[General Principles]
After a woman has just given birth, one should not merely attribute her condition to deficiency (Qi and Blood deficiency); it is likely that blood stasis is stagnating internally and obstructing the flow of Qi. If tonifying agents are prescribed in such cases, nourishing Yin is more appropriate than nourishing Yang.
During the period approaching delivery, to prepare for preventing dystocia and promoting labor, it is necessary to harmonize Qi and Blood. The recommended formula is Baochan Wuyou San (Safety During Childbirth Without Worries Powder).
Bai Shao (Radix Paeoniae Alba, 2 qian) Huang Qi (Radix Astragali, 8 fen) Tu Si Zi (Semen Cuscutae, 1 qian) Dang Gui (Radix Angelicae Sinensis, 1.5 qian) Hou Po (Cortex Magnoliae Officinalis, 7 fen) Ai Ye (Folium Artemisiae Argyi, 7 fen) Jing Jie Sui (Herba Schizonepetae Spica, 8 fen) Zhi Qiao (Fructus Aurantii, 6 fen) Chuan Xiong (Rhizoma Chuanxiong, 1.5 qian) Bei Mu (Bulbus Fritillariae Cirrhosae, 1 qian) Qiang Huo (Rhizoma Notopterygii, 5 fen) Gan Cao (Radix Glycyrrhizae Uralensis, 5 fen) Sheng Jiang (Rhizoma Zingiberis Recens, 1 slice)
The verse states:
When preparing drugs to induce labor,
Dang Gui, Chuan Xiong, Bai Shao, and Bai Zhu ensure safety without worry.
Add Tu Si Zi and Hou Po along with Qiang Huo and Zhi Qiao;
Sheng Jiang, Bei Mu, Gan Cao, Jing Jie, and Ai Ye accompany them.
(Commentary by Bo'an: This formula is extremely mild in nature but possesses significant efficacy.)
In cases of difficult labor where the fetal head cannot descend due to the immobility of the pubic symphysis (Jiao Gu), it is caused by inactive Qi and Blood. The recommended formula is Jiawei Foshou San (Modified Buddha's Hand Powder).
Dang Gui (Radix Angelicae Sinensis, 5 qian) Chuan Xiong (Rhizoma Chuanxiong, 3 qian) Gui Ban (Carapax et Plastrum Testudinis, 8 qian) Xu Yu Tan (Sanguis Carbonized, 3 qian)
The verse states:
Do not panic when inducing labor at term;
Dang Gui, Chuan Xiong, Gui Ban, and Xu Yu Tan are excellent.
The formula named Foshou is truly miraculous and effective,
Ensuring safe delivery without injury to the mother's body.
(Commentary by Bo'an: In cases where the fetus is delivered but the placenta remains retained, urgently decoct 2 liang of Dang Gui washed with wine, 5 qian of Chuan Xiong, 1 liang each of Yi Mu Cao (Herba Leonuri), Ru Xiang (Resina Draconis), Mo Yao (Resina Myrrhae), and 3 qian of Hei Jing Jie Sui (Carbonized Herba Schizonepetae Spica), then administer orally. However, if an obstetric hospital is available, sending the patient to the hospital for treatment is the most appropriate course of action.)
At the beginning of the postpartum period, symptoms such as abdominal pain and fainting are caused by blood stasis rushing upward and attacking the heart. The recommended formula is Jiawei Shixiao San (Modified Shixiao Powder).
Dang Gui (Radix Angelicae Sinensis, 3 qian) Chuan Xiong (Rhizoma Chuanxiong, 2 qian) Ling Zhi (Faeces Trogopterori, 2 qian) Pu Huang (Pollen Typhae, 1.5 qian) Tong Bian (Urine of a Healthy Young Boy, 1 cup)
The verse states:
At the beginning of new childbirth, blood stasis must be expelled;
Dang Gui, Chuan Xiong, and Ling Zhi must be decocted urgently.
Add Pu Huang and Tong Bian;
Do not be alarmed by pain, fainting, irritability, or confusion.
After the postpartum blood stasis has been completely expelled, if various deficiencies and damages appear simultaneously, it indicates a severe depletion of Qi and Blood. The recommended formula is Shiquan Dabu Tang (Ten Ingredients Great Tonifying Decoction).
Sheng Di Huang (Radix Rehmanniae Preparata, 3 qian) Bai Shao (Radix Paeoniae Alba, 2 qian) Chuan Xiong (Rhizoma Chuanxiong, 1 qian) Dang Shen (Radix Codonopsis, 3 qian) Bai Zhu (Rhizoma Atractylodis Macrocephalae, 3 qian) Fu Ling (Sclerotium Poriae cocos, 3 qian) Dang Gui (Radix Angelicae Sinensis, 2 qian) Gan Cao (Radix Glycyrrhizae Uralensis, 8 fen) Rou Gui (Cortex Cinnamomi, 3 fen) Huang Qi (Radix Astragali, 3 qian)
The verse states:
After childbirth, once blood stasis is gone and Qi and Blood are depleted,
Dang Gui, Chuan Xiong, Sheng Di, Bai Shao, Bai Zhu, and Ren Shen treat the condition.
Fu Ling and Gan Cao combined with Rou Gui,
Regulate the spirit and prevent physical fatigue.
