First, if the fetus dies in the womb due to a heat pathogen (heat disease), the body becomes cold immediately after fetal death, foam appears at the corners of the mouth, the fingernails turn cyanotic black, and the four limbs become cold. Administer this decoction to warm and expel the dead fetus; it will be expelled naturally within a short time.
Second, for difficult labor, including transverse or breech presentation, or retention of the placenta (fetal membrane), this decoction is effective for all such conditions.
Third, for postpartum retention of the placenta, administer this decoction, and the placenta will be expelled subsequently. The blood retained within the placenta will also discharge naturally. This applies equally to cases where the umbilical cord or placental membranes have ruptured.
Fourth, for postpartum syncope due to blood loss, which physicians may misidentify as "dark wind" (an obscure wind syndrome), administer this decoction for immediate recovery.
Fifth, for dry mouth and mental oppression, which physicians may misidentify as "chest and diaphragm discomfort," this condition is actually known as "blood accumulation." Administer this decoction for immediate recovery.
Sixth, for alternating chills and fever, which physicians may misidentify as malaria, administer this decoction for immediate recovery.
Seventh, for postpartum edema of the four limbs, which physicians may misidentify as water qi (fluid retention), this is caused by qi and blood obstruction leading to coldness in the limbs. First, administer this decoction for immediate relief, then administer the "transforming medicine" to purge lochia and waste products. The transforming medicine is referred to as Heishen Wan (Black Spirit Pill).
Eighth, for postpartum visual hallucinations of spirits or ghosts, which physicians may misidentify as wind evil, administer this decoction for recovery.
Ninth, for postpartum aphonia lasting within one month, administer this decoction for recovery.
Tenth, for postpartum abdominal pain accompanied by diarrhea and dysentery, administer this decoction urgently, followed by Tiaozhong Tang (Regulate Middle Decoction). (Note: Tiaozhong Tang is also known as Zhishi San [Calcined Stone Powder] for stopping diarrhea, as cited in *Chuchan Shujing*.)
Eleventh, for postpartum generalized body pain, administer this decoction for recovery.
Twelfth, for postpartum hematuria and constipation, which are often treated unsuccessfully as Wulin (Five Types of Dysuria), administer this decoction for recovery.
Thirteenth, for postpartum metrorrhagia (profuse bleeding), defined as incomplete expulsion of lochia that persists due to lack of treatment or consumption of salty and sour foods. If the abdomen feels vaguely painful and the condition is difficult to treat, accompanied by liver heat and stiffness of the back tendons, administer this decoction urgently, followed by Heishen Wan (Black Spirit Pill) for recovery.
Fourteenth, for postpartum stagnation of qi and blood leading to asthma, cough, and hypersomnia. This refers to conditions occurring after the first month when the menstrual pulse has not yet cleared, caused by the accumulation of hot wheat flour forming pathology, or by qi masses that surge to the heart upon attack, causing heart pain,急促 breathing, cough, weakness in the four limbs, and intermittent sweating. If heart pain occurs with amenorrhea, it is named Baizheng (Decayed Syndrome). If abdominal pain surrounds the navel with a red face, administer this decoction for recovery.
Fifteenth, for postpartum epigastric and abdominal fullness with irregular vomiting, which physicians call Fanwei (Reverse Stomach/Regurgitation), if unresponsive to other treatments, administer this decoction urgently to vomit out gastric waste products, leading to recovery.
Sixteenth, postpartum blackening of the mouth and nose with rising qi and epistaxis is considered incurable.
Seventeenth, postpartum急促 breathing and wheezing from the throat is considered incurable.
Eighteenth, postpartum stroke (Zhongfeng), caused by getting out of bed without cause, sexual intercourse within 100 days, or fright from moxibustion. When the zang-fu organs are struck by wind, symptoms include dry eyes, stiffness of the waist and back tendons, opisthotonos (arching of the back like a bow), and trismus (locked jaw); this is incurable.
The formula for Puhuang Heishen San (Polygonum Catharticum Black Spirit Powder) is listed at the end of the first discussion.
The formula for Heishen Wan (Black Spirit Pill) is listed at the end of the twelfth discussion.
