伤寒咳嗽气逆上冲,脉象散乱的为死证,这是因为形体亏损的缘故。咳嗽且小便失禁的,不可发汗,发汗则会导致四肢厥冷。(论见“遗尿”条。)

If the exterior syndrome of Shanghan (Cold Damage) is not resolved, and there is fluid retention in the epigastrium, presenting with dry retching, fever, and cough, or accompanied by thirst, diarrhea, dysphagia, difficulty in urination with fullness in the lower abdomen, or wheezing, Xiao Qing Long Tang (Minor Blue Dragon Decoction) is indicated.
Mahuang (Ephedrae Herba), Shaoyao (Paeoniae Radix Alba) (each 3 qian), Wuweizi (Schisandrae Fructus) (0.5 liang), Ganjiang (Zingiberis Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Xixin (Asari Radix et Rhizoma), Guizhi (Cinnamomi Ramulus) (bark removed), Banxia (Pinelliae Tuber) (each 3 qian).
Crush the herbs. Take 5 qian per dose, add 1.5 cups of water, decoct until reduced to 0.8 cup, strain, and take warm. If there is mild diarrhea, remove Mahuang and add Yuanhua (Genkwa Flos) the size of a bullet, stir-fried until red. If there is thirst, remove Banxia and add Gualougen (Trichosanthis Radix) 3 fen. If there is dysphagia, remove Mahuang and add Fuzi (Aconiti Lateralis Radix Praeparata) 1.5 qian, processed. If there is difficulty in urination and fullness in the lower abdomen, remove Mahuang and add Fuling (Poria) 1 liang. If there is wheezing, remove Mahuang and add Xingren (Armeniacae Semen Amarum) 3 fen, skin and tips removed.

(See commentary under "Alternating Chills and Fever".)
For cough where the exterior syndrome has resolved and the stomach is excess, it belongs to Yangming disease. If there is dizziness, no aversion to cold, and ability to eat, throat pain will certainly occur. (See commentary under "Stomach Excess".)
For cough with only desire to sleep, it belongs to Shaoyin disease. If accompanied by diarrhea, abdominal pain, difficulty in urination, and heavy/painful limbs, use Zhen Wu Tang (True Warrior Decoction) added with Wuweizi, Xixin, and Ganjiang. If diarrhea persists for six to seven days and transforms into cough, vomiting, thirst, and inability to sleep, use Zhu Ling Tang (Polyporus Decoction). If diarrhea is accompanied by delirious speech and difficult urination, it is due to injury from fire therapy. If there are cold extremities, use Si Ni San (Four Counterflow Powder) added with Wuweizi and Ganjiang. (See commentary under "Desire to Sleep".)
In Shanghan, if cough and rebellious qi rise, and the pulse is scattered, death ensues; this is because the physical form is depleted. For cough with urinary incontinence, sweating therapy is contraindicated; if sweat occurs, cold extremities will result. (See commentary under "Urinary Incontinence".)
