After sweating, vomiting, or purging treatments, deficiency-heat vexation occurs leading to insomnia. If the condition is severe, it will inevitably manifest as tossing and turning, restlessness, and a feeling of stifling discomfort in the heart (Zhongxin Ao Nao). Zhi Zi Chi Tang (Gardenia and Fermented Soybean Decoction) is the primary prescription. If after sweating or purging there is vexation and heat, accompanied by a sensation of obstruction in the chest (Xiong Zhong Zhi), Zhi Zi Chi Tang should also be used. Zhang Zhongjing stated that patients with a history of loose stools should not take this formula.
[Zhi Zi Chi Tang]
Fei Zhi Zi (Gardeniae Fructus, four liang, crushed) Dou Chi (Fermented Soybean, half he)
Take two zhan of water. First decoct Zhi Zi until one zhan remains, then add Dou Chi and decoct together until seven-tenths remain. If vomiting occurs after taking the medicine, stop further administration. Another version states: if bowel movement becomes rapid/loose, stop further administration.
[Zhu Sha An Shen Wan] Treats mental restlessness, palpitations (Zheng Chong), dizziness with nausea, chaotic Qi in the chest with heat, and symptoms resembling stifling discomfort (Ao Nao). These are caused by latent fire in the blood above the diaphragm, steaming upward causing unrest. The appropriate method is to weigh and balance, suppressing floating Yin fire and nourishing Yuan Qi of the Upper Jiao.
Zhu Sha (Cinnabar, one qian, separately ground, processed by water flotation, dried in shade) Huang Lian (Coptidis Rhizoma, remove roots and clean, one qian two fen) Sheng Di Huang (Rehmanniae Radix Recens, three fen)
Dang Gui (Angelicae Sinensis Radix, remove radicle) Gan Cao (Glycyrrhizae Radix et Rhizoma, honey-fried, each half qian)
Grind all into powder. Soak in wine and use steamed dough as binder to make pills the size of millet seeds. Coat with Zhu Sha powder. Take fifteen pills per dose, swallowed with saliva. Take after meals; this follows the principle of gradual treatment.
The Nei Jing states: "When heat is excessive, treat with sweet-cold herbs; drain with bitter herbs." Huang Lian's bitter-cold nature removes heart vexation and damp-heat, serving as the Jun (Monarch) herb. Gan Cao and Sheng Di Huang's sweet-cold nature drains fire, tonifies Qi, and generates Yin blood, serving as the Chen (Minister) herbs. Dang Gui supplements insufficient blood; Zhu Sha anchors floating fire and calms the Shen (Spirit).
[Ba Wu Ding Zhi Wan] Gently tonifies Heart Qi, calms the Spirit and subdues fright, clears Phlegm-Stasis and Heat from the diaphragm.
Yuan Zhi (Polygalae Radix, remove core) Shi Chang Pu (Acori Tatarinowii Rhizoma) Mai Men Dong (Ophiopogonis Radix) Fu Shen (Poria with Hostwood)
Bai Fu Ling (Poria, remove core, each one liang) Bai Zhu (Atractylodis Macrocephalae Rhizoma, half liang) Ren Shen (Ginseng Radix et Rhizoma, one liang five qian) Niu Huang (Calculus Bovis, two qian, separately ground)
Grind into powder, mix evenly with Niu Huang. Use refined honey to make pills the size of sesame seeds. Coat with Zhu Sha powder. Take twenty to thirty pills per dose, with warm water. No fixed time for administration.
