Abdominal fullness. The waist feels weak and soft, as if sitting in water. In women, there is pain in the lower abdomen. There is a sensation of tenesmus (urgency with incomplete evacuation) and convulsions/spasms. Menstruation is irregular. Leukorrhea (white discharge) and metrorrhagia (blood-tinged discharge). Pain surrounds the umbilicus on both sides, extending to the lumbar spine, with Qi rushing to the heart and abdomen.
The *Neijing* states: The upper half of the body is governed by Heaven Qi; the lower half is governed by Earth Qi. The middle is the Tianshu point. Tianshu is located at the junction where Heaven and Earth Qi intersect. Regardless of the up-and-down circulation of the twenty-seven meridians throughout the body, they are locked at this pass. Furthermore, there must be a Qi that persists and binds them, firmly maintaining them between the upper and lower parts. Thus, it can gather to become strong and powerful. Therefore, human strength originates from the Lü muscle (back muscles), which is located below the costal margin. This is precisely the Dai Mai (Girdle Vessel). Thus, the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel) transmit to the Qicheng (Street of Qi) acupoint, thereby belonging to the Dai Mai, and connecting with the Du Mai (Governor Vessel) and Taichong Mai. The reason these vessels can nourish the Heart and Lungs upwards also relies on the holding function of the Dai Mai.
When the Dai Mai suffers disease, its symptoms are all downward rather than upward. This is due to deficiency of the Kidney and Liver in the lower part, causing True Yin not to nourish; and stagnation of the Heart and Spleen in the upper part, causing Qi to fail to circulate up and down. Consequently, Dampness stagnates and transforms into Heat, pouring downward into the Small Intestine and Blood Sea. Then the disease arises. Therefore, leukorrhea and blood-tinged discharge result from Heart and Spleen depression/stagnation above and Kidney and Liver Yin deficiency below, with pathogenic Heat lingering, becoming a disease of excessive discharge from the Dai Mai. Abdominal fullness is due to the failure of Middle Jiao Qi to move. The waist feeling weak and soft as if sitting in water is due to deficiency of both Yin and Yang, where the Middle Jiao is weak and lacks the power to stabilize. Pain surrounding the umbilicus and lumbar spine, rushing to the heart and abdomen, is due to Yin Qi invading the lower part. Yang Qi cannot overcome it nor guard the Tianshu point, so Yin Qi invades, leading to Jue-Ni (cold reversal) conditions. Essentially, the key to binding and locking the mechanisms lies entirely in the Dai Mai. If the Dai Mai cannot maintain its own Qi, its symptoms will all sink downward and fail to rise. Treatment has branch (biao) and root (ben) aspects. Methods of ascending/descending and tonifying/reducing lie in seeking the root cause to treat it.
In ancient times, examining the source of diseases, deducing their origins, and accurately assessing them, none surpassed the *Neijing*. Later generations pursued fragmented interpretations, thus obscuring their origins and failing to understand the principles of Yin-Yang and Zang-Fu organs. Now, collecting the texts from the sixteen volumes of the *Neijing*, summarizing scattered references, and combining them into eight chapters on disease conditions, so that later learners may find clear guidance.
