Although the treatment of women's diseases has its specialized branch, there are times when one cannot fully adhere to the methods of the Sages. Nowadays, families of high status reside in deep chambers, surrounded by curtains, and further wrap their arms with cotton cloth during pulse diagnosis. Consequently, they cannot employ the divine skill of observation (Wang), nor can they fully utilize the subtleties of pulse palpation (Qie). Among the four diagnostic methods—observation, listening/smelling, inquiry, and palpation—two are often lacking. The Yellow Emperor said: "In treating any disease, observe the patient's form, Qi, color, and luster. If form and Qi are harmonious, it is called treatable; if the color is floating and the pulse slippery, it is easily cured. If form and Qi are discordant, it is called difficult to treat; if the complexion is dull and lacks luster, it is called difficult to recover." It is also said: "The way to diagnose disease lies in observing a person's courage or timidity, bones, muscles, and skin, thereby knowing their deficiency or excess, which serves as the method of diagnosis." If a woman's pulse does not correspond to her illness, and the physician cannot see her physical form, relying solely on the pulse to prescribe medicine—is this feasible? In this sense, how can one exhaust the art of medicine? This is a common flaw in the world; physicians inevitably ask exhaustive questions, but patients, seeing the multitude of inquiries, and given that some physicians lack precision in their medical knowledge, often distrust the prescribed medicines. Such cases are numerous. Even Bian Que was not trusted when he observed the complexion of the Marquis of Qi, let alone those whom one cannot see at all. Alas! It can indeed be said to be very difficult.
The *Mai Jing* (Pulse Classic) states: "If the Cun Kou pulse is faint (Wei) and choppy (Se), faintness indicates insufficiency of Wei Qi (Defensive Qi), and choppy texture indicates depletion of Ying Qi (Nutritive Qi). Insufficiency of Wei Qi leads to shortness of breath and restlessness of the body. Deficiency of blood leads to bodily inversion/discomfort. When both Ying and Wei are deficient, speech becomes weak. If the Zu Yang pulse is faint and choppy, faintness indicates deficiency of Stomach Qi; deficiency leads to shortness of breath, dry throat, and bitter taste in the mouth. Choppy texture indicates loss of fluids. If the Shao Yin pulse is faint and slow, faintness indicates lack of essence; slowness indicates cold in the uterus. Choppy texture indicates stagnant blood leading to amenorrhea; this is called 'Ju Jing' (residing menstruation), occurring once every three months. A faint pulse with both blood and Qi deficiency in a young person indicates excessive blood loss. Amenorrhea during lactation is also called 'Ju Jing', occurring once every three months, with a faint, weak, and choppy pulse. For a young person, this indicates infertility; for a middle-aged person, it indicates sterility. If all six pulses are deep, wiry, fine, and rapid, the disease has taken root; Yuan Qi (Original Qi), Ying Qi, and Wei Qi are all deficient; even medication will be ineffective.
Formulas passed down through distant lineages are general formulas for gynecology. One formula can treat several conditions and should not be confined to specialized sub-fields. These were established by renowned sages and are commonly used effective formulas by the people. Although termed 'general,' one must not be rigid or dogmatic but must know the flexibility of modification according to circumstances. Only prescribing based on the specific condition can be considered miraculous.
(Tai Ping Hui Min He Ji Ju Fang) Si Wu Tang (Four Substances Decoction) is said to treat various women's diseases, but it is only applicable to blood disorders. If accompanied by Spleen and Stomach weakness, it is difficult to use. Why? Because Bai Shao Yao (White Peony Root) is sour and cold, capable of suppressing vital energy; Sheng Di Huang (Raw Rehmannia Root) is sweet and cloying, causing stagnation in the chest and diaphragm. For those with weak Spleen and Stomach, it further damages and weakens them, leading to blocked blood vessels and more severe menstrual obstruction. If Spleen and Stomach weakness is severe, use Si Jun Zi Tang (Four Gentlemen Decoction) alone to strengthen the Spleen. Once the Spleen is slightly strengthened, the two formulas can be combined into Ba Wu Tang (Eight Substances Decoction); taking it then will avoid bias or error. In the past, Zhang Shengdao used Si Wu Tang to treat hundred diseases in women, adding Wu Zhu Yu (Evodia Fruit) for decoction. For individuals with a Yang constitution, add less Wu Zhu Yu; for those with a Yin constitution, add more Wu Zhu Yu. This is the skillful use of Si Wu Tang. With Wu Zhu Yu, the cold and cloying nature of Bai Shao Yao and Di Huang cannot harm the Spleen and Stomach, is it not excellent?
Dang Gui (Radix Angelicae Sinensis) (2 qian), Bai Shao Yao (Radix Paeoniae Alba) (1 qian), Chuan Xiong (Rhizoma Chuanxiong) (7 fen), Sheng Di Huang (Radix Rehmanniae Recens). (*Yang Sheng Fang* uses Shu Di Huang; *He Ji Ju Fang* also uses it. *Ben Cao* states: Men should use cooked Rehmannia, women raw, because raw Rehmannia moves blood, so raw is appropriate here.)
Decoction methods and modifications are found in *Zhong Si Guang Yu*, Volume Middle.
