阳证也。太阳乃寒水所化。故《机要》名为寒疟。麻黄、羌活,太阳经之汗药也。故以为君;

恶寒与发热每日发作一次,或隔日发作一次,或三日发作一次的,都称为疟疾;这里提到的头痛、身体发热、脊柱僵硬、脉象浮,都是太阳病(太阳经受邪所致的病证)的表现。太阳经由寒水之气化生。所以《黄帝内经机要》称之为寒疟。麻黄、羌活,是治疗太阳经表证的发汗药。所以作为君药(主药);
Ephedrae Herba (Mahuang, 麻黄) [devoid of nodes] Notopterygii Rhizoma et Radix (Qianghuo, 羌活) Saposhnikoviae Radix (Fangfeng, 防风) Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [each 3 qian]
When malaria attacks, if there is headache, fever, stiffness of the spine, and a floating pulse, it is named Cold Malaria; this formula governs it.
Chills and fever occurring once daily, every other day, or every three days are all named Malaria. The mentioned headache, fever, stiffness of the spine, and floating pulse are all manifestations of Taiyang Syndrome (disease pattern caused by pathogenic invasion of the Taiyang channel). The Taiyang channel is transformed from cold water qi. Therefore, *Jingyue Quanshu* names it Cold Malaria. Ephedrae Herba and Notopterygii Rhizoma et Radix are diaphoretic herbs for the Taiyang channel. Thus, they serve as the Jun herb (principal herb); Saposhnikoviae Radix is an assistant to various wind-dispelling herbs, thus serving as the Zuo herb (assistant herb); Glycyrrhizae Radix et Rhizoma can harmonize the various herbs and simultaneously aid in releasing the exterior, thus serving as the Shi herb (envoy herb). This formula is a agent that attacks excess patterns. When prescribing for clinical cases, patients with blood deficiency should add Siwu Tang (Four Substances Decoction), and those with qi deficiency should add Ginseng Radix et Rhizoma (Renshen, 人参) and Atractylodis Macrocephalae Rhizoma (Baizhu, 白术). It entirely depends on flexible application, not rigid adherence.
Angelicae Dahuricae Radix (Baizhi, 白芷) [2 qian] Anemarrhenae Rhizoma (Zhimu, 知母) Gypsum Fibrosum (Shigao, 石膏) [each 5 qian]
When malaria attacks, if there is eye pain, nasal dryness, thirst, spontaneous sweating, insomnia, a long pulse, heat without chills, or more heat than chills, it is named Hot Malaria; this formula governs it.
This section describes Yangming Syndrome (disease pattern caused by pathogenic invasion of the Yangming channel). Because there is heat without chills, or more heat than chills, *Jingyue Quanshu* names it Hot Malaria. Angelicae Dahuricae Radix resolves pathogenic factors in the Yangming channel; Gypsum Fibrosum clears heat from the Yangming fu-organs; Anemarrhenae Rhizoma nourishes the yin of the Yangming channel. For deficient patients, Ginseng Radix et Rhizoma should be added. For patients with robust constitution and dry stool, this formula is insufficient; purgation should be used, employing Da Chaihu Tang (Major Bupleurum Decoction) from the Shanghan Men (Treatise on Cold Damage), followed by regulation with this formula.
Bupleuri Radix (Chaihu, 柴胡) [devoid of rhizome] Scutellariae Radix (Huangqin, 黄芩) [stir-fried] Ginseng Radix et Rhizoma (Renshen, 人参) Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) Pinelliae Rhizoma (Banxia, 半夏) [processed method] Zingiberis Rhizoma Recens (Shengjiang, 生姜) Jujubae Fructus (Dazao, 大枣)
When malaria attacks, if there is deafness, hypochondriac pain, alternating chills and fever, bitter taste in the mouth, propensity to vomit, and a wiry pulse, it is named Wind Malaria; this formula governs it.
This section describes Shaoyang Syndrome (disease pattern caused by pathogenic invasion of the Shaoyang channel). Since Shaoyang corresponds to Jia Wood and represents Wind in Heaven, *Jingyue Quanshu* names it Wind Malaria. Bupleuri Radix and Scutellariae Radix can harmonize pathogenic factors in the Shaoyang channel; Pinelliae Rhizoma and Zingiberis Rhizoma Recens can disperse vomiting associated with the Shaoyang channel; Ginseng Radix et Rhizoma and Glycyrrhizae Radix et Rhizoma can tonify the deficiency of Middle Qi. Tonifying the Middle Qi prevents pathogens from entering inward. Detailed examination can be found in the Shanghan Men (Treatise on Cold Damage).
