In cases of *Shanghan* (Cold Damage), the use of fire needles will inevitably cause fright.
If the pulse in *Shanghan* is floating, and the physician erroneously uses fire methods to force sweating, leading to the loss of *Yang Qi* (Vital Energy), it will inevitably result in fright, mania, and restlessness.
In *Shanghan*, if the pulse is neither wiry nor tight but weak, weakness implies thirst; if subjected to fire therapy, it will inevitably lead to delirious speech (*Zhanyu*).
In *Taiyang Bing* (Taiyang Disease), if treated with fire fumigation and sweat cannot be obtained, the patient will inevitably become irritable; if the condition persists beyond the expected course, it will inevitably lead to bloody stool.
If *Yangming Bing* (Yangming Disease) is subjected to fire therapy, slight sweating on the forehead combined with dysuria will inevitably lead to jaundice.
In *Shaoyin Bing* (Shaoyin Disease), coughing with diarrhea accompanied by delirium indicates that the pathogen has been coerced by fire qi; dysuria will inevitably occur due to the forced sweating of Shaoyin.
For all cases of diarrhea with pain in the anal region, moxibustion or hot compresses using moxa-warmed Zhi Shi (Immature Bitter Orange) or roasted salt are appropriate.
A fine and rapid pulse contraindicates moxibustion; because fire becomes a pathogenic factor, it causes irritability and rebelliousness.
When the pulse is floating, the exterior should be resolved by sweating; if moxibustion is used instead, the pathogen has no outlet, and due to fire, the pathogen intensifies. The disease from the waist down will inevitably become heavy and bi-syndrome (numbness/obstruction); this is called "Fire Reversal" (Huo Ni).
When the pulse is floating and heat is severe, moxibustion must not be used; this is an Excess pattern. Treating Excess as Deficiency, the fire stirs the blood, causing dry throat and hemoptysis.
In early-stage Shaoyin Disease (one or two days), if the mouth feels normal and there is aversion to cold on the back, moxibustion is indicated.
In Shaoyin Disease with vomiting, diarrhea, cold extremities, and a deficient pulse, seven moxa cones should be applied to the Shaoyin channels.
In Shaoyin Disease with diarrhea and a faint, choppy pulse, accompanied by vomiting and sweating, frequent attempts at defecation with little output occur; warming the upper body with moxibustion is indicated. (Another version suggests fifty cones applied to Jueyin.)
In cases of diarrhea with cold extremities and absent pulse, moxibustion is indicated for treating Jue (Reversal Cold), involving the Jueyin channel. If the limbs remain cold after moxibustion and slight panting occurs, death ensues.
In Shanghan lasting six or seven days, if the pulse is faint, extremities are cold, and there is irritability, moxibustion should be applied to Jueyin; if reversal cold does not resolve, death ensues.
When the pulse is hurried and extremities are cold, moxibustion is indicated.
Do not needle when the patient is extremely angry, immediately after sexual intercourse, severely fatigued, heavily intoxicated, overly full, extremely thirsty, or greatly frightened.
Do not needle when there is high fever, profuse sweating, chaotic pulse, or when the disease presentation contradicts the pulse.
The superior practitioner needles before the disease arises; the next best needles before it peaks; then needles when it declines. A practitioner who violates these principles is said to damage the physical form.
