Wind-cold causing headache and aversion to cold, nasal congestion with heavy voice, sneezing, runny nose but no sweating, this is a sudden contraction of cold pathogenic factors and epidemic qi. It is appropriate to use Ye Su (Perilla Leaf), Bai Zhi (Angelicae Dahuricae Radix), Qian Hu (Peucedani Radix), Sang Pi (Mori Cortex), Jie Geng (Platycodonis Radix), Bo He (Menthae Haplocalycis Herba), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Zhi Ke (Aurantii Fructus Immaturus) and other herbs, employing the Bitter-Pungent-Warm method to release the exterior.
Wind pathogenic factors must be accompanied by cold, injuring the Lung first. The Lung organ is empty, resembling an inverted plate; when eroded by wind-cold, its clear descending function cannot operate normally, and Qi fails to diffuse and transform, thus causing nasal congestion and heavy voice. The symptoms of headache, aversion to cold, and absence of sweating occur because the Taiyin Lung serves as the external defense of the body. Although the presentation of acute infection resembles Shanghan (Cold Damage), the pathology remains within the Lung throughout, without transmission or alteration. Thus, as stated in Neijing (Inner Canon): "If struck at the nape, it descends into the Taiyang channel; if struck on the face, it descends into the Yangming channel; if struck on the cheek, it descends into the Shaoyang channel." In the initial stage, it is appropriate to use Ye Su, Bai Zhi, Bo He, and Qian Hu, utilizing pungent herbs to vent the Wei (Defensive) level pathogens; and using Sang Pi, Ju Hong, Zhi Ke, and Jie Geng, utilizing bitter herbs to descend rebellious Qi. This corresponds precisely to what Neijing states: "The Lung dislikes upward rebellion of Qi, so bitter substances are used to drain it."
Wind-cold presenting with sweating, reduced headache, yet persistent nasal congestion with heavy voice, cough with profuse phlegm, indicates cold pathogenic factors invading the Lung. It is appropriate to use Su Geng (Perillae Caulis), Qian Hu, Xing Ren (Armeniacae Semen Amarum), Xiang Bei (Fritillariae Thunbergii Bulbus), Sang Pi, Di Gu Pi (Lycii Cortex), Ju Pi (Citri Reticulatae Pericarpium), and Pi Pa Ye (Loquat Leaf) and other herbs, employing methods to diffuse Lung Qi and penetrate pathogens.
At this point, wind pathogens have dissipated, but cold remains. If not dispersed, other complications may arise. Therefore, Su Geng, Qian Hu, and Xing Ren are used to descend Qi; Sang Pi and Di Gu Pi are used to clear and drain the Lung; Ju Pi and Xiang Bei are used to resolve phlegm; and Pi Pa Ye is used to clear Qi. Since the pathogen resides in the Upper Jiao (Upper Burner), light and floating herbs are selected to reach the Lung site appropriately.
Wind-cold presenting with heavy and turbid nasal breathing, wheezing with rebellious Qi, hoarse phlegm sounds, and stiff pain in the chest and hypochondrium, indicates binding of cold and fluid retention obstructing the Lung collaterals internally. It is appropriate to use Su Zi (Perillae Fructus), Qian Hu, Sang Pi, Xing Ren, Ju Hong, Ban Xia (Pinelliae Rhizoma), Fu Ling (Poria), Xuan Fu Hua (Inulae Flos), and Pi Pa Ye and other herbs, employing methods to descend Qi and withdraw fluid retention.
Heavy and turbid nasal obstruction, loss of Lung's clear descending function, wheezing, cough, and chest stiffness result from chronic fluid retention binding internally. Fluid retention is a Yin pathogenic factor; it moves due to cold, obstructing Lung collaterals internally and attacking the skin and hair externally. Unless internal fluid retention is withdrawn and external wind-cold is dispersed, how can both be resolved simultaneously? Therefore, it is appropriate to use Su Zi, Qian Hu, Sang Pi, and Xing Ren to open Lung Qi and descend rebellion; Fa Ban Xia (Prepared Pinellia), Fu Ling, Xuan Fu Hua, and Ju Hong to wash away fluid retention and harmonize the Stomach. This allows Lung Qi to open and cold-fluid to transform, leading to gradual resolution of the disease.
