方见咳嗽门中，埭头王姓湿气郁肺之方，加兜铃蝉衣芦根。
殷左（三十三岁，十一月） 久嗽曾失血，肺失清肃，声嗄不扬，良由水不涵木，木火刑
金使然也。脉象弦数，治宜清金平木。
方见咳嗽门史姓之方，去燕根、旋复花，加冬花、凤凰衣等分。
杨左（五月） 金水双亏，喉痹声嗄，所谓金破则无声也，脉形弦数，治宜滋阴降火。
方见咳嗽门史姓方，去苏子、梨膏，加射干、金果榄、猪肺露、青芦根。
吴 肺痈已成，咳吐脓血，气逆痰稠，右胁引痛，脉右弦滑兼数，治宜清肃上中。

年芥菜卤 白杏仁 炙冬花 银花露 冬瓜煎汤代水
如初起去川贝紫苑款冬芦梗芥菜卤，加桃仁苦桔梗川郁金全栝蒌芦根。
肺痿宜从金水双亏法。

某患者：突发剧烈咳嗽导致失音，且有气逆症状，这是因为肺被寒邪郁闭所致。
三拗汤加桔梗、白前。
宋姓患者（二月就诊）：去年夏天暑风侵袭肺部，未能及时清热解表，导致肺热蒸腾津液成痰，痰阻肺气，咳嗽至今未愈，声音沉重浑浊不清亮，这就是所谓的“金实则无声”（肺气壅实则声音不出），脉象弦滑而濡。治疗应当清肃上焦和中焦。
方剂见咳嗽门中，埭头王姓患者湿气郁肺的方子，加紫菀、蝉蜕、芦根。
殷姓左侧患者（33岁，十一月就诊）：长期咳嗽曾有过咯血，肺失去清肃功能，声音嘶哑不清亮，这主要是由于肾水不足不能涵养肝木，导致肝火刑克肺金所致。脉象弦数，治疗应当清肺金、平肝木。
方剂见咳嗽门中史姓患者的方子，去掉燕根、旋复花，加款冬花、凤凰衣各等分。
杨姓左侧患者（五月就诊）：肺金与肾水双亏，喉痹导致声音嘶哑，这就是所谓的“金破则无声”（肺体受损则声音不出），脉形弦数，治疗应当滋阴降火。
方剂见咳嗽门中史姓患者的方子，去掉苏子、梨膏，加射干、金果榄、猪肺露、青芦根。
吴姓患者：肺痈已经形成，咳嗽吐脓血，气逆痰稠，右胁部牵引疼痛，右脉弦滑兼数，治疗应当清肃上焦和中焦。
生薏苡仁 真川贝母 连翘 青芦梗 甜瓜仁（如果没有可用冬瓜子代替） 炙紫菀 经霜桑叶 陈年芥菜卤 白杏仁 炙款冬花 银花露 冬瓜煎汤代水。
如果是初起阶段，去掉川贝、紫菀、款冬、芦梗、芥菜卤，加桃仁、苦桔梗、川郁金、全瓜蒌、芦根。
肺痿应当从肺肾双亏的方法论治。
Shen, left side (February): Wind-phlegm disturbs the Lung, causing failure of Lung Qi to diffuse and descend. The Lung is the gate of voice; there is coughing with hoarseness and indistinct voice. The pulse is wiry and rapid. Treatment should aim to clear and purify the Upper and Middle Jiao.
Scrophulariae Radix (Xuanshen), Armeniacae Semen Amarum (Xingren), Thunbergiae Caulis (Tongcao), Cicadae Periostracum (Jinchan Yi), Fried Loranthi Fructus (Chao Doulou), Inulae Flos (Xuanfu Hua), Fritillariae Cirrhosae Bulbus (Xiangbei), Fresh Phragmitis Rhizoma (Huoshui Lugeng), Fried Bursae Fructus (Chao Niuxiang), Univalve Concha (Sheng Geqiao), Fresh Bambusae Descendens Succus (Xian Zhuru).
Certain patient: Sudden severe cough with aphonia, accompanied by rebellious Qi. This is due to Lung constraint by Cold.
San'ao Decoction (Three Unrestrained Decoction) plus Platycodonis Radix (Jiegeng) and Cynanchi Stauntonii Caulis (Baiqian).
Song (February): Last summer, Summer-heat and Wind pathogens invaded the Lung, failing to be cleared and resolved in time. Lung heat steams fluids into phlegm, phlegm obstructs Lung Qi, resulting in persistent coughing. The voice is heavy, turbid, and indistinct. This is what is referred to as "Metal being solid results in no sound" (loss of voice due to excess pathogen in the Lung). The pulse is wiry, slippery, and soggy. Treatment should aim to clear and purify the Upper and Middle Jiao.

Yin, left side (33 years old, November): Long-term cough previously accompanied by hemoptysis. The Lung has lost its clarity and descending function, leading to hoarseness and indistinct voice. This is primarily due to Kidney Water failing to nourish Liver Wood, causing Liver Fire to attack Metal (Wood-Fire punishing Metal). The pulse is wiry and rapid. Treatment should aim to clear Metal and calm Wood (clear Lung and pacify Liver).

Yang, left side (May): Dual deficiency of Lung Metal and Kidney Water. Pharyngitis with hoarseness. This is what is referred to as "Metal broken results in no sound" (loss of voice due to Lung tissue damage). The pulse form is wiry and rapid. Treatment should aim to nourish Yin and descend Fire.

Wu: Lung abscess (Fei Yong) has formed. Coughing up pus and blood, rebellious Qi, thick phlegm, pain radiating from the right hypochondrium. The right pulse is wiry, slippery, and rapid. Treatment should aim to clear and purify the Upper and Middle Jiao.
Coicis Semen (Sheng Yiren), Sichuan Fritillaria Bulb (Zhen Chuanbei), Forsythiae Fructus (Lianqiao), Green Phragmitis Node (Qing Lugeng), Benincasae Semen (Tian Guaren; if unavailable, replace with Benincasae Semen Dongguazi), Honey-fried Asteris Radix (Zhi Ziwan), Mulberry Leaf after Frost (Jingshuang Sangye), Aged Mustard Seed Brine (Chennian Jiecailu), White Armeniacae Semen (Bai Xingren), Honey-fried Adenophorae Radix (Zhi Donghua), Lonicerae Flos Dew (Yinhua Lu), Winter Melon decocted water substitute.
If in the initial stage, remove Chuanbei, Ziwan, Donghua, Lugeng, and Jiecailu; add Persicae Semen (Taoren), Platycodonis Radix (Kujiegeng), Curcumae Radix (Chuan Yujin), Trichosanthis Fructus (Quangualu), and Phragmitis Rhizoma (Lugen).
Lung flaccidity (Fei Wei) should be treated according to the method for dual deficiency of Lung and Kidney.
