Various diseases in women arise primarily from the imbalance of blood and qi (vital energy). Among these, abnormalities occurring postpartum are indeed difficult to treat. Within the hundred days after childbirth, pathological changes are particularly numerous: some experience dizziness or fainting upon sitting, dry mouth upon lying down, incessant talking followed by chest oppression; some develop metrorrhagia (profuse vaginal bleeding) due to consuming hot-natured foods; others suffer lower back pain from drinking cold water or taking cooling medicines. When stagnant blood enters the abdomen, it causes distension and tinnitus in the hypochondrium; when lochia rushes upward, it induces vomiting and regurgitation. If the spleen is affected by blood stagnation, symptoms resembling exterior cold syndromes appear; if liver qi is disturbed by pathogens, irritability and anger increase; if the lungs are disrupted, there is chest stuffiness and coarse breathing; if the stomach has not settled, abdominal distension prevails. Stagnant blood circulating throughout the body causes soreness and pain in all joints; stagnating in the limbs causes swelling of the skin; loss of voice occurs because stagnant blood enters the heart; dribbling hematuria also results from blood entering the abdomen. Generally, weakness is caused by accumulated blood; dysfunction of the Triple Burner initiates various diseases. Whenever consulting medical texts, one often fails to fully grasp the true principles of prenatal and postnatal care. For those in poverty or living in villages, facing emergencies while pondering solutions, their spirits are exhausted and confused, unsure how to proceed. Suddenly, a person of extraordinary appearance appears and says, "I possess a secret technique; do you know it?" In the midst of sudden awe and reverence, he adds, "Please maintain sincerity. Since your heart desires to save lives, this matter should bring great joy. I have a secret formula for postpartum conditions named Wu Jin Fang (Black Gold Formula), which I impart to you; cherish it." I immediately knelt to receive the essentials, instantly feeling refreshed and mentally clear, as if enlightened by a deity in a dream. Thus, based on this formula, I compiled and popularized it, harmonizing spiritual intent, aspiring to establish eighteen postpartum formulas as follows.
First Discourse Question: What is the cause of fetal death in utero?
Answer: It occurs because the mother suffers from a heat disease. After six or seven days, the internal organs become extremely hot, steaming and cooking the fetus, leading to fetal death. How can one know this? The body becomes cold after death, does not remain in the uterus, but falls out. There is cold pain below the umbilicus, foam at the corners of the mouth, cold abdomen, cyanotic fingernails, and cold extremities, with life hanging in the balance. Administering this medicine three doses will warm the fetus, allowing it to pass naturally within moments.
Second Discourse Question: What is the cause of dystocia (difficult labor)?
Answer: Once the fetus is formed, it feeds on maternal blood. By the tenth month, excess blood coagulates into a mass, commonly called the "child pillow" (placental clot). When birth begins, this mass ruptures into stagnant blood, wrapping around the fetus, causing difficulty in delivery. Taking this powder three times disperses the stagnant blood, allowing natural delivery within moments. Even in breech presentation, where the position differs, this treatment is effective.
Third Discourse Question: What is the cause of retained placenta?
Answer: After delivery, stagnant blood flows into the placenta, causing it to swell and making expulsion difficult. Taking this powder disperses the blood within the placenta, allowing it to exit naturally shortly thereafter. If the umbilical cord breaks, this powder should still be taken to facilitate expulsion.
Fourth Discourse Question: What is the cause of postpartum syncope (blood晕), where the patient cannot rise after three days, sees ghosts, or falls into unconsciousness without recognizing people?
Answer: Three days postpartum, blood and qi are unstable. They scatter through the five zang organs and rush to the lungs. Laypeople do not recognize this condition, calling it "dark wind." Treating it with this method ensures safety; administering this medicine brings immediate recovery.
Fifth Discourse Question: What is the cause of postpartum chest oppression and thirst?
Answer: Within seven days postpartum, blood and qi are unsettled. This is often due to eating hot noodles, which bind with blood in the heart, causing thirst. Laypeople call this "chest diaphragm congestion." Treating it with this method ensures recovery; self-administration of this powder leads to immediate cure.
Sixth Discourse Question: What is the cause of alternating chills and fever with thirst postpartum?
Answer: Postpartum debility allows stagnant blood to enter the lungs, causing fever, or the spleen, causing chills, resulting in thirst. Laypeople mistake this for malaria. Treating it with this method ensures recovery; taking this powder brings immediate cure.
Seventh Discourse Question: What is the cause of postpartum limb edema with chills and fever?
Answer: Stagnant blood flows inward, filling the five zang organs, causing stagnation in the limbs that cannot return, leading to swelling. Laypeople mistake this for edema. How to distinguish between water edema and blood edema? Water disease presents with asthma and scanty urine; blood disease presents with thirst and cold limbs. First take this powder, then take Xuan Yuan Wan (Hei Shen Wan / Black Divine Pill) to purge foul matter, leading to natural recovery.
Eighth Discourse Question: What is the cause of incoherent speech, mania, seeing ghosts, chills, fever, and thirst postpartum?
Answer: Postpartum stagnant blood attacks the heart. The heart rejects the pathogen, gets steamed by the stagnant blood, leading to chest oppression, heat, thirst, restlessness, and manic speech. Laypeople attribute this to wind or divine evil. Treating it with other methods fails; only taking this powder leads to cure.
Ninth Discourse Question: What is the cause of postpartum mutism?
Answer: The human heart has seven apertures and three hairs. Postpartum stagnant blood ascends and blocks these heart apertures, causing mutism. If laypeople do not remove the stagnant blood from the heart apertures, the illness persists. Taking this medicine gradually restores speech.
