Amber, Pearl Powder (Zhenzhu Fen), Iron Powder (Tiefen), Trichosanthes Root Powder (Tianhuafen), Cinnabar (Zhusha), Arctostaphylos Hypothermic Stone (Hanshuishi), Niter (Yanxiao), Rhubarb (Dahuang) soaked in wine, and Raw Licorice Root (Sheng Gancao) are each taken in equal parts and ground into fine powder. For each dose, take three qian, mixed with Mint Decoction (Bohe Tang).
This formula is the primary treatment for accumulated heat in the five zang organs of men, women, and children, characterized by chest oppression and mental confusion, dry mouth and tongue, spiritual trance, epilepsy, mania, and other syndromes.
"Clarity" can calm the spirit; Amber and Pearl are both substances possessing clarity/luster, thus they are used to stabilize the Shen Po (Spiritual Soul); "Heaviness" can dispel timidity; Iron Powder and Cinnabar are both heavy substances, thus they are used to settle fright and mania; "Coldness" can eliminate heat; Niter, Rhubarb, and Hanshuishi are all cold-natured substances, thus they are used to remove accumulated heat; when heat is excessive, thirst inevitably occurs, and Trichosanthes Root can generate fluids; when fire is blazing, urgency inevitably arises, and Raw Licorice Root serves to relieve urgency.
Excess Fire (Shi Huo) can be purged, suitable for using Scutellaria Root (Huangqin), Coptis Rhizome (Huanglian), Gardenia Fruit (Zhizi), and Phellodendron Bark (Huangbai); Deficiency Fire (Xu Huo) can be tonified, suitable for using Ginseng Radix (Renshen), Astragalus Root (Huangqi), Atractylodes Macrocephala Rhizome (Baizhu), and Licorice Root (Gancao), which is referred to as "Warming can eliminate great heat." If one mistakenly uses Scutellaria, Coptis, Gardenia, and Phellodendron to treat Deficiency Fire, the fire will become even more blazing. Why is this? Because these four herbs are all descending agents; excessive descent leads to the loss of Yin fluids. When Yin is depleted, it is insufficient to control the Fire, thus causing the Fire to become even more blazing.
For various syndromes without Fire, one should exercise discretion regarding the use of these four herbs.
Blazing Fire (Zhuang Huo) certainly should not exist, but Mild Fire (Shao Huo) also cannot be absent. As stated, Heaven cannot generate all things without this Fire, and humans cannot sustain life without this Fire. Therefore, in cases where various syndromes have been excessively treated with cold-cooling methods, nearly resulting in a lack of Yang, one must carefully select and apply treatments. In the past, Ancients called a single herb preparation of Aconite Lateralis Radix Preparata (Fuzi) "Taiyang Dan"; they combined Tianxiong, Fuzi, and Sichuan Aconite Root (Chuanwu) to form "Sanjian Tang"; and they called Sulfur "Jinye Dan." All these were used to nourish True Yang, strengthen True Fire, and preserve the vital energy of the body.
Scholars versed in change, I hope for your instruction.
Preface states: Without Heat, there are no macules (Ban); without Dampness, there are no papules (Zhen). These two statements summarize the general characteristics of macules and papules. The causes of disease vary among Wind, Cold, Summerheat, and Dampness; the methods of syndrome differentiation differ between Exterior, Interior, Deficiency, and Excess. This lies in the individual's own comprehension and cannot be fully exhausted on paper.
