[Prescription Verse] Shili Ju (Pomegranate Carbuncle) arises above the elbow tip. Initially presenting as millet-like blisters with spreading roots, it is hard, swollen, and painful. Upon rupture, it everts like a pomegranate, accompanied by severe chills and fever.
[Annotation] This condition occurs about one cun above the elbow tip, belonging to the Tianjing (TE10) acupoint of the Sanjiao Meridian of Hand-Shaoyang. At onset, yellow millet-like small blisters appear; the base feels expanded, the color is red and inflamed, the texture is hard and painful, and the swelling resembles an inverted cup. After rupture, it everts like a pomegranate, with chills and fever resembling malaria. It is caused by the interaction of Ministerial Fire (Xianghuo) of the Sanjiao and external dampness. In the early stage, Chan Su Wan (Toad Venom Pill) should be taken to induce sweating. Externally, apply nine moxa cones, paste Chan Su Bing (Toad Venom Cake), and cover with Wan Ying Gao (Ten Thousand Response Plaster). Apply Chonghe Gao (Harmonizing Ointment) on the inflamed area, and internally administer Juhua Qingzao Tang (Chrysanthemum Clearing Dryness Decoction); if irritability and high heat are severe, take Huxin San (Heart-Protecting Powder). After nine days, if thick pus forms, pain decreases, appetite returns, and both exterior and interior signs recede, it is a favorable prognosis (Shun); otherwise, it is unfavorable (Ni). After rupture, wash the area with decoction of Chrysanthemum stamens. Then, burn Chrysanthemum flowers to ash while retaining their medicinal property, mix with a small amount of Calomel (Qingfen), and apply externally for excellent effect. During the stages of draining pus, eliminating necrotic tissue, and generating flesh, internal and external treatments follow the protocols in the "Carbuncles and Ulcers" section.
