Lu (82 years old): Suffered from short, frequent, and painful urination. Experienced dryness and thirst in the evening, worsening at midnight. The pulse indicated excess Yang and deficiency of Yin, undoubtedly due to a depletion of Kidney Water (Kidney Yin). Being of advanced age, he could not detach from worldly affairs; with numerous social engagements, his Five Zang Emotions remained unsettled, resulting in a syndrome where Yang fails to interact with Yin. The Classics state: "The Bladder is the official of the metropolitan area; when Qi transformation occurs, urine can be excreted." Treatment should focus on nourishing Yin to subdue Yang. Jin Gui Shen Qi Wan (Kidney Qi Pill from the Golden Cabinet) was prescribed, taken three qian morning and night.
Qian (34 years old): White turbidity in urine accompanied by pain, onset during late summer. Caused by Damp-Heat obstructing the orifices; treated with clearing heat and promoting diuresis.
Prescription: Bixie (Dioscorea Hypoglauca Rhizome), Bianxu (Polygonum Aviculare Herba), Qumai (Dianthus Superbus Herba), Chifuling (Poria Cocos with Red Cortex), Huashi (Talcum), Mutong (Akebia Trifoliata Caulis), Danzhuye (Lophatherum Herb).
Cured after four doses.
Gu (25 years old): Previously suffered from hematemesis (vomiting blood); recently developed Baiyin (white seminal emission or urethral discharge), occurring after urination. Pulse was rapid and small, especially faint at both Chi positions. He reported symptoms worsened with exertion. Treatment combined tonification and astringency.
Prescription: Dangshen (Codonopsis Radix), Huangqi (Astragali Radix), Shudi (Rehmanniae Radix Preparata), Yurou (Corni Fructus), Shanyao (Dioscoreae Oppositae Rhizoma), Wuweizi (Schisandrae Chinensis Fructus), Xianglian (Nelumbinis Semen), Qianshi (Euryales Semen).
Symptoms ceased after one dose; switched to pill form for maintenance, and hematemesis also resolved.
Shi (27 years old): Widowed in youth, leading to emotional depression, resulting in Zhoubi (generalized bi-syndrome/pain). Frequently experienced abdominal pain, difficulty walking, and significantly reduced appetite. Treatment required simultaneous soothing of Liver and tonification of Spleen.
Prescription: Dangshen (Codonopsis Radix, 2 qian), Zheng Baizhu (Steamed Atractylodis Macrocephalae Rhizoma, 2 qian), Cangzhu (Atractylodis Rhizoma, 1 qian), Chaihu (Bupleuri Radix, 0.5 qian), Xiangfu (Cyperi Rhizoma, 1.5 qian), Guishen (Angelicae Sinensis Radix, 1.5 qian), Yizhiren (Alpinia Oxyphylla Fructus, 0.7 qian), Jupi (Citri Reticulatae Pericarpium, 0.8 qian).
With modifications based on symptom changes, cured after forty doses.
Yu (75 years old): Atrophic pain in both knees, unable to stand or walk. Pulse was wiry and rapid. This was not caused by Phlegm coagulation or Qi stagnation; using drying and dispersing agents would be ineffective and exacerbate the condition. It stems from insufficient Yin and Blood in old age, restless Liver Fire, and tendons lacking nourishment. The disease belongs to Wei Syndrome (Atrophy Syndrome); nourishing Blood and harmonizing the Liver is the established method.
Prescription: Shudi (Rehmanniae Radix Preparata), Guishen (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba), Niuxi (Achyranthis Bidentatae Radix), Weilingxian (Saposhnikoviae Radix), Gouqi (Lycii Fructus), Fangji (Stephaniae Tetrandrae Radix), Bixie (Dioscorea Hypoglauca Rhizoma).
Qian (29 years old): Fullness and pain in the Middle Jiao, which subsequently transformed into Huangdan (Jaundice). Pulse was wiry, rapid, and choppy, indicating internal stagnation of Liver Fire. During the humid summer season, Damp-Earth was dominant; Damp-Heat steamed each other, impairing Spleen transport and transformation, thus forming this syndrome. Treatment required simultaneous clearing/detoxifying and soothing/dispersing.
Prescription: Chuanlian (Coptidis Rhizoma, 0.8 fen), Chaihu (Bupleuri Radix, 0.6 fen), Shanzhi (Gardeniae Fructus, 1.5 qian), Danpi (Moutan Cortex, 1.5 qian), Xiangfu (Cyperi Rhizoma, 1.5 qian), Jupi (Citri Reticulatae Pericarpium, 1 qian), Mutong (Akebia Trifoliata Caulis, 0.7 qian), Ychen (Artemisiae Scopariae Herba, 1.5 qian), Zexie (Alismatis Rhizoma, 1.5 qian).
After six prescriptions, jaundice subsided, chest fullness resolved, and the patient recovered.
