因为脾虚无法运化,所以食欲不振;
脾主四肢肌肉,所以感到疲倦乏力、嗜睡懒动;
痰多的患者,适宜使用枳桔二陈汤加入苍术、白术,以祛除湿气、化解痰浊;
再加上外感风邪,内伤生冷饮食,导致寒邪凝结而成病;
厥阴经属于肝脏,肝性急迫,所以会出现牵引睾丸和少腹部的绞痛;
大抵而言,热证多表现为松弛纵缓,寒证多表现为疼痛;病在血分者位置固定不移,病在气分者位置游走多动;
Damp-Phlegm (Shi Tan) syndrome manifests as spleen deficiency with dampness leading to poor appetite, fatigue and lethargy, a desire to lie down, and a sallow complexion. If phlegm is abundant, Zhijie Erchen Tang (Bitter Orange and Platycodon Two-Chen Decoction) is indicated; if fluid retention (Yin) is prominent, Guiling Gan Zhu Tang (Poria, Cinnamon Twig, Licorice, and Atractylodes Macrocephala Decoction) is indicated.
[Note]
Damp-Phlegm arises because children overconsume raw, cold, or greasy foods, damaging the Spleen and Stomach. This leads to Spleen Earth becoming deficient and damp, failing to transform and transport fluids, thereby forming Damp-Phlegm, which is slippery and easily expectorated. Due to Spleen deficiency and failure of transportation, there is an aversion to food; since the Spleen governs the four limbs, there is fatigue and a tendency to sleep; as the Spleen corresponds to the Element of Earth and the color Yellow, the complexion is often yellow. For patients with abundant phlegm, Zhijie Erchen Tang added with Cangzhu (Atractylodis Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) should be used to dispel dampness and resolve phlegm. For those with severe fluid retention, Guiling Gan Zhu Tang must be used to support Yang and dissipate fluids; with appropriate regulation, the phlegm will naturally resolve.
(Zhijie Erchen Tang) Formula see previous section on "Breast".
(Guiling Gan Zhu Tang) Ingredients: Poria (Fuling), Cinnamon Twig (Guizhi), Raw Licorice Root (Sheng Gancao), Stir-fried Atractylodis Macrocephalae Rhizoma (Baizhu). Administration: Decoct with fresh Ginger (Shengjiang) as a guide.
(Verse for Memorization)
Guiling Gan Zhu treats damp-phlegm and fluid retention, dispelling dampness, promoting urination, and supporting Yang.
The formula contains Poria, Cinnamon Twig, and Raw Licorice; Atractylodis Macrocephalae Rhizoma is stir-fried with soil, using Fresh Ginger as a guide.
Various Hernia (Shan) syndromes belong to diseases of the Jueyin Liver Meridian and the Ren Mai (Conception Vessel). Externally, it is caused by the aggregation and coagulation of Wind-Cold pathogens; internally, it is due to Damp-Heat being constrained by Cold. The symptoms all involve traction pain in the testicles and abdomen. Fetal Hernia (Tai Shan) is mostly due to congenital insufficiency. Generally, one must examine whether it is Heat or Cold: Heat tends to cause flaccidity and relaxation, while Cold causes pain; involvement of the Blood level results in fixed pain, while involvement of the Qi level results in moving pain; Dampness causes heavy bearing-down sensation, while Deficiency causes light bearing-down sensation.
[Note]
The Jueyin Liver Meridian circles the external genitalia and enters the lower abdomen. The Ren Mai originates below the Zhongji point (CV3), ascends above the pubic hair line, and follows the interior of the abdomen up to the Guanyuan point (CV4); therefore, all hernia syndromes are attributed to these meridians. Children suffering from this condition often have congenital insufficiency and inherent weakness of the Zang-Fu organs. Additionally, they may contract external Wind pathogens or suffer internal injury from raw/cold foods, causing Cold pathogens to coagulate. Alternatively, Damp-Heat may stagnate in the Middle Jiao and be bound externally by Cold pathogens; the pathogenic factors take advantage of the deficiency to enter the Blood level and flow into the Jueyin Meridian. Since Jueyin belongs to the Liver, and the Liver's nature is swift and urgent, this causes traction pain in the testicles and colicky pain in the lower abdomen. There is also a syndrome called "Fetal Hernia," mostly caused by excessive weeping and sadness during pregnancy, disturbing Yin Qi, which knots and fails to disperse, resulting in the infant developing this condition immediately after birth. Broadly speaking, Heat presents with flaccidity, Cold with pain; pathology in the Blood level is fixed, pathology in the Qi level moves; swelling and bearing-down due to Dampness feel heavy, while those due to Deficiency feel light. Treatment based on the specific syndrome will accurately address the disease.
