[Tang] When a child suffers from abdominal pain, bends the waist, cries dryly without tears, has a cyan and pale complexion, black lips, and cold limbs, this is termed "Coiling Intestine Internal Hanging" (Panchang Neidiao). For any such presentation, immediately decoct green onion soup to wash and rub the abdomen, and apply moxibustion or hot compress with green onions to the umbilicus and abdomen. After some time, urine will naturally discharge amidst the pain, and the pain will cease immediately. Subsequently, administer oral medication.
Frankincense (Ruxiang) and Myrrh (Moyao) (take a small amount of transparent pieces of each, grind finely).
Take a piece of Costus Root (Muxiang), grind it in a mortar, add one fen of water, boil for several boils, and mix with the powders of Frankincense and Myrrh.
This medicine takes effect after a single dose.
[Qian] Gouteng Paste: Treats severe abdominal pain in children, dry crying followed by lying supine, named "Coiling Intestine Internal Hanging".
Frankincense (ground), Myrrh (ground), Costus Root (Muxiang), Turmeric Rhizome (Jianghuang) (each 4 qian), Semen Momordicae (Mubiezi Ren) (21 pieces).
First, grind the latter three ingredients into a fine powder, then incorporate the first two ingredients. Mix with refined honey to form a paste, and store. For each year of age, administer half the size of a soap pod; adjust the dosage according to circumstances.
Administer dissolved in Gouteng Decoction, at any time. Follow up with Weixiang San.
Weixiang San
Curcuma Zedoaria Rhizome (Pengzhu) (0.5 liang), Asafoetida (Awei) (1 qian).
First dissolve Asafoetida in warm water, soak Curcuma Zedoaria Rhizome for a day and night, dry it, and grind into powder. Take one zi or half a qian per dose, decoct Perilla Leaf (Zisu) and rice drink, and take mixed on an empty stomach.
[Wu] Suanru Wan: Treats night crying due to abdominal pain from cold syndrome.
Garlic (one clove, slow-roasted until fragrant and cooked, mashed), Frankincense (separately ground, 5 fen).
Grind together and make pills the size of mustard seeds. Take seven pills per dose, delivered with breast milk.
The above four formulas treat cold-type abdominal pain belonging to "Coiling Intestine Internal Hanging," characterized by bending the waist and dry crying.
[Qian] Stomach deficiency-cold presents with a sallow-white complexion, abdominal pain, and loss of appetite; tonify the spleen, primarily using Yihuang San. If there is no diarrhea, use Tiaozhong Wan. (Yihuang San treats painful diarrhea; Tiaozhong Wan treats pain without diarrhea.)
Tiaozhong Wan
Atractylodes Macrocephala Rhizome (Baizhu), Ginseng Radix (Renshen), Licorice Root (Gancao) (stir-baked, each 0.5 liang), Dried Ginger (Ganjiang) (processed, 4 qian).
Grind into fine powder, mix with honey to make pills the size of mung beans. Take five to seven pills up to fifteen pills before meals, with warm water. (Haicang says: This follows Zhongjing's Lizhong principle.)
Danggui San: Whenever children cry at night, it is due to internal cold causing abdominal pain, cyan complexion, cold hands, and refusal to suckle; this formula is suitable. It is effective.
Angelica Sinensis Radix (Danggui) (remove rhizome), White Peony Root (Baishao), Ginseng Radix (Renshen) (each 1 qian), Licorice Root (Gancao) (roasted, 5 fen), Platycodon Root (Jiegeng), Tangerine Peel (Chenpi) (each 1 qian).
Cut into pieces, decoct to obtain half a sheng of liquid, take small amounts frequently until cured.
The above three formulas treat cold-type abdominal pain due to stomach deficiency, characterized by loss of appetite.
Heat-type pain also causes incessant crying, occurring at night, with red face and scorched lips, and dark yellow urine; administer Sanhuang Wan, taken down with Renshen Tang. (Sanhuang Wan formula see Heat Fever methods.)
[Dan] Abdominal pain with night crying. Administer Niuhuang Wan the size of a bean, emulsified and fed; write the character "Tian" below the umbilicus, and relief is immediate. (From *Shenghui Fang*)
[Qian] Accumulation pain presents with warm breath, sallow-white complexion, lackluster eyes, excessive whites of the eyes, lethargy, aversion to food, or sour-smelling stool; eliminate accumulation, preferably using Xiaoji Wan. For severe cases, use Baibing Zi to purge. Afterwards, harmonize the stomach using Baizhu San. (Xiaoji Wan, Baibing Zi formulas see Pi masses. Baizhu San formula see Xiaoke.)
[Dan] Food stagnation causing abdominal pain and hardness. Must use Perilla Seed (Zisu) and Radish Seeds (Luobozhi) etc.
[Shi] Children who like eating zongzi may develop abdominal pain. Use Coptis Rhizome (Huanglian) and Fermented Rice Ball (Baijiuyao) in equal parts, make into pills and take.
