The symptom of aphasia (loss of speech) may arise from Heart disease where the Heart Qi cannot ascend to connect with the tongue, or from Spleen disease where the Spleen Qi cannot move the root of the tongue, or from Kidney disease where the Kidney Qi cannot ascend to connect with the Heart. Although the causes of illness differ, the sites of pathology generally do not extend beyond these three meridians. Postpartum aphasia is mostly caused by the failure of communication between the Heart and Kidney, along with deficiency of Qi and Blood. Even if there are minor pathogenic factors, they are all due to insufficiency of Yuan Qi (Original Qi). The ancient formula Qizhen San (Seven Treasures Powder) is indicated for this condition. If accompanied by overthinking injuring the Spleen, manifesting as fatigue and reduced appetite, Guipi Tang (Restore the Spleen Decoction) should be added. If accompanied by dual deficiency of Qi and Blood, manifesting as internal heat and afternoon fever (Bū Rè), Bazhen Tang (Eight Treasures Decoction) should be added. If accompanied by Spleen deficiency generating Phlegm, manifesting as poor appetite, nausea, and vomiting, Liujunzi Tang (Six Gentlemen Decoction) should be added. If accompanied by Kidney Qi deficiency and cold, manifesting as cold extremities and bi-syndrome pain, Dihuang Yinzi (Rehmannia Infusion) should be added. If accompanied by Kidney Water deficiency with flaming Fire, manifesting as internal heat and red face, Liuwei Dihuang Tang (Six-Ingredient Pill with Rehmannia) should be added. With such regulation and treatment, gradual recovery should naturally occur. If one recklessly employs methods to dispel Wind and attack Phlegm, the approach would be far off the mark.
Ren Shen (Ginseng Radix), Shichangpu (Acori Tatarinowii Rhizoma), Sheng Di Huang (Rehmanniae Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma) (each 1 liang); Fang Feng (Saposhnikoviae Radix), Chen Sha (Cinnabar) (processed separately, levigated with water, each 0.5 liang); Xi Xin (Asari Radix et Rhizoma) (1 qian). Grind into powder. Take 2 qian per dose, mixed with Bohe Tang (Mint Decoction).

If postpartum fever occurs suddenly without exposure to Wind-Cold, it is due to Blood deficiency. Siwu Tang (Four Substances Decoction) should be used to tonify Yin and Blood, combined with Hei Ganjiang (Blackened Ginger Rhizome) for its bitter-warm nature to treat according to the syndrome (Congruent Treatment), gathering the floating and scattered Yang, allowing it to return and root in the Yin, thus the fever will subside. If there is no response, add Tongzi Xiaoye (Child's Urine) as a guiding agent, and efficacy is assured. However, many postpartum fevers are caused by Spleen deficiency and Food Stagnation; mistaking this for Blood deficiency will result in ineffectiveness. The method should be to regulate diet and harmonize the Spleen and Stomach, using Wuyi Yigong San (Five Agents Yi Gong Powder) added with Shenqu (Medicated Leaven) and Maiya (Hordei Fructus Germinatus). Generally, Wind-Cold fever persists day and night; whereas Blood deficiency and Food Stagnation fevers occur in the late afternoon and resolve in the early morning, making these two syndromes similar. However, Food Stagnation must present with acid regurgitation, belching of putrid food, and fullness/stuffiness in the chest and diaphragm, which are clearly distinguishable. In contrast, Blood deficiency lacks these specific symptoms. Furthermore, postpartum patients may suffer from severe deficiency of both Qi and Blood, presenting with aversion to cold, fever, irritability, and thirst, which is a critical syndrome of Yang dissipating with Yin. Shiquan Dabu Tang (Ten Great Tonifying Decoction) is indicated; if ineffective, add Fuzi (Aconiti Lateralis Radix Praeparata). If vomiting, diarrhea, poor appetite, abdominal pain, and a deep-thin pulse or floating-large but forceless pulse occur, Lizhong Tang (Regulate the Middle Decoction) should be added. These are all signs of false heat due to deficiency-cold. Mistaking them for excess Fire and treating with clearing/cooling methods can lead to disaster instantly.
