo He (Menthae Haplocalycis Herba), Huang Lian (Coptidis Rhizoma), Chen Sha (Cinnabaris), Niu Huang (Bovis Calculus), Zhi Zi (Gardeniae Fructus), Mu Tong (Akebiae Caulis), Long Gu (Fossilia Ossis Mastodi et Scapulae), Qing Dai (Indicum)] is used. These are ground into fine powder and formed into pills with refined honey. Dosage is adjusted according to the child's age and size, administered with light ginger soup to dissolve the pill.
[Chen Shouzhen's Note] This condition represents true fright in children. Since their qi and blood are not yet fully developed and their spirit is timid, encountering extraordinary fright causes the soul and spirit to tremble, leading to abnormal behavior, nocturnal jumping awakenings, and daytime startle responses. Treatment involves using two qian of Poria with Hostwood processed with Cinnabar, one and a half qian of Ophiopogonis Tuber, two qian of Calamushi Hirtae Spadix, one and a half qian of Rehmanniae Radix Preparata, five fen of Ziziphi Spinosae Semen stir-fried, one qian of Dragon Teeth, and five fen of Glycyrrhizae Radix et Rhizoma. Using ten stems of Junci Medullia as a guide, decocted in water prepared using silver or gold vessels, this formula calms the spirit and stabilizes the soul, thereby resolving the fright naturally.
Some cases arise from excessive fire in the Heart and Liver, compounded by external constraint of wind-cold, preventing free circulation. Symptoms often appear suddenly with high fever, restlessness, red face and lips, phlegm obstruction, rapid breathing, locked jaw, and constipation/urinary difficulty. The ancient method primarily prescribes Zhibao Dan (Precious Treasure Pellet) [containing Ephedrae Herba, Saposhnikoviae Radix, etc., totaling twenty-six ingredients, ground into fine powder and formed into pills with refined honey, each weighing five fen, dissolved in light ginger soup].
Cases caused by excessive phlegm inducing fright are treated with Niuhuang Wan (Cow Bezoar Pill) for purgation [containing Black Qian Niu, White Qian Niu, Arisaematis Cum Bile, Aurantii Fructus Immaturus, Pinelliae Rhizoma, Gleditsiae Spina, Rhei Radix et Rhizoma, etc., ground into powder and formed into honey pills; administration follows the above method].
Cases where extreme heat generates wind are treated with Liangge San (Cooling the Diaphragm Powder) for clearing and resolving [containing Scutellariae Radix, Rhei Radix et Rhizoma, Forsythiae Fructus, Mang Xiao (Natrii Sulfas), Glycyrrhizae Radix et Rhizoma, Gardeniae Fructus, Menthae Haplocalycis Herba, using Bambusae Folium and raw honey for decoction and administration].
For Liver heat, Xieqing Wan (Draining Green Pill) is prescribed [containing Gentianae Radix, Gardeniae Fructus, Rhei Radix et Rhizoma, Notopterygii Rhizoma et Radix, Saposhnikoviae Radix, Chuanxiong Rhizoma, etc., ground into powder, formed into honey pills, taken with Bamboo Leaf and Mint tea].
For phlegm combined with heat, Qingre Huatan Tang (Clearing Heat and Resolving Phlegm Decoction) is used [containing Citri Reticulatae Pericarpium, Ophiopogonis Tuber, Pinelliae Rhizoma, Poria with Red Coir, Scutellariae Radix, Bambusae Concha, Glycyrrhizae Radix et Rhizoma, Coptidis Rhizoma, Aurantii Fructus, Platycodonis Radix, Arisaematis Cum Bile, etc., with Fresh Ginger and Junci Medullia as guides, decocted in water].
For Heart meridian heat, Liangjing Wan (Cooling Fright Pill) is prescribed [containing Gentianae Radix, Saposhnikoviae Radix, Indicum, Uncariae Ramulus cum Uncis, Coptidis Rhizoma, Bovis Calculus, etc., ground into powder, bound with wheat paste into pills, dissolved in water decocted with gold vessels].
[Chen Shouzhen's Note] What is commonly referred to as "Acute Convulsive Wind" (Ji Jing Feng) in popular practice is essentially obstruction by phlegm-fire. Children may generate phlegm due to wind-cold invasion or milk/food retention. Accumulated phlegm transforms into fire, or summer-heat also generates fire. If not cleared promptly, fire rises and phlegm increases. Phlegm-fire obstructs upward, closing the Lung orifices, leading to closure of all orifices, fixed gaze, wheezing, coma, and unconsciousness. Severe fire causes Liver dryness and tendon tension, resulting in various manifestations. The root cause is phlegm-fire stagnation and internal stirring of Liver wind. One should gently support the child with hands, avoiding tight holding which damages meridians and may lead to disability. Initially, administer fresh Lotus Root Juice, Pear Juice, or Water Chestnut Juice slowly. Upon awakening, mild cases use Coptidis Rhizoma three fen, Forsythiae Fructus one qian, Gardeniae Fructus five fen, Calcined Talcum Three qian, Akebiae Caulis six fen, Light Scutellariae Radix eight fen, Small Aurantii Fructus five fen, Trichosanthis Pulvis three qian, Plantaginis Seed eight fen, Raw Glycyrrhizae Radix et Rhizoma three fen, with fresh Acori Tatarinowii Rhizoma one fen as a guide, decocted in flowing water. Severe cases add processed Arisaematis Cum Bile three fen and Calamushi Hirtae Spadix one and a half qian. If heat pathogens block orifices causing deep coma, add half a pill of Ju Fang Zhibao Dan (Official Formulation Precious Treasure Pellet), dissolved before administration. Alternatively, dissolve Zhibao Dan in plain water first, then administer other medicines.
