The *Huangdi Neijing* states: When examining eye pain, if the red vessels extend from top to bottom, it is a Taiyang disease; if they extend from bottom to top, it is a Yangming disease; if they move from outside to inside, it is a Shaoyang disease. Based on this theory, distinguishing between superficial and deep nebulae (opacities) is clear. Applying this to treat symptoms is as effective as striking a drum with a drumstick.
For all cases of red vessel nebulae, those initially developing from top to bottom belong to the Taiyang channel. Since the Taiyang channel governs the exterior, the symptoms must involve pain in the supraorbital ridge, or pain in the brain and nape, or swelling and pain in half of the head. The treatment method should be warming and dispersing. For the warming method, use equal parts of aged tea (Lachá) and salted Chuan Fuzi (Prepared Aconite Root), decocted and taken orally for immediate recovery. Dai Yuanli once treated this syndrome using one qian of Chuan Fuzi as a single dose, resulting in immediate recovery. For the dispersing method, formulas such as Xiaku San (Prunella Decoction) from the *Jianyao Jizhong Fang* or Xuanqi Tang (Select Extraordinary Decoction) by Li Dongyuan are suitable. One formula consists of Banliang (half tael) of Fuzi (Aconite Root), a large pinch of sprouted tea, one qian of Baizhi (Angelicae Dahuricae Radix), and half a qian each of Xixin (Asari Radix), Chuanxiong (Chuanxiong Rhizome), Fangfeng (Saposhnikoviae Radix), Qianghuo (Notopterygii Rhizoma et Radix), and Jingjie (Schizonepetae Herba). Decoct and take orally for miraculous effect.
[From *Jianyao Jizhong Fang*] Xiakucao (Herba Prunellae) treats supraorbital ridge pain and nebulae growing from top to bottom, with repeated efficacy. It must be taken concurrently with Tuiyun Wan (Cloud-Removing Pill). The Xiakucao formula is found under the entry for Eye Ball Pain.
[Li Dongyuan] Xuanqi Tang (Select Extraordinary Decoction) treats supraorbital ridge pain and nebulae growing from top to bottom with excellent results. (Formula see entry for Brow Pain.)
Qianghuo Chu Yi Tang (Notopterygium Nebula-Removing Decoction) treats Sun-Taiyang cold-water qi, where nebulae obscure the eyes, preventing vision.

For red vessel nebulae, those initially developing from bottom to top, or from inside to outside, all belong to the Yangming channel. Since the Yangming channel governs the interior, symptoms often include heat or constipation. The treatment method should involve purging and cooling.
For purging, formulas such as Liuqi Yin (Flowing Qi Decoction) from the *Ju Fang*, Xieqing Wan (Draining Green Pill) by Qian Yi, or Wenbai Wan (Warming White Pill) from the *Ju Fang*, added with Huanglian (Rhizoma Coptidis - Coptis Rhizome) and Huangbai (Phellodendron Bark), have proven effective repeatedly. For cooling, formulas like Huanglian Yanggan Wan (Coptis Pig Liver Pill) are suitable.

[Zhang Yuansu/Haizang] Mr. Li Dongyuan treated wind-heat toxins after pox/rashes, where nebulae and qi halos obscured vision, using Xieqing Wan (Draining Green Pill) for great effect; it is easy to treat when first noticed. (*Baoming Ji* states: Pox-induced nebulae can also be treated; use half the amount of Dahuang in Xieqing Wan.)
The wife's niece was obese. At the age of fifteen, she developed an eye disease, recurring every month or two. During attacks, her eyes were red, swollen, gritty, and painful to open. This persisted for three years. After taking wind-dispelling and heat-clearing agents, and common eye drops, a stubborn nebula formed in the left eye, spreading from the outer canthus to cover the pupil. The right eye also had a nebula growing from bottom to top. The classic states: Moving from inside/outside is Shaoyang disease; moving from bottom to top is Yangming disease. I believed this indicated stagnation in the Shaoyang and Yangming channels. The pulse was short, slippery, and solid, appearing shorter in the morning. Jiegu Lao said: A short pulse indicates stagnation suppressing the zang-fu organs; purging is appropriate. Thus, I used Wenbai Wan (Warming White Pill), reducing Chuanxiong and Fuzi by two-thirds, and adding more Longdan (Gentian Root) and Huanglian (Coptis Rhizome), following Li Dongyuan's "Five Accumulations" method. Starting from two pills, I increased by one pill daily until severe diarrhea occurred, then reduced the dosage. Restarting from two pills, suddenly one day, black clots of blood, hard and ink-like, were passed during diarrhea. From then on, gradual recovery occurred, and the nebula completely disappeared.
[Popular] Huanglian Yanggan Wan (Coptis Pig Liver Pill) treats nebulae growing from bottom to top or inside to outside with miraculous effects. (Formula see Internal Obstruction entry.)
For red vessel nebulae initially developing from outside to inside, it belongs to the Shaoyang channel. Since Shaoyang governs the half-exterior and half-interior, the treatment method should be harmonizing, such as with Shenxian Tuiyun Wan (Immortal Cloud-Removing Pill).
[Li Dongyuan] Shenxian Tuiyun Wan (Immortal Cloud-Removing Pill) treats all types of nebulae, halos, internal/external obstructions, and dimness without clarity, with repeated efficacy. It is a wonderful formula.

