Commentary: There are few young scholars who value learning; they only know how to cherish wealth, not learning. When faced with tasks, they realize the preciousness of learning and regret their ignorance and narrow experience. Therefore, pitying them thus, I have neglected sleep and food, explored profound and subtle principles, compiled them into sections, to leave for those who have not yet awakened. If one can proficiently study this volume, it is sufficient to become a great physician.
For cases of cold above the diaphragm, fullness in the lower abdomen, borborygmus, qi in the bladder, and cold diarrhea, purgative medicines should be administered. After ingestion, milky-like green-yellow water and green mud should be discharged. For mild cases, stopping after one or two doses of purgatives to remove the pathogen is sufficient; for severe cases, five or six times, administering purgatives to achieve frequent large-scale defecation three times a day, until the root cause is completely exhausted, will lead to permanent cure. This purging method stabilizes after two or three bowel movements following the Si hour (9-11 AM), returning to normal, without causing pathological diarrhea. For stubborn diseases located above the diaphragm, such as accumulation of phlegm due to long-standing cold, hernia masses (Shen Jia), hard lumps from retained food, rebellious cough with upward rushing Qi, constant spitting of saliva, and rebellious Qi rushing up to the chest, hypochondrium, throat, and pharynx—all caused by accumulated cold in the stomach—they must be induced to vomit until recovery. Mild cases require one or two inductions; severe cases require five or six. The vomitus initially consists of cold foam and sour fluid, then yellow juice; severe cases produce red-purple fluid. If previously suffering from Zhu disease (chronic/infectious disease), black blood may be expelled. Vigorous vomiting induced by emetic medicine causes temporary severe distress but resolves spontaneously without causing deficiency. After consuming cold drinks/food, ears will not suffer from tinnitus/deafness, and limbs will not suffer from numbness. Some patients may feel a lump blocking the throat during vomiting that cannot be expelled; drinking one or two he of medicinal wine will cause expulsion of a lump about the size of a fist, resembling a segment of egg yolk. Severe cases expel ten pieces; mild cases expel five or six.
The root of these various conditions lies in not expelling this fundamental pathology today. Even if skilled physicians prescribe one or two doses of decoctions to suppress symptoms, recurrence will happen upon dietary indiscretion later. A skilled physician should administer this medicine to expel the root once, ensuring lifelong health.
Method of Expelling Pathogens via Vomiting and Purging:
For chronic patients, those who are weak and emaciated, the elderly, and nobles:
These individuals should take small doses. If illness persists over days, it will resolve internally; there is no need to add emetic or purgative drugs.
If emetic/purgative drugs are added and damage is excessive, and vomiting/diarrhea do not stop, taking one square-cun scoop of soybean bran with water will stop it. If soybean bran is unavailable, chewing Isatis leaf or Vigna angularis leaf also works. For men suffering from Five Exhaustions and Seven Injuries (Wu Lao Qi Shang) with depleted Yang Qi, emaciation, and skeletal prominence, administration leads to recovery. Within ten days to a month, skin and muscles become plump and lustrous, brain fills, and essence overflows.
