Bo Ren Hua treated a patient who suffered from chest fullness and distension, feeling restless as if palpitations (heart palpitations) were present, with dizziness and headache in the head and eyes, a desire to vomit but unable to do so, and sudden forgetfulness. At that time, one arm was partially paralyzed. The pulse at the Guan position (middle cun) above was slippery and smooth, deep and forceful upon pressing. This was diagnosed as accumulated fluid retention and stagnant phlegm obstructing the chest and diaphragm. This condition likely resulted from consuming rich flavors, strong wine, fatty foods, and roasted or fried items, leading to heat accumulation generating dampness; dampness gathering causes phlegm-fluids and retained drinks to ascend excessively. Wang Taipu stated: "When pathology is excessive in the upper part and does not cease, vomiting should be used to drain it." The treatment method should employ emesis. Waiting for the clear weather of spring, the treatment was performed according to this method by stimulating the throat with an object. Shortly after, large amounts of black, stubborn phlegm resembling maltose syrup (three to four sheng) were vomited. Two days later, it was repeated once more. After three or four times, the chest felt thoroughly clear and comfortable.
A case recorded in *Zhengzhuan*: A person developed a syndrome of fullness and obstruction due to hard labor in summer and autumn. Several physicians treated him with qi-regulating and dissipating herbs, all without effect. The pulses at the Guan positions on both hands were floating, surging, wiry, and choppy, while the pulses behind the Guan positions were deep and hidden. This indicated thick phlegm above the diaphragm, where the Qi of Spleen Earth was overly abundant and high, causing Liver Wood to be depressed and unable to extend. Emesis should be used, following the principle of "releasing Liver depression." However, during the winter month when Yang Qi descends, this method cannot yet be applied. Initially, herbs to resolve phlegm and soothe Liver Qi, and to drain the excessive Qi of Spleen and Stomach were given. Pingwei San (Regularize the Stomach Powder) was modified with Banxia (Pinellia Rhizome), Fuling (Poria), Qingpi (Green Tangerine Peel), Chuanxiong (Sichuan Lovage Rhizome), Caolongdan (Gentiana Scabra Root), Xiangfu (Cyperus Rhizome), Sharen (Amomum Fruit), Chaihu (Bupleurum Root), Huanglian (Coptis Rhizome), and Gualou (Trichosanthes Fruit). The condition improved by three or four tenths. By early February of the next year, Dao Cang Fa (Cow Stomach Emptying Therapy) was administered, resulting in recovery.
