When a person's vital Qi is robust, the external complexion is lustrous, and the internal pulse is harmonious. If the Qi of the Five Zang and Six Fu organs is depleted, the pulse loses its foundation, and the complexion loses its luster. In such cases, not one in a hundred will survive. Although they may still be able to eat, drink, walk, and stand, they appear listless and unaware, oblivious to the approach of death. This is indeed a painful general principle, which is listed below.
If a patient stares with wide eyes and desires water, yet has hardness and fullness in the epigastrium, and the pulse is soft (Ru) and faint, death ensues.
In cases of hematemesis or epistaxis, if the pulse is floating, large, firm, and rapid, death ensues.
If a patient speaks deliriously, has body heat but cold hands and feet, and the pulse is minute and fine, death ensues.
In cases of profuse diarrhea that does not stop, if the pulse is tight, large, and slippery, death ensues.
If a patient suffers from headache and eye pain, and the pulse is choppy and short, death ensues.
If a patient suffers from abdominal pain, and the pulse is floating, large, and long, death ensues.
If a patient suffers from abdominal pain and wheezing, and the pulse is slippery, frequent, and tight, death ensues.
In cases of four-inverse (Si Ni, cold extremities), if the pulse is floating, large, and firm, death ensues.
If a patient cannot hear, and the pulse is floating, large, and choppy, death ensues.
If a patient suffers from cephalalgia (brain pain), and the pulse is slow and large, death ensues.
Pain on the left side accompanied by pain on the right, or pain above accompanied by pain below, indicates death.
Pain in the lower part accompanied by a pathological pulse indicates death.
In cases of four-inverse where the patient is unresponsive to calls, and the pulse is absent, death ensues.
If the patient's pulse should be large but is instead small, death ensues.
In obese individuals, if the pulse is thin and nearly absent, death ensues.
In emaciated individuals, if the pulse is restless, death ensues.
If the human pulse, which should naturally be slippery and smooth, becomes choppy, death ensues.
If the human pulse, which should naturally be long, becomes short, death ensues.
If the Chi pulse at the wrist corresponds too slowly to the Cun Kou pulse above, death ensues.
In Warm Disease (Wen Bing), if after three or four days there is no sweating, and the pulse is excessively rapid, death ensues.
In Warm Disease, if the pulse is minute and fine, movement is sluggish, and the chest feels obstructed, death ensues.
In Warm Disease, if fever is severe but the pulse is conversely small and weak, death ensues.
When the disease is severe, if the pulse is irregular and disordered, death ensues.
In Warm Disease, if there is abdominal pain and dysentery, death ensues.
In Warm Disease, if there is no sweating, or if sweating does not reach the feet, death ensues.
In Malaria (Nue), if there is stiffness and spasm of the waist and spine, and convulsions, death ensues.
If there is heart and abdominal distension and fullness, with unremitting pain, and the pulse is hard, large, and surging, death ensues.
If bloody dysentery does not stop, and the body is hot with a rapid pulse, death ensues.
If there is abdominal fullness and four-inverse, and the pulse is long, death ensues.
In Heat Disease, after seven or eight days, sweating should occur; if it fails to occur and the pulse is absent, death ensues.
In Heat Disease, after seven or eight days, if there is no sweating, restlessness and mania, blackened tongue and lips, and the pulse is conversely fine and weak, death ensues.
In Heat Disease, if sweating has not occurred but the pulse is excessively vigorous, death ensues.
In Heat Disease, if sweating occurs but the pulse remains, moving back and forth and becoming larger, death ensues.
In coughing diseases, if the pulse is rapid and the body is emaciated, death ensues.
In sudden severe coughing, if the pulse is scattered, death ensues.
In coughing diseases, if the form is fat but the pulse is severely tight, death ensues.
In coughing accompanied by vomiting, with involuntary fecal incontinence, and a string-tight pulse nearing absence, death ensues.
In various coughing and wheezing diseases, if the pulse is deep and floating, death ensues.
In diseases of rebellious Qi ascending, if the pulse is rapid, death ensues.
In diseases of muscle heat, with emaciation, prolapse of the rectum, persistent heat, and an extremely tight pulse, death ensues.
In Yang hernia (Yang Pi) with tendon contraction, if the pulse is extremely rapid, death ensues.
In Wind-Stroke (Zhong Feng) with flaccidity and insensibility, if the pulse is tight, death ensues.
In asthma/wheezing with urgent breathing and constriction around the torso, if the pulse is choppy, death ensues.
In chills and fever with convulsions, if the pulse is large, death ensues.
In metal wounds (Jin Chuang) with unceasing bleeding, if the pulse is large, death ensues.
In falls and injuries with internal trauma, if the pulse is small and weak, death ensues.
In Cold Damage (Shang Han), if the body is very hot but the pulse is conversely small, death ensues.
In four-inverse with sweating, if the pulse is deficient and slow, death ensues.
In hole-diarrhea (Dong Xie) where food intake ceases, if the pulse is tight, death ensues.
In intestinal dysentery (Chang Pi) with discharge of white pus, death ensues.
In intestinal dysentery with discharge of pus and blood, if the pulse is suspended and absolute, death ensues.
In intestinal dysentery with discharge of pus and blood, if the body is cold and the pulse is absent, death ensues.
In coughing diseases, if the pulse is deep and firm, death ensues.
In accumulations and masses within the intestines, if the pulse is deficient and weak, death ensues.
In Water Qi (Shui Qi) diseases, if the pulse is faint and small, death ensues.
In water swelling like a drum, if the pulse is deficient, small, and choppy, death ensues.
In projectile diarrhea, if the pulse is floating, large, and slippery, death ensues.
In conditions of both interior and exterior deficiency, with inability to sleep peacefully, cold body, minute and fine pulse, vomiting, and inability to retain food, death ensues.
In attacks of cold Qi ascending, if the pulse is rebellious and choppy, death ensues.
In sudden death, if the pulse is firm yet minute, death ensues.
In Heat Disease lasting three to five days, with headache, body heat, and normal appetite, if the pulse is straight and rapid, death occurs on the eighth day.
In chronic diseases, if the pulse is excess/firm, death ensues.
Also, if the pulse is deficient and slow, deficient and minute, deficient and slippery, or string-tight, death ensues.
In acute diseases, if the pulse is string-tight and rapid, death ensues. All these ominous pulses indicate that ten occurrences result in ten deaths, and a hundred occurrences result in a hundred deaths; they are incurable.
