When bladder heat transfers to the small intestine, it causes obstruction in the intestines and difficulty in defecation; the heat rises upward, resulting in mouth soreness and ulceration (kou mi). (The full text is seen in the section on Diagnosis of Disease Transmission.)
[Li Gao] Yi Re Tang (Decoction for Transferring Heat): Treats kou mi. The *Huangdi Neijing* states: "When bladder heat transfers to the small intestine, causing intestinal obstruction and difficult defecation, it manifests as kou mi." People who enjoy drinking alcohol often suffer from this disease. Zhang Yuansu used Wuling San (Five-Ingredient Powder with Poria) combined with Dao Chi San (Guide Out the Red Powder), which proved miraculous.
[Zhu Zhenheng] Treatment for widespread white ulceration in the mouth. Bi Bo (Long Pepper) 1 liang, Nong Huang Bo (Thick Phellodendron Bark) 1 liang 6 qian, ground into powder. Decoct with rice vinegar for a few boils, then mix with the above medicine. When saliva flows out, spit it out, then rinse the mouth with water to heal. Severe cases may require a second treatment.
[Luo Tianyi] Hu Huang Lian San (Coptis and Picrorhiza Powder): Treats kou mi.
Hu Huang Lian (Picrorhizae Rhizoma) 5 fen, Huo Xiang (Pogostemonis Herba) 1 qian, Xi Xin (Asari Radix) 3 qian, Huang Lian (Coptidis Rhizoma) 3 qian.
Grind the above into powder. Use half a qian each time, apply dryly inside the mouth, rinse and spit out.
Bi Xiao San (Surely Effective Powder): Treats kou mi.
Bai Fan (Alumen) and Da Huang (Rhei Radix et Rhizoma) in equal parts.
Grind the above into fine powder. Apply dryly before sleep until all saliva drains, then rinse with warm water.
According to the theory of Six Qi transformations: Kou mi all belongs to heat. The classics state: "In the recurrence of Shaoyin, fire qi erupts internally and rises upward to cause kou mi," and the treatment uses bitter-cold herbs, which is correct.
[Jisheng Fang] Jiajian Ganlu Yin (Modified Sweet Dew Decoction): Treats men, women, and children with guest-heat in the stomach, halitosis, gingival recession (ya xuan), red eyes, mouth sores, and all types of sore pain whether resolved or unresolved. (Zhu Zhenheng said: "Ganlu Yin is a medicine for the heart, lungs, and stomach.")
Shu Di Huang (Prepared Rehmanniae Radix), Sheng Di Huang (Rehmanniae Radix), Tian Men Dong (Asparagi Radix, dehearted), Huang Qin (Scutellariae Radix), Pi Pa Ye (Loquat Leaf, de-hairy), Shan Yin Chen (Artemisiae Scopariae Herba), Zhi Qiao (Aurantii Fructus), Jin Cha Shi Hu (Dendrobii Nobile Caulis), Gan Cao (Licorice Root, roasted) (each 1 liang), Xi Jiao (Rhino Horn) 3 qian.
Grind the above into powder. Take 2 qian each time, add one cup of water, decoct to seven-tenths, remove dregs, and take warm after meals and before bed. For children, divide one dose into two servings, adjusting according to circumstances.
This prescription was obtained from a high-ranking official's family. The use of Xi Jiao (Rhino Horn) in this formula is very reasonable and effective every time. A servant in my clan suffered from gingival recession and halitosis, with teeth gradually loosening and falling out. I gave him two doses, and he recovered instantly. No one failed to recover using this prescription. It was not recorded in the first collection of *Jisheng Fang*. Because this prescription was hard to obtain, I dare not disclose it lightly; it has miraculous effects when taken.
[Jisheng Fang] Treatment for halitosis. Xiang Ru (Moslae Herba) one handful, decoct in ten dou of water to obtain three sheng. Slowly hold in the mouth and swallow/spit. (Zhu Zhenheng said: "Only Xiang Ru Tang can treat halitosis.")
[Luo Tianyi] Case Report on Lung Heat Causing Foul Breath: Liang Jimin consumed rich food and alcohol, and due to excessive mental labor, his lung qi was injured, resulting in foul-smelling breath, thick sticky sputum, throat discomfort, bitter taste, and dry mouth. Treated primarily with Jiajian Xie Bai San (Modified White-Draining Powder).
Sang Bai Pi (Mori Cortex) 3 qian, Di Gu Pi (Lycii Cortex) 1.5 qian, Gan Cao (Licorice Root, roasted) 1.5 qian, Zhi Mu (Anemarrhenae Rhizoma) 0.7 qian, Huang Qin (Scutellariae Radix) 0.5 qian, Wu Wei Zi (Schisandrae Fructus) 21 grains.
Mai Men Dong (Ophiopogonis Radix) 0.5 qian, Jie Geng (Platycodonis Radix) 2 qian.
The *Nanjing* states: "The Heart governs the five odors; when entering the Lungs, it becomes fishy/foul odor." This is one instance. Due to injury from excessive consumption of hot qi, Heart Fire insults Lung Metal. Sang Bai Pi and Di Gu Pi are bitter and slightly cold; they descend latent fire in the Lungs and tonify qi, serving as the Sovereign (Jun) herbs. Huang Qin and Zhi Mu are bitter and cold; they treat foul odor and clear/benefit Lung qi, serving as the Minister (Chen) herbs. The Lung desires consolidation; urgently eat sour to consolidate it. Wu Wei Zi is sour and warm, consolidating Lung qi. Mai Men Dong is bitter and cold, treating thick sticky sputum, bitter taste, and dry mouth, serving as the Assistant (Zuo) herb. Jie Geng is pungent and warm, light and floating; it treats rebellious phlegm and benefits the throat/pharynx, serving as the Envoy (Shi) herb.
Chop the above herbs together as one dose. Add two cups of water, decoct to one cup, remove dregs, and take warm after meals. Twice a day. Avoid alcohol, damp dough, and pungent-hot foods.
*Shengji Zonglu* Treatment for halitosis, also treating tooth swelling and pain. Decoct Xi Xin (Asari Radix) to obtain a strong juice. Drink hot and induce vomiting; the disease will heal.
[Zhang Congzheng] A man from the Shang family, over twenty years old, suffered from breath smelling like a toilet, such that even relatives refused to speak with him. Dai Ren (Zhang Congzheng's courtesy name) said: "Metal of the Lung naturally governs fishy odor. If Metal is overwhelmed by Fire, and Fire governs stench, it should be like this. Over time, it becomes putrid; putrefaction belongs to the Kidney. This is extreme excess manifesting as Water transformation. Since the disease is in the upper jiao, emesis is indicated. Use Cha Tiao San to induce emesis and remove seven-tenths of the pathogen. At night, use Zhou Che Wan and Jun Chuan San to purge five to seven movements. By morning, the odor ceased."
*Zhenjiu Jiayi Jing*: Mouth swelling and foul odor are governed by Laogong (PC8).