[Tang] Sanleng San: Treats abdominal pain due to qi accumulation.
Amomum Fruit (Sharen), Licorice Root (Gancao), Alpinia Oxyphylla Fruit (Yizhi) (stir-baked, remove shell), Burreed Tuber (Sanleng), Curcuma Zedoaria Rhizome (Pengzhu), Green Tangerine Peel (Qingpi) (stir-baked, equal parts).
Grind into powder, take with plain boiled water.
Worm pain presents with a sallow-white complexion, heart and abdominal pain. Saliva and clear fluid flow from the mouth, pain occurs at fixed times; treat primarily with Anchong San. Children with innate timidity often suffer from this.
(Tian says: Worm pain involves crying while bending and straightening, restlessness in sitting and lying, self-pressing of the heart and abdomen, frequent loud crying, abnormal facial color, either cyan or yellow, white lips. Also lackluster eyes and spitting saliva; this is worm pain.)
[Qian] Anchong San: Treats pediatric worm pain.
Lead Carbonate (Hufen) (stir-baked yellow), Chrysanthemum Carancas Seed (Heshi) (stir-baked yellow), Toosendan Fruit (Chuanlianzi) (remove peel and seed), Alum (Bai fan) (calcined, 2.5 qian).
Grind into fine powder. Take one zi per dose, or five fen for older children, mixed with rice drink, taken during pain.
[Tang] Treating moving worms with unbearable pain. Use Dry Resin of Toxicodendron (Ganqi) 0.5 liang, Areca Seed (Binglang) one piece (raw), Yaolao one piece, grind finely again. Mix the three items into powder, take with hot wine on an empty stomach. After a while, expel the worms, and cure immediately. (Yaolao is likely mud objects burned in kilns.)
Another prescription: Use Dry Resin of Toxicodendron (Ganqi) one liang, crush, stir-fry until smoke ceases. Mix with fresh well water and raw sesame oil, take on an empty stomach.
[Qian] Anchong Wan: Treats deficiency of upper and middle jiao, or stomach cold causing worm movement and pain.
Dry Resin of Toxicendron (Ganqi) (stir-fry until smoke ceases, 2 fen), Realgar (Xionghuang) (1 fen), Croton Seed Frost (Badou Shuang) (1 qian).
Grind into fine powder, make pills with paste the size of millet seeds. Dosage depends on the child's size. Take down with decoction of East-growing Pomegranate Root, or Vitex Root, or Xuyi Tang, five to seven pills up to twenty to thirty pills, taken when pain occurs.
Xuyi San: Main indications same as above. Whenever children cry loudly during pain, and the philtrum, nose tip, lips, and mouth turn black simultaneously, pulse diagnosis should be deep, weak, and wiry; now the pulse is large instead, indicating worm syndrome.
White Xuyi (Bai Wuyi), Dry Resin of Toxicodendron (Ganqi) (stir-baked, equal parts).
Grind into fine powder. Take one zi or five fen, or one qian, mixed with rice drink, taken when pain occurs.
[Dan] Ascariasis. Use Vitex Root (Kuliu Gen) as the chief herb, assisted by Erchen Tang decoction.
[Qian] Accumulation pain, food pain, and deficiency pain are largely similar. Only worm pain presents with bland taste and spontaneous saliva flow; treat according to symptoms. Worm pain resembles epilepsy; because children are innately timid, stomach deficiency-cold causes worm movement and heart pain, slightly resembling epilepsy, but without eye deviation or hand convulsions; treat primarily with Anchong San.
Ms. Xin's five-year-old daughter suffered from worm pain. Various physicians treated her with Croton Seed (Badou), Dry Resin of Toxicodendron (Ganqi), Nardostachys (Naxiao) etc., without effect. By the fifth day, she cried much, bent and straightened, slept restlessly, pressed her heart and abdomen herself, cried loudly at times, had varying facial colors (cyan, yellow, white, black), lackluster and slow eyes, white lips, and spitting saliva. On the sixth day, her chest was elevated and she lay restlessly. Qian Yi was summoned and examined carefully. After three doses of Xuyi San, the cyan color in the eyes remained. Qian Yi was greatly alarmed, saying: "This disease is critical; if diarrhea ensues, it is a adverse sign." The next day, Ms. Xin told Qian Yi: "Indeed, there was diarrhea at midnight." Qian Yi looked into the chamber pot, seeing liquid like medicine juice, stirred it with a stick, and saw pill fragments. Qian Yi said: "This child has thick flesh, implying robust Qi, yet the presentation is deficient; it is untreatable." Ms. Xin asked: "Why?" Qian Yi said: "Spleen deficiency and stomach cold cause worm movement; now the eyes are cyan, indicating Liver overacting on Spleen. Plus the diarrhea indicates extreme Qi deficiency. The pills passing with feces indicate Spleen and Stomach collapse. Combined with the mismatch between form and disease, I know it is fatal." Five days later, the condition became comatose, and she died on the seventh day.
