Heart fire scorches the Lung metal, combined with wind-dampness toxicity, causing faint rashes to appear on the skin. This condition is treated initially with Qianghuo San (Notopterygium Decoction) to disperse wind and dampness, followed by Xiaodu Yin (Detoxification Decoction) to completely eliminate residual heat.
[Note]
Yinzhen (Urticaria/Rash), also known as "hidden rash," arises from heart fire scorching the Lung metal, compounded by external contraction of wind-dampness. It invariably presents with intense itching and a red color, appearing faintly within the skin, hence the name "Yinzhen." Initially, Jiajian Qianghuo San (Modified Notopterygium Decoction) is used to disperse wind and dissipate dampness; subsequently, Jiawei Xiaodu Yin (Additive Detoxification Decoction) is employed to clear heat and resolve toxicity. Once both the exterior and interior are cleared, the rash resolves.
(Jiawei Qianghuo San / Modified Notopterygium Decoction): Composition includes: Qianghuo (Rhizoma et Radix Notopterygii), Qianhu (Radix Peucedani), Boheye (Folium Menthae Haplocalycis), Fangfeng (Radix Saposhnikoviae), Chuanxiong (Rhizoma Chuanxiong), Zhike (Fructus Aurantii, stir-baked with bran), Jiegeng (Radix Platycodonis), Chantui (Periostracum Cicadae), Lianqiao (Fructus Forsythiae, heart removed), Sheng Gancao (Radix Glycyrrhizae Preparata), Chi Ling (Poria with Red Core). Decoct with fresh ginger water for oral administration.

For Yinzhen, Qianghuo San is apt,
Qianghuo, Qianhu, Bohe, and Fangfeng act.

Smallpox originates from fetal toxin, latent since the beginning of physical formation, triggered by external pathogens, making it unavoidable for humans. However, its eruption may result from contracting seasonal miasma, catching wind-cold, dietary indiscretion, or fright. One disease invokes another, leading to diverse complications and unpredictable changes. Favorable outcomes are rare, while dangerous and adverse cases are numerous; even with every possible effort, recovery is rarely achieved. This difficulty explains why ancient methods of variolation emerged in Jiangxi (Jiangyou) and spread to the capital region. Tracing its origin, it is said that during the reign of Emperor Zhenzong of Song, a divine being appeared on Mount Emei and inoculated the son of Chancellor Wang Dan, curing him, thus spreading the practice. Although this account seems vague, logically it demonstrates the merit of assisting nature's transformative powers and the subtlety of adapting to times. Natural smallpox occurs after infection, whereas variolation is administered before illness; natural smallpox is treated when disease manifests, while variolation regulates health before disease onset. Through gradual薰蒸 (fumigation/inoculation) from exterior to interior and interior to exterior, the fetal toxin is fully expelled. Why then worry about diverse complications and unpredictable changes that leave skilled physicians helpless? This is indeed an excellent method to remove danger and walk on flat ground, avoiding peril and seeking safety. However, variatology is mostly transmitted orally and recorded in few medical texts. Fearing future generations will dismiss it as absurd, and with long-standing tradition lacking verification, such profound principles and good methods might be rendered useless, their miraculous efficacy lost. Is this not truly regrettable? Now, having carefully studied, meticulously evaluated, and precisely compiled the method of variolation into this book, I hope it will serve as a bridge for variolation, enabling all infants to reach the realm of longevity.