Tenth Discourse Question: What is the cause of postpartum abdominal pain and dysentery?
Answer: Within the first month postpartum, drinking cold water mixes with blood and flows into the large intestine. The intestine absorbs the blood, causing illness characterized by indigestion, diarrhea, or distension, and intermittent pus and blood. Urgently take this powder, followed by Tiao Zhong Tang (Regulate Middle Decoction), for immediate effect.
Eleventh Discourse Question: What is the cause of generalized body pain postpartum?
Answer: Postpartum, the joints open up. Stagnant blood flows into the limbs, stagnating for a long time without dispersing, forming void distension and causing pain. Taking this powder removes stagnant blood from the joints, bringing immediate relief.
Twelfth Discourse Question: What is the cause of postpartum blood stasis with red, scanty urination and painful defecation?
Answer: Stagnant blood flows into the small intestine, blocking the water passages, causing difficult urination; it flows into the large intestine, blocking the anus, causing difficulty in both urination and defecation. Doctors mistake this for the Five Types of Strangury (Wu Lin). Treating it otherwise fails. Take this powder, followed by Hei Shen Wan (Black Divine Pill), to expel clots and achieve immediate cure. (Alternatively, use Pai Nong Nei Bu San from Hong's Formulas for immediate effect.)
Thirteenth Discourse Question: What is the cause of postpartum metrorrhagia (Beng Zhong)?
Answer: Stagnant blood and lochia fail to stop. Over time, or after eating salty/sour foods, it transforms into metrorrhagia. Blood coagulates and turns black, resembling chicken liver. Symptoms include alternating chills/fever, stiffness in the back and shoulders, and oppression, named Beng Zhong. Take this powder first, then Hei Shen Wan (Black Divine Pill) for stability.
Fourteenth Discourse Question: What is the cause of postpartum blood/qi imbalance, blockage, coughing, and fright?
Answer: After one month postpartum, menstruation has not returned. Failure to observe dietary restrictions, such as eating hot noodles, causes illness. Accumulations form masses. Upon attack, there is rapid breathing, coughing, limb temperature fluctuations, chest oppression, dry mouth, back stiffness, irritability, night terrors, general weakness, sweating, heart pain, amenorrhea, named Blood Syndrome (Xue Zheng). Abdominal pain, excessive sleep, and ruddy complexion occur. Without treatment, this becomes bone-steaming consumption. Treatment requires careful observation; other medicines rarely work. Only taking this medicine makes the ailment recede.
Fifteenth Discourse Question: What is the cause of abdominal distension and vomiting/regurgitation postpartum?
Answer: Stagnant blood stops between the spleen and stomach. Qi clashes uncontrollably, stagnates, causing vomiting and distension of the chest and hypochondrium. Laypeople mistake this for Fan Wei (regurgitation). Other treatments fail. Urgently take this medicine to vomit foul matter from the stomach, then consume congee.
Sixteenth Discourse Question: What is the cause of black lips/nose and epistaxis postpartum?
Answer: Stagnant blood transmits to the zang-fu organs, filling them, and rushes to the head and face but cannot retreat. The face becomes dry and dark/black. Rushing to the nose causes black epistaxis. This results from severe postpartum deficiency-heat. Other treatments fail. This is named Heart-Lung Stagnant Blood; take this medicine.
Seventeenth Discourse Question: What is the cause of postpartum rapid breathing with a sound like a cat in the throat?
Answer: Stagnant blood rushes into the heart and lungs. Qi and blood combine to form clots entering the throat. If these symptoms appear, urgently take this medicine.
Eighteenth Discourse Question: What is the cause of postpartum wind-stroke accompanied by black blood spots?
Answer: Going outdoors into the wind within seven days postpartum, or engaging in sexual activity within one hundred days, or having moxibustion sores, can lead to wind-stroke. Wind takes time to manifest slowly. Symptoms include qi stagnation, lower back pain, tendon stiffness resembling opisthotonos, locked jaw. These indicate wind-pathogen invasion. Dark face, reddish-black body, stagnant blood flowing into zang-fu organs filling them, and spreading to the skin causing blood spots, result from this transformation. Urgently take this medicine for certain cure.
Xuan Jing Wan (Circulating Menstruation Pill)
Crotonis Fructus (Badou): Fifteen seeds, peel and remove heart/membrane, use raw, place in silk bag, boil in ash lye for ten boils, remove, grind, press oil out with paper, regrind.
Take three qian of Wu Jin San (Black Gold Powder), mix with unfermented wine to a paste consistency, place in a porcelain jar, simmer over double boiler until thickened into an extract. Add Badou powder, mix well, and roll into pills the size of mung beans. Swallow fifteen pills with cooked water. Supplement with cold congee. Avoid toxic foods.
Wu Jin San (Black Gold Powder)
Capillis Cinereus (Hair Ash): From men, no need to wash, burn to ash.
Cyprini Squamae Cinereus (Carp Scale Ash): Burn to ash.
Angelicae Sinensis Radix (Danggui): Roast, remove radicle.
Corydalis Rhizoma (Yanhusuo): Use raw.
Inkus Optimus (Hao Mo): Pass through sieve.
Cinnamomi Cortex (Rougui): Do not expose to fire, remove rough bark.
Draco Sanguis (Qilinjie).
Paeoniae Radix Rubra (Chishaoyao): Cut.
Carbonis Herbae (Baicaoshuang): Scrapings from the bottom of the pot.
Nine ingredients in equal parts, pound and sift into powder. Take two qian per dose, mixed with warm wine. Alternatively, decoct half a cup of young boy's urine with a piece of ginger, mix the powder into this decoction. Adding a little Moschus (Shexiang) is even better.
