Pain in the lower back is caused by coldness of Kidney Water leading to stagnation of Liver Wood. The Kidneys reside in the lumbar region; when Kidney Water is cold, Spleen Earth becomes damp. If the cause is cold and the pattern is deficiency, Guizhi Jiang Ling Ejiao Tang (Cinnamon Twig, Ginger, Poria, and Donkey-hide Gelatin Decoction) is the primary formula. If caused by external contraction of Wind-Cold, it should be treated according to the methods for Cold Damage diseases, which is not covered in this example.
