[From *Shanghan Ji*] For hypochondriac pain caused by cold damage, treat with Zhigong (TE6) and Yanglingquan (GB34).
If there is bitter taste in the mouth, dryness of the throat, accompanied by deafness and fullness in the chest, it belongs to Shaoyang disease (disease of the Shaoyang channel). Vomiting and purging are contraindicated; Chaihu Tang (Bupleurum Decoction) is appropriate. (See discussion under "Bitter Taste".)
In combined Taiyang and Yangming disease, if there is wheezing and fullness in the chest, Mahuang Tang (Ephedra Decoction) is appropriate. (See discussion under "Wheezing".)
In Shao Yin disease, if there is diarrhea, sore throat, accompanied by fullness in the chest and vexation, Zhufu Tang (Pig Skin Decoction) is appropriate. (See formula discussion under "Diarrhea".)
After inappropriate purging in Taiyang disease, if the pulse is rapid and there is fullness in the chest, Guizhi Qushaoyao Tang (Cinnamon Twig Decoction without Paeonia) is indicated. (See formula discussion under "Aversion to Cold"). If the pulse is rapid but there is no Jie Xiong (knotted chest syndrome), it indicates the pathogen is resolving. (See discussion under "General Principles".)
After purging, if there is fullness in the chest and difficulty in urination, accompanied by vexation, palpitations, delirium, heaviness of the body, and inability to turn over, Chaihu Jia Longgu Muli Tang (Bupleurum Decoction with Dragon Bone and Oyster Shell) is indicated. (See formula discussion under "Palpitations"). If accompanied by hiccups and a thick tongue coating, it is Shi Bi (Dampness Bi syndrome). (See discussion under "Body Pain and Dampness Bi"). After sweating and purging, if there is vexation-heat and a sensation of obstruction in the chest, Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) is the primary treatment. (See discussion under "Vexation").
If symptoms include fever and aversion to cold, or spontaneous sweating associated with Guizhi Tang pattern, but without headache or stiffness of the neck, with a hard and obstructive sensation in the chest and Qi rushing up to the throat causing breathing difficulties, Gouti San (Melon Seed Powder) is the primary treatment. (See discussion under "Rushing Upward Qi").
With hypochondriac pain, deafness, and both Cun (inch) and Chi (cubit) pulses appearing wiry, it indicates disease of the Shaoyang channel. (See discussion under "General Principles". *Renwu Shu* states: Treat with Chaihu Tang.) When pathogens form various excess patterns in the chest, resulting in depression and pain in the chest, inability to eat, a desire for someone to press on the chest, yet vomiting saliva instead, along with more than ten episodes of diarrhea, a slow pulse, and a slightly slippery Cun Kou (radial artery at the wrist) pulse, this condition can be treated with emesis; the diarrhea will cease after vomiting.
After emesis and purging, if there is a feeling of nausea, chest pain, loose stools, abdominal fullness, and vexation, Tiaowei Chengqi Tang (Regulating the Stomach and Directing Downward Decoction) is appropriate. (See discussion under "Vomiting").
With hypochondriac pain, deafness, and both Cun and Chi pulses appearing wiry, it indicates disease of the Shaoyang channel. (See discussion under "General Principles"). With bitter taste, deafness, and fullness in the chest, it is Shaoyang Wind Strike. (See discussion under "Bitter Taste"). Before taking the pulse, if the patient necessarily covers their chest with their hands, and upon the physician's instruction to cough, they do not cough, this indicates bilateral deafness. The reason for this is excessive sweating leading to deficiency.
