For those presenting with swelling, re-puncture using a sharp needle. (If swelling does not recede after needling, puncture again, using a sharp needle, i.e., the *Linsong* or three-edged needle.) The main branch ascends to *Quya* (ST7), follows the front of the ear, connects to the outer canthus of the eye, ascends to the frontal angle and knots there; pain in areas where branches diverge is accompanied by muscle spasms (tendon contraction). (The "main branch" refers to the primary branch. "Angle" refers to the upper angle of the ear. In all places traversed, tendon contraction and pain occur.) Treatment involves *Fanzhen* (burning needles), termed *Zhongxia Bi* (Midsummer Tendon-Bi). (The Hand-Taiyang tendon system corresponds to the Qi of the fifth month.) Disease of the Hand-Shaoyang tendon system manifests as tendon contraction and tongue rolling in the areas it traverses. (The Hand-Shaoyang tendon system originates at the tip of the fourth finger, knots at the wrist, ascends along the forearm, knots at the elbow, winds around the lateral aspect of the arm, ascends to the shoulder and travels to the neck; its branch enters at the bend of the jaw and ties to the root of the tongue, hence tendon contraction and pain occur in the traversed areas.) Treatment involves *Fanzhen*, termed *Jixia Bi* (Late Summer Tendon-Bi). (The Hand-Shaoyang meridian corresponds to the Qi of the sixth month.) Disease of the Hand-Yangming tendon system manifests as branch pain and tendon contraction in the areas it traverses, inability to raise the shoulder, and inability to turn the head left or right. (The Hand-Yangming tendon system originates at the tip of the second finger, knots at the wrist, ascends and knots at the lateral elbow, ascends and knots at *Naoshu* (SI14); its branch reaches the scapula and runs alongside the spine; its straight course goes from the shoulder up to the neck; another branch ascends to the cheek and knots at *Quanliao* (SI18); another straight course ascends to the left frontal angle, networks the head, and descends to the right mandible, thus causing branch pain and tendon contraction in the traversed areas.) Treatment involves *Fanzhen*, termed *Mengxia Bi* (Early Summer Tendon-Bi). (The Hand-Yangming meridian is the confluence of two Yang meridians, hence it corresponds to the Qi of the fourth month.) Disease of the Hand-Taiyin tendon system manifests as branch tendon contraction in the areas it traverses, severe pain, formation of *Xiben* (a type of accumulation disease affecting the lungs/chest), rib tightness, and hemoptysis. (The Hand-Taiyin tendon system originates at the thumb, ascends along the finger, knots behind the thenar eminence (*Yuji*), ascends along the arm, enters the elbow, ascends along the medial aspect of the arm, enters the axilla, emerges from *Quepen* (LI15), knots at the anterior shoulder (*Jianyu*), ascends to knot at *Quepen*, descends to knot in the chest interior, spreads through the diaphragm, converges below the diaphragm to reach the lateral hypochondrium; thus, in the areas it traverses, severe tendon contraction and pain occur as described.) Treatment involves *Fanzhen*, termed *Zhongdong Bi* (Midwinter Tendon-Bi). (The Hand-Taiyin meridian corresponds to the Qi of the eleventh month.) Disease of the Pericardium (Hand-Jueyin) tendon system manifests as branch tendon contraction in the areas it traverses, extending anteriorly to chest pain and *Xiben*. (The Hand-Jueyin tendon system originates at the middle finger, knots at the medial aspect of the elbow, ascends along the medial arm, knots in the axilla, descends and spreads, flanking the ribs anteriorly and posteriorly. Its branch enters the axilla, spreads in the chest, and knots at the diaphragm, hence disease occurs in the traversed areas as described.) Treatment involves *Fanzhen*, termed *Mengdong Bi* (Early Winter Tendon-Bi). (The Hand-Jueyin meridian is the end of two Yin meridians, hence it corresponds to the Qi of the tenth month.) Disease of the Hand-Shaoyin tendon system manifests as internal tension, *Fuliang* (a hard, latent accumulation) supporting the heart, and lower manifestation as *Zhouwang* (a net-like pulling sensation at the elbow); tendon contraction and tendon pain occur in the areas it traverses. ("Cheng" means supported from below. "Fuliang" refers to a hard, latent accumulation. "Wang" implies pulling tight like a net. The Hand-Shaoyin tendon system originates at the medial side of the little finger, knots at the *Rui Gu* (pisiform bone), ascends and knots at the medial elbow, enters the axilla, flanks the breast internally, knots in the chest, and descends to tie to the umbilicus; thus, on the medial side it presents as tension and Fuliang, externally as Zhouwang, and in traversed areas as tendon contraction and pain.) Treatment involves *Fanzhen*; if *Fuliang* forms accompanied by expectoration of blood and pus, it is incurable and fatal. (Both above and below the umbilicus are called Fuliang. If Fuliang has formed and blood/pus appear in sputum, the disease is severe and organs are damaged, leading to death and being incurable.) This is termed *Jidong Bi* (Late Winter Tendon-Bi). (The Hand-Shaoyin tendon system corresponds to the Qi of the twelfth month.) For treatment involving *Fanzhen*: use burning needle *Jieci* (robbing puncture), take the cessation of symptoms as the endpoint, and take the painful point as the acupoint. (*Fanzhen* is a heated needle. *Jieci* uses the fire energy to rob/scatter cold pathogens. *Fanzhen* is also called *Cuizhen*. The meaning is that knowing the arrival of Qi sets the limit. Taking pain as the Shu point means the painful spot is the acupoint.) Diseases of the tendon systems: Cold causes reverse bending and tendon tension; Heat causes tendon relaxation and flaccidity, with loss of function of the Yin organs/genitals; Yang tension causes reverse bending, Yin tension causes inability to extend while flexing. (The following summarizes the diseases of the tendon systems mentioned above. Yang tension and Yin tension refer to the Foot-Taiyang and Foot-Shaoyin tendons, both relating to back diseases; Yang tension externally causes hyperextension and bending backward, Yin tension internally causes inability to extend while flexing forward.) *Cuici* (burning needle puncture) treats cold-induced tension. In heat cases, tendons are relaxed and flaccid; do not use *Fanzhen*. (Tendon-Bi diseases are mostly due to cold, hence the above mentions treating with *Fanzhen Jieci*; however, some are due to heat, for which treatment should avoid heat methods and not use *Fanzhen*; verification lies in whether the tendons are tense or flaccid.) When the tendons of the Foot-Yangming and Hand-Taiyang are tense, it causes deviation of the mouth and eyes; facial tension prevents sudden vision adjustment. Treatment follows the method stated above. (This further explains the syndrome of facial paralysis/deviation, which must be due to disease of the Foot-Yangming and Hand-Taiyang tendon systems.)
