Shengma (Rhizoma Cimicifugae), Shengdihuang (Radix Rehmanniae Recens), Shudihuang (Radix Rehmanniae Praeparata), Dangguishao (Ramulus Angelicae Sinensis), Shenggancao (Radix Glycyrrhizae uralensis), Dahuang (Radix et Rhizoma Rhei Palmati, baked), Taoren (Semen Persicae, pounded into paste), Mahren (Semen Cannabis, each 2 qian), Honghua (Flos Carthami, 0.5 qian).
For the above herbs, except for Taoren and Mahren which are separately pounded into paste, the remaining herbs are cut into pieces the size of hemp seeds. Generally taken as one dose, add two cups of water, include the pastes of Taoren and Mahren, decoct until one cup remains, remove dregs, and take warm on an empty stomach.
Treating constipation, dryness, or complete obstruction due to latent fire in the Spleen and Stomach, accompanied by loss of appetite. This is caused by wind accumulation or blood stasis leading to obstruction; using methods to moisten dryness, harmonize blood, and disperse wind will naturally restore smoothness.
Taoren (Semen Persicae, soaked in hot water, skin and tip removed), Mahren (Semen Cannabis, each 1 liang), Dangguishao (Ramulus Angelicae Sinensis), Dahuang (Radix et Rhizoma Rhei Palmati, baked), Qianghuo (Rhizoma et Radix Notopterygii, each 1 liang).
For the above herbs, except for Taoren and Mahren which are separately pounded into paste, the remaining herbs are ground into a very fine powder, mixed with refined honey to form pills the size of Sophora japonica seeds. Take 30-50 pills per dose, swallowed with plain boiled water on an empty stomach.
If the condition is severe with no bowel movement and涩 (astringency/stagnation), and pathogenic factors are strong, urgently add Jiuxi Dahuang (Alcohol-washed Radix et Rhizoma Rhei Palmati) to promote bowel movement.
If dryness is due to Blood deficiency causing dry stools, add Taoren (Semen Persicae) and Jiuxi Dahuang (Alcohol-washed Radix et Rhizoma Rhei Palmati).
If stagnation is due to Wind accumulation preventing bowel movement, add Mahren (Semen Cannabis) and Dahuang (Radix et Rhizoma Rhei Palmati).
If stagnation is due to Wind-Dampness, add Wei Zaojiaoren (Baked Gleditsia sinensis seed), Dahuang (Radix et Rhizoma Rhei Palmati), and Qinjiao (Radix Gentianae Macrophyllae) to promote bowel movement.
If the pulse is choppy (Se), the body feels itchy, Qi is stagnant, and there is no bowel movement, add Yuliren (Semen Pruni Armeniacae Amarum) and Dahuang (Radix et Rhizoma Rhei Palmati) to resolve Qi dryness.
If the condition is Yin-Cold type, where cold accumulation causes obstruction and no bowel movement, use Banliu Wan (Half-Sulfur Pill) from the Bureau Formulary, or decoct Fuzi (Radix Aconiti Lateralis Preparata) and Ganjiang (Rhizoma Zingiberis) soup, but serve it cold. Although the syndrome is Yin-Cold, Yang-tonifying medicines should be administered. If there is no bowel movement, among ten doses, provide one dose to slightly loosen the bowels without allowing complete blockage; this is the general principle of treatment.
If the patient has a Yin syndrome or Yin-Cold syndrome but presents with restlessness and a firm, solid pulse, a small amount of Bitter-Cold medicine should be added to the Yang medicines to clear heat and calm restlessness. Once restlessness subsides, do not add more.
For those with Yin-restlessness who wish to sit in a well, palpation of the Kidney pulses on both sides will reveal weakness, or a deep, thin, and slow pulse. This is easy to differentiate. Knowing that there is an external pathogenic factor, medication should be adjusted flexibly.
Treating no bowel movement for five days, yellow-red urine, generalized edema especially severe on the face and abdomen, yellowish and numb skin, heaviness like carrying mountains, fatigue and weakness, flaccidity of limbs, frequent vertigo/blacking out, sensation of fire in eyes, cold tearing, anosmia, lower abdominal urgency/pain, pulsation at the umbilicus with hardness and pain upon pressure.
Qingpi (Pericarpium Citri Reticulatae, rind removed), Jiu Huanglian (Rhizoma Coptidis, wine-processed, each 1 fen), Jiu Huangbai (Cortex Phellodendri, wine-processed), Juhong (Exocarpium Citri Reticulatae Rubrum), Zhigancao (Radix Glycyrrhizae uralensis, roasted, half portion), Shengma (Rhizoma Cimicifugae, each 2 fen), Huangqi (Radix Astragali Membranacei), Renshen (Radix Ginseng), Chao Qu (Massa Medicata Fermentata, fried, each 5 fen), Mahuang (Herba Ephedrae, stems not removed, 5 qian), Xingren (Semen Pruni Armeniacae, four pieces).
Cut the herbs into two doses. Add two large cups and a half of water, first decoct Mahuang (Herba Ephedrae) until boiling, skim off foam, then add other herbs and decoct together until one cup remains. Remove dregs, take warm after meals.
This syndrome involves latent Wind-Damp-Heat in the Ying-Blood level. Wood-Fire overacting on the Yang pathways leads to excess above and deficiency below, shortness of Yuan-Qi, and upper asthma due to Yin-Fire damaging Qi. The pathology involves the Kidney Water and represents excessive overcoming. Currently encountering winter cold, seasonal factors attack Liver-Wood, while strengthening its mother organ Lung-Metal, which overcomes Fire and suppresses Wood. This represents extreme excess leading to reactive compensation. Symptoms include fearfulness, frequent yawning/sighing, sneezing, nasal obstruction/anosmia, blurred vision, excessive sadness, amnesia, lower abdominal urgency/pain, generalized jaundice, abdominal distension, severe facial/ocular edema, inability to eat, bloody sputum/phlegm/nasal discharge, eye ulcers, and constipation. All these are suitable for treatment with this prescription.
