[Haizang] When a Shanghan (Cold Damage) disease occurs during the years governed by Taiyang or Taiyin Si Tian (the annual governing Qi in Five Movements and Six Qi theory), if purgation is used excessively, it often transforms into Yin Jaundice. There are two scenarios: first, Cold Water Qi is excessive, with Water attacking Earth; second, Earth Qi is deficient, allowing Water to invade. Both scenarios frequently lead to this disease. The first scenario is treated with Yinchen Fuling Tang (Cinnamon Twig and Poria Decoction) added with Danggui (Angelica Sinensis) and Guizhi (Cinnamon Twig); the second with Yichen Jupi Tang (Inula and Tangerine Peel Decoction) added with Shengjiang (Fresh Ginger), Baizhu (Atractylodes Macrocephala), and Banxia (Pinellia); the third with Yichen Fuzi Tang (Inula and Aconite Decoction); the fourth with Yichen Sini Tang (Inula and Reversus Aconite Decoction); the fifth with Yichen Jiangfu Tang (Inula, Ginger, and Aconite Decoction); and the sixth with Yichen Wuzhuyu Tang (Inula and Evodia Decoction). The formulas are listed below. For jaundice with difficult urination, irritability, and thirst, treat with Yinchen Tang (Inula Decoction) added with Fuling (Poria), Zhuling (Polyporus), Huashi (Talcum), Danggui (Angelica Sinensis), and Guongui (Cassia Bark). (Han Shi calls this Yinchen Ling Tang.) For jaundice with irritability, wheezing, vomiting, but no thirst, treat with Yinchen Tang added with Chenpi (Tangerine Peel), Baizhu (Atractylodes Macrocephala), Shengjiang (Fresh Ginger), Banxia (Pinellia), and Fuling (Poria). (Han Shi calls this Yinchen Chenpi Tang.) For jaundice with coldness throughout the four limbs and body, treat with Yinchen Tang added with Fuzi (Prepared Aconite Lateral Root) and Gancao (Licorice). (Han Shi calls this Yinchen Fuzi Tang.) For jaundice with cold extremities and spontaneous sweating above the waist, treat with Yinchen Tang added with Fuzi (Prepared Aconite Lateral Root), Ganjiang (Dried Ginger), and Gancao (Licorice). (Han Shi calls this Yinchen Jiangfu Tang.) For jaundice with incessant cold sweat, treat with Yinchen Tang added with Fuzi (Prepared Aconite Lateral Root) and Ganjiang (Dried Ginger). (Han Shi calls this Yinchen Fuzi Tang.) For jaundice where previous treatments with Ginger and Aconite have failed and the pulse remains slow, treat with Yinchen Tang added with Wuzhuyu (Evodia), Fuzi (Prepared Aconite Lateral Root), Ganjiang (Dried Ginger), Mutong (Akebia Stem), and Danggui (Angelica Sinensis). (Han Shi calls this Yinchen Zhuyu Tang.) Zhao Zongyan developed jaundice due to excessive purgation, presenting with a deep, fine, slow, and forceless pulse. Medications were administered sequentially, achieving significant effect with Yinchen Fuzi Tang.
Regarding Haizang's sequential medication method, it refers to first administering Han Shi's Yinchen Fuling Tang, then Yinchen Chenpi Tang, and subsequently Yinchen Fuzi Tang. Later, Scholar Zhao followed this sequence. Scholar Zhao developed jaundice due to premature purgation, presenting with a faint pulse at the Cun position and weak pulse at the Chi position, along with cold body. Sequential medication was applied, achieving significant effect with Yinchen Sini Tang. For injury from cold and internal cold, with weak pulse and Qi deficiency transforming into Yin Jaundice, Zhang Zhongjing prescribed Lizhong Tang (Regulate the Middle Decoction) added with Yinchen (Artemisia Capillaris).
Yinchen Tang treats Shanghan-induced jaundice, characterized by yellowing of the eyes and red urine.
Ingredients: Yinchen (Artemisiae Scopariae Herba), Shanzhi (Gardeniae Fructus), Chaihu (Bupleuri Radix), Huangbai (Phellodendri Cortex, honey-fried), Huangqin (Scutellariae Radix), Shengma (Cimicifugae Rhizoma), Longdancao (Gentianae Radix) [each half liang], Dahuang (Rhei Radix et Rhizoma, stir-fried) [one liang].
Prepare by chopping, decoct in water, and take on an empty stomach. Stop when bowel movement becomes loose.
[Haizang] For alternating chills and fever with whole-body yellowing, treat with Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Zhizi (Gardeniae Fructus).
[From "Yanyi"] A monk suffered from Shanghan, but sweating was incomplete, leaving residual heat. His body and face turned yellow, accompanied by frequent heat, lasting a year without cure. Physicians treated it as dietary stagnation jaundice, which was incorrect, and the disease persisted. Upon inquiry, it was found that his diet had not decreased. He was then given this prescription. After five days, the illness reduced by one-third; after ten days, by two-thirds; after twenty days, the illness completely disappeared. The formula uses Yinchen (Artemisiae Scopariae Herba) and Zhizi (Gardeniae Fructus) each three qian, Qinjiao (Gentianae Macrophyllae Radix) and Shengma (Cimicifugae Rhizoma) each four qian. Powdered, take three qian per dose, decoct in four he of water until two he remain, strain, and take warm after meals. Stop when harmony is achieved.
[From "Huoren Shu"] Wuling San added with Yinchen Hao Tang treats jaundice with thirst and difficult urination.
Yinchen Hao Tang (ten parts) Wuling San (five parts).
Mix the two powders well. Take three qian per dose, three times daily, mixed with water.
Another prescription: For Shanghan about to develop jaundice, urgently use Goudi powder (Thlapsidis Semen). Hold water in the mouth, insufflate a small amount (one zi) into the nose to discharge yellow fluid. This is very effective. Immediately follow with Yinchen Tang mixed with Wuling San taken internally, which is most effective.
[From "Bencao"] To treat damp-heat jaundice originating in the head and Gallbladder, use Goudi San (Thlapsidis Powder).
Ingredients: Goudi (Thlapsidis Semen) [twenty seeds], Chixiaodou (Vigna Angularis Seed), Shumi (Panicum Miliaceum Grain) [each fourteen grains].
Grind into fine powder. Take a grain-sized amount (like a soybean), insert into the nose to inhale. Yellow fluid should discharge. Do not blow the powder into the nose.
[From "Yunqizi"] Jie Xiong (Chest Binding) with jaundice originates from Taiyang affecting the interior. (Treat according to Jie Xiong methods.) Pi Qi (Stagnant Mass) with jaundice originates from Taiyin affecting the interior. (Treat according to Pi Qi methods.)
[From "Huoren Shu"] Question: Baihu Syndrome also presents with body heat, irritability, thirst with desire to drink, and difficult urination. Why does it not cause jaundice? Answer: Baihu Syndrome is similar to Jaundice Syndrome. Generalized sweating indicates Heat dispersing outward, which is Baihu Syndrome. Sweating only on the head and face, with no sweating below the neck, indicates Jaundice Syndrome.
[Cheng Wuji] Shanghan jaundice indicates the disease has reached its extreme, and many cases are incurable. It is not only when the pulse is absent near the radial styloid process at the Cun position, or when breath from the nose is cold, that it is considered incurable. Furthermore, if the body appears smoked/dark, with fixed staring and head shaking, this indicates Heart Qi exhaustion. If the area around the mouth is dark/black, with soft/sweaty skin and yellowing, this indicates Spleen Qi exhaustion. These are all incurable conditions.
