Lochia retention is caused by damage to the Zang-Fu organs from postpartum overexertion, leading to deficiency and depletion of Qi and Blood. It may also result from cold pathogens lingering in the uterine collaterals, or from exposure to wind and cold after childbirth, where wind-cold invades the deficiency and clashes with the blood, causing stagnation and obstruction. The blood accumulates internally and thus fails to discharge. Treatment should focus on warming the channels and promoting blood circulation, allowing the blood to flow freely. In cases where dryness-inducing herbs have been taken, leading to dried-up blood and inability to resolve stagnation, only warming and tonifying Qi and Blood will restore free flow; purgative methods must be avoided as they may induce other diseases.
Postpartum lochia non-stop is caused by injury to the meridians and blood during delivery, resulting in deficiency and insufficiency that prevents consolidation. Alternatively, if lochia is not fully expelled, new blood cannot settle securely. It may also stem from Yin deficiency generating internal heat, which clashes with the blood level, or from lingering cold causing disharmony of Qi and Blood. These conditions require detailed examination of pulse patterns. In cases of extreme deficiency, warming and tonifying to generate new tissue will naturally resolve stasis; in cases of mild deficiency, removing stasis to generate new tissue is appropriate.
For postpartum lochia non-stop: If Liver Qi is hot and fails to generate blood, use Liuwei Wan (Six-Ingredient Pill with Rehmannia); if Liver Qi is deficient and fails to store blood, use Xiaoyao San (Free and Easy Wanderer Powder); if Spleen Qi is deficient and fails to control blood, use Liujunzi Tang (Six-Gentlemen Decoction); if Stomach Qi sinks and fails to command blood, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); if Spleen channel depression generates heat and blood fails to return to its source, use Jiawei Guipi Tang (Modified Heart-Spleen Tonic Decoction); if Liver channel fire anger causes Nutritive blood to run recklessly, use Jiawei Siwu Tang (Modified Four-Substance Decoction); if both Qi and Blood are deficient, use Shiquan Dabu Tang (Ten-Accordance Great Tonifying Decoction); if Liver channel wind pathogen causes blood to boil, use Yiwu Fangfeng Wan (Single Herb Saposhnikovia Pill); if excessive sexual activity or anger injures the Chong and Ren vessels, causing prolonged bleeding, use Liuhuang Dihuang Tang (Six-Ingredient Rehmannia Pill) added with Ejiao (Donkey-Hide Gelatin), Maidong (Ophiopogon Root), and Wuweizi (Schisandra Berry).
The head is the convergence point of all Yang meridians. After childbirth, the Five Zang organs are all deficient, Stomach Qi is weak, and dietary nourishment is insufficient, causing deficient Yang to lose its anchor and rush upward to the head. This results in an excess of Yang and deficiency of Yin, leading to headache. Occasionally, headaches are caused by retained blood (stasis), which is essentially turbid Qi residing in the upper body. Even if symptoms such as fever and aversion to cold appear, only Shenghua Tang (Generate the New Decoction) with modifications should be used; one must strictly avoid using Qianghuo (Notopterygium Rhizome) and Duhuo (Angelica Pubescens). This condition arises from the loss of True Yang, allowing Turbid Yin to invade the upper regions and sink into the Marrow Sea, causing distension and pain. Without the restoration of Clear Yang ascending, Turbid Yin cannot descend. This is a disease caused by internal pathogens, unlike external pathogens which can be resolved through diaphoresis.
