Treats malaria caused by phlegm accumulation, where there are no exterior (Taiyang) or interior (Yangming) syndromes.
Qingpi (Exocarpium Citri Reticulatae), Houpo (Cortex Magnoliae Officinalis), Caoguo (Fructus Tsaoko), Banxia (Rhizoma Pinelliae Chienpiao), Chaihu (Radix Bupleuri), Baizhu (Rhizoma Atractylodis Macrocephalae), Gancao (Radix Glycyrrhizae), Fuling (Poria), Huangqin (Radix Scutellariae). Decoct in water and take orally.
[Note] Malaria belongs to Shaoyang disease (disease of the half-exterior and half-interior). If accompanied by Taiyang exterior syndrome (aversion to cold, fever, stiff neck), treat with Mahuang Guizhi Gemi Tang (Ephedra and Cinnamon Twig Half-Dose Decoction) via sweating method; if accompanied by Yangming interior syndrome (high fever, profuse sweating, great thirst, constipation), treat with Daxiehu Tang (Major Bupleurum Decoction) via purgation method; if not accompanied by exterior or interior syndromes, or if it recurs after sweating or purgation, treat according to the harmonizing method for Shaoyang. This formula combines Xiaochaihu Tang (Minor Bupleurum Decoction) and Sijunzi Tang (Four Gentlemen Decoction) to clear Shaoyang heat while protecting the Spleen, hence named "Qingpi Yin" (Clear Spleen Drink). Ginseng is omitted because Qi is not deficient; Caoguo and Houpo, which have strong aromas and flavors, are added to eliminate gastric accumulation; Qingpi is added, assisting Fuling and Banxia, to break the root of phlegm. Ancient sages said: "No phlegm, no malaria; no accumulation, no malaria." This decoction exemplifies this principle. For patients with Qi deficiency, add Ginseng; for those with excess Qi, add Binglang (Areca Seed); for those with excessive heat, add Shigao (Gypsum Fibrosum); for those with profuse sweating, add Guizhi (Cinnamon Twig). These adjustments should be made based on clinical judgment.
Treats dysentery characterized by bloody and white pus-like stool, tenesmus, and urgent pain.
Shaoyao (Radix Paeoniae Alba), 2 liang; Danggui (Radix Angelicae Sinensis), 5 qian; Huanglian (Rhizoma Coptidis), 5 qian; Huangqin (Radix Scutellariae), 5 qian; Binglang (Semen Arecae), 3 qian; Muxiang (Radix Aucklandiae), 3 qian; Gancao (Radix Glycyrrhizae), 3 qian. Take 0.5 liang per dose, decoct in water and drink. If dysentery does not subside, add Dahuang (Radix et Rhizoma Rhei).
[Note] Dysentery arises in summer and autumn, either due to external damp-heat and summerheat pathogens or internal injury from raw and cold foods, leading to fermentation of dampness and heat. Initially, the pathology is in the Fu organs (intestines), which are easier to treat; later, it transmits to the Zang organs, which are difficult to treat. What are Fu organ diseases? Disease in the Large Intestine transforms according to Metal, so the stool color is white; disease in the Small Intestine transforms according to Fire, so the stool color is red. Therefore, red dysentery often presents as Jin Kou Li (anorexic dysentery/cannot eat), because the Small Intestine is close to the Stomach, allowing turbid qi to attack upward, causing vomiting and inability to eat. What are Zang organ diseases? Transmission to the Heart results in unceasing heat and bloody watery stool; transmission to the Kidney results in unremitting dysentery like a leaking roof; transmission to the Spleen results in inability to ingest fluids, hiccups, and anorexia. This decoction treats early-stage disease located in the Fu organs. Danggui and Baishao (White Peony Root) are used to regulate Blood; Muxiang and Binglang are used to regulate Qi. When Blood is harmonized, pus and blood can be eliminated; when Qi is regulated, tenesmus will cease naturally. Huangqin and Huanglian dry dampness and clear heat. Gancao harmonizes the Middle Jiao and moderates the properties of other herbs. If tenesmus and pain are severe, or if dysentery does not subside after taking the medicine, add Dahuang. This applies the therapeutic principle of "treating obstruction with draining methods" (using purgatives to treat diarrhea/dysentery).