Grind in a wooden mortar into powder. Take 2 qian mixed with tea water, once after breakfast and once before bed. Treats sudden red eyes. Avoid damp wheat and alcohol. Chushi must be authentic; if unavailable, use leaves. Otherwise, the other herbs are ineffective. Do not bake during preparation, and avoid iron vessels. I observe that this formula uses Chuye but not Chushi, likely matching yin and yang. Chushi is yang; using leaves (without fruit) is yin. Therefore, without true Chushi, it is ineffective.
[Luo Tianyi] Wuxiu Chongming Wan (Five Fragrances Double Clarity Pill) treats eye nebulae obscuring vision, grittiness, and dimness. Regular use clears and benefits the head and eyes.
Ganjuhua (Flos Chrysanthemi Indici - Chrysanthemum Flower, opened): 500 flowers

Grind into fine powder. Make pills with refined honey, size of a bullet. Take one pill per dose, chew thoroughly, swallow frequently. Can be held in mouth after meals anytime. Strictly avoid alcohol, wheat, and hot foods before bed. (The above two formulas are suitable for those without heat.)
Nebulae occur when wind-heat is heavy, or foreign bodies enter the eye. This is liver qi flourishing and manifesting on the exterior. Once nebulae form, their superficial nature is clear; they should be dispersed and removed. If instead one uses draining/purgative methods, pathogenic qi retreats inward, making the nebula deeper. Pathogenic qi that is unsettled is called "Heat Nebula" and floats. Pathogenic qi that has settled is called "Ice Nebula" and sinks. Pathogenic qi that is firm and deep is called "Sunken Nebula." One should use dispersing agents to agitate the pathogenic qi again so the nebula floats, then assist with nebula-removing drugs to allow it to disappear naturally. Chronic cases cannot be cured quickly; removal requires months or years.
For newly formed nebulae, use surface-dispersing formulas like Li Dongyuan's Qianghuo Chu Yi Tang. For those with hot blood and deficiency, use Tuiyun Wan (Cloud-Removing Pill) type formulas.
[From *Baoming Ji*] For Ice Nebulae persisting long-term, use Lingyangjiao San (Antelope Horn Powder).
Lingyangjiao (Cornu Saigae Tataricae - Saiga Antelope Horn)

Xixin (Radix Asari - Asarum Root): Equal parts
Gancao (Radix Glycyrrhizae - Licorice Root): 0.5 qian
Grind into powder. Half is made into pills with refined honey. Take 50-70 pills per dose. Use the other half as powder, swallow pills with rice water after meals.
To treat Sunken Nebulae, also use Lingyangjiao San type formulas, adjusting as needed. For yin deficiency with heat, concurrently take Shenxian Tuiyun Wan (Immortal Cloud-Removing Pill).
[Li Dongyuan] Buyang Tang (Tonify Yang Decoction) treats Yang failing to overcome Yin, i.e., Yin excess and Yang deficiency, leading to blockage of the nine orifices. Qing-white nebulae appear at the outer canthus. This is depression/stagnation in the Foot-Taiyang and Foot-Shaoyin channels, preventing Foot-Jueyin Liver channel qi from ascending to the eyes, thus causing internal obstruction of Qing-white nebulae. One should benefit Liver Yang qi from below the Nine Origins in the Taiyang and Shaoyin channels, allowing it to rush upward to Heaven. First tonify Yang, then drain Yin-fire of the Foot-Jueyin Liver channel at the Biao points of Foot-Taiyang and Foot-Shaoyin. This is the secondary treatment. *Neijing* states: For Yin excess and Yang deficiency, first tonify Yang, then drain Yin. This is the correct method. Take Buyang Tang daily in the early morning on an empty stomach. Take Xiayin Wan (Drain Yin Pill) before bed. Do not take if weather changes, severe cold/wind, overwork, previous day's dietary irregularity, mental fatigue, or weak qi. Wait for physical balance and normal weather. Then take to tonify Yang, allowing Yang qi to ascend, reach the extremities of the Liver channel, and clear the orifices of the eyes.
