Chai Ling Tang (From *Ren Shu*) Treats malaria with more heat than chills, dry mouth, vexation, and insomnia. (This is Xiaochaihu Tang combined with Wuling San. Xiaochaihu Tang see Jaundice section. Wuling San see Sanjiao section.) Yu Chang notes: Chai Ling Tang from *Ren Shu* is a key medicine for treating malaria. However, I dare not readily include it in the main selection, but keep it as backup, because Wuling San promotes urination, which might consume fluids if wood-fire overacts on the stomach. In cases of extreme thirst seeking water to save oneself, promoting urination would violate the principle. Practitioners should consider this carefully.
Banxia San Treats Phlegm-Malaria with regular onset, more heat than chills, headache in forehead/temples, twitching of chest muscles, immediate vomiting upon eating, reddish complexion. Take this. Pinelliae Rhizoma Preparatum (Banxia, soaked seven times, powdered, mixed with ginger juice to make cakes, dried), Agastache Herba (Huoxiang), Notopterygii Rhizoma et Radix (Qianghuo), Ligustici Chuanxiong Rhizoma (Xiongqiong, each Yi fen), Sinapis Semen (Qianniu, Ban liang). The above are fine powder. Take three qian per dose, taken with plain boiled water after meals.
Lujiang Yin Treats Spleen-Stomach Phlegm-Malaria presenting with chills and fever. Fresh Ginger (Shengjiang, Si liang), with skin, crushed to extract one bowl of juice. Dew-exposed overnight until dawn. Take cold on an empty stomach.

The above are chopped. Take one liang per dose, two zhan of water, three slices of ginger, one jujube, decoct to eight fen. Take early on the day of attack. Yu Chang notes: This formula treats chronic maternal malaria where pathogens are scattered and interior/exterior are chaotic. Expanding the method to seek cure, yet still centering on Xiaochaihu Tang, which is gratifying.
For malaria caused by labor and worry/sorrow, with much sweat, poor appetite, and severe fatigue, use Buzhong Yiqi Tang. (Formula see Xu Lao section.)
Xiaochaihu Tang plus Dichroae Radix (Changshan) cuts malaria with miraculous effect. (Formula see Jaundice section.)
For women with chronic malaria, use Xiaochaihu Tang combined with Siwu Tang. (Xiaochaihu Tang see Jaundice section, Siwu Tang see Women section.)
For children with malaria and abdominal masses, use Shengdi, Shaoyao, each Yi qian; Chenpi, Chuanxiong, Stir-fried Huangqin, Banxia, each Yi qian; Gancao, San fen. Decoct with ginger, mix with vinegar-fried Biejia powder. Effective.
*Zhengchuan* records two men, aged forty to fifty, each suffering from malaria for three years. Both attacked on Yin, Shen, Si, Hai days. One attacked by day, starting at Si hour and subsiding at Shen hour. One attacked by night, starting at Hai hour and subsiding at Yin hour. Day attacks are Yang within Yin; treatment should tonify qi and resolve exterior. Use Xiaochaihu Tang with doubled Chaihu and Renshen, plus Baizhu, Chuanxiong, Gegen, Chenpi, Qingpi, Cangzhu. Night attacks are Yin within Yin; treatment should tonify blood and soothe liver. Use Xiaochaihu Tang combined with Siwu Tang, plus Qingpi. Give ten doses each, decocted with ginger and jujube. Take one dose daily, two hours before onset. After eight doses, both recovered with heavy sweating on the same day.
Danxi treated a man who developed malaria due to labor-induced cough, then took diaphoretic drugs, leading to fever, short tongue, slurred speech, loud phlegm rattle, and surging-slippery pulse. First used Dushen Tang plus Zhulv (bamboo sap) and Geke (clam shell). After one dose, sticky phlegm was vomited, tongue normalized. Later used Huangqi Renshen Tang, cured in half a month.
A woman had malaria every three days, poor appetite, amenorrhea for three months, pulseless, intermittent chills. Proposed treating as deficiency-cold. Suspected error, re-diagnosed: she combed hair, spoke, and moved normally. Realized error. Amenorrhea was not lack of blood but phlegm obstruction. Pulselessness was not qi-blood depletion but heat from accumulated phlegm hiding the pulse. Treated as excess-heat with Sanhua Wan. After ten days, appetite improved, pulse appeared, slightly wiry. Stated that with restored stomach qi, malaria would self-resolve and menses return without medicine. Advised bland diet. Result confirmed.
A woman had malaria for over a month, alternating days at Shen/You hours, head/body pain, much chills preferring very hot soup, hidden pulse, pale/dull face. Treated as excess-heat. Used Shizao Tang powdered, pill-sized like millet grains. Took ten pills swallowed with saliva, three times daily. Ate bland rice for half a month. Recovered with heavy sweating.
A woman had dysentery, changed to malaria due to grieving for her child, attacking 5-6 times daily, sweating like rain, no vomiting, faint-rapid pulse, extreme fatigue. No pathogen to treat; yin deficiency with yang dispersion, life in danger. Used Renshen, Baizhu two liang; Shaoyao one liang; Huangqi half liang; Honey-fried Gancao two qian. Made into four large doses. Cured.
