[Note] Regarding eye diseases, internal obstructions (Neizhang) manifest as dim vision without swelling or pain, originating internally due to the disturbance of the Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright) affecting the viscera, damaging the Heart and Kidney through overexertion. External obstructions (Waizhang) manifest as redness, swelling, and pain, with clear vision not being dimmed, originating from the invasion of the Six Exogenous Pathogens (Wind, Cold, Summerheat, Dampness, Dryness, Fire), where heat accumulates in the meridians. Therefore, internal obstructions are mostly deficiency patterns, while external obstructions are mostly excess patterns. Zhang Hezi stated: "The eyes do not become diseased without fire." This statement does not only refer to internal obstructions but is primarily made regarding external obstructions. For external obstructions characterized by redness, swelling, and pain, one may either disperse exogenous pathogens, purge internal heat, or address both simultaneously, leading to immediate recovery. However, in cases where wind-fire attacks upward, lingers without dispersing, condenses into nebulae, and obscures clarity, neither simple dispersion nor purging can achieve immediate cure. The formula Xidao San can both attack wind-heat and remove nebulae, excelling in both aspects. The formula uses the full dosage of Fangfeng Tongsheng San, targeting wind-heat primarily. Doubling the dosage of Guiwei and Chishao follows the principle that treating wind requires treating the blood first; when blood circulates, wind extinguishes itself. Adding Qianghuo, Duohuo, Manjingzi, and doubling Fangfeng aims to dispel wind specifically at the Taiyang exterior level. Since the interior counterpart of the Taiyang meridian is the Shaoyin meridian, Xixin is added to directly penetrate the Shaoyin. Adding Yuanshen helps to descend and calm kidney fire, addressing the interior while treating the exterior. Adding Muzei, Chantui, Caojueming, Baijili, and Juhua serves to assist the group of wind-dispelling and heat-clearing herbs in eliminating the nebula formed by the sudden stagnation of wind-heat.
