A man over fifty years old suffered from cold damage (Shanghan) with coughing, and his throat made sounds like snoring. After taking one dose of Dushen Tang (Ginseng Decoction), the snoring sound ceased; after two or three more doses, the cough gradually subsided; only after consuming two to three jin (approx. 1-1.5 kg) of medicine did the illness fully resolve.
[Mei] To treat chronic Xieke (cough with wheezing sound) where the throat makes noise preventing sleep, take Baiqian (Cymbopterygium Root) pounded into powder, mixed with warm wine for a dose of two qianbi (approx. 6-9 grams). ([Bencao Yan Yi / Elucidation of Materia Medica] states: Baiqian stabilizes Lung Qi.)
[Zhang Zhongjing] For cough with upward rushing of Qi, and a sound in the throat resembling that of a water chicken (frog), Shegan Mahuang Tang (Thoroughwort and Ephedra Decoction) is the primary treatment.
Shegan (3 liang) Mahuang (Ephedrae Herba) Shengjiang (Fresh Ginger Root) (each 4 liang) Xixin (Asari Radix et Rhizoma) Ziwan (Asteris Radix) Kuandonghua (Farfarae Flos) (each 3 liang) Dazao (Jujubae Fructus) (7 pieces) Wuweizi (Schisandrae Fructus) Banxia (Pinelliae Rhizoma) (each half sheng)
The above nine ingredients are decocted in 12 sheng of water. First, boil Mahuang until it boils twice, skim off the foam on the surface, then add the remaining herbs. Boil until 3 sheng of liquid remains. Divide into three warm doses.
[Qianjin Fang] Baiqian Tang: Treats rebellious Qi rising with cough, bodily edema, shortness of breath and abdominal fullness, inability to lie down day or night even while leaning against a wall, and a sound in the throat like a water chicken.
Baiqian Ziwan Banxia Daji (Euphorbiae Pekinensis Radix) (each 3 liang)
The herbs are cut into small pieces, soaked in 10 sheng of water overnight, and boiled the next morning until 3 sheng of liquid remains. Divide into three doses.
[Su Wen / Basic Questions] Yin contends within, Yang disturbs without; Pohan (sweat associated with the Po soul) is not stored; four extremities become cold (Sini). When this cold rises, it steams the Lungs, causing panting and wheezing. (From "Treatise on Differentiating Yin and Yang") If daily activities remain normal but respiration produces sound, this indicates reversal of the collateral vessels of the Lung. The collateral Qi cannot follow the meridians up and down, thus stagnating in the main channels without flowing. Since the pathology of the collaterals is slight, daily activities remain normal despite respiratory sounds. (From "Treatise on Reversed Regulation") When the Du Mai (Governor Vessel) is diseased, there is upward rushing Qi with sound; treat the point in the center of the throat located in the depression of the collarbone. (This is Tiantu [CV-22]). When the pathological Qi rushes upward to the throat, treat Jian; Jian is located above, flanking the chin. (Wang Bing’s commentary identifies this as Tianrong [SI-17]).
[Jia Yi Jing / Acupuncture and Moxibustion Classic] For cough, throat鸣 (wheezing/ringing), and spitting blood, Da Zhong (KI-4) is indicated. For chest and hypochondriac distension and fullness, inability to bend forward or backward, Yongke (painful cough) with rebellious Qi, and hoarse sounds in the throat and pharynx, Tai Xi (KI-3) is indicated. For rebellious Qi with cough, and hoarse panting in the throat, Fu Tu (ST-12) is indicated.
