The body surface feels cool, accompanied by pain and cold limbs. The pulse is deep, rapid, and forceful, (treated with Chengqi Tang.) Wang Bing stated: When the body is cold and there is cold extremities, yet the pulse is slippery and rapid, beating forcefully against the fingers upon palpation, this is named Yang Excess Rejecting Yin (Yang Sheng Ju Yin). For constipation, Da Chengqi Tang (Major Order the Qi Decoction) is used to purge.
[Formulas] Xiao Chengqi Tang (Minor Order the Qi Decoction) and Da Chengqi Tang.
Zhang Zhongjing said: Yang Du syndrome (Yang Toxin Syndrome) results from contracting malignant toxic qi from heaven and earth, entering the Yang meridians. Its initial symptoms include high fever, sore throat, skin eruptions resembling brocade patterns, restlessness, flushed face, delirium, and a pulse that is surging, slippery, rapid, and hurried. It is suitable to use (Yang Du Shengma Tang or Qingdai Yi Wu Tang.) In Yang Du syndrome, the pathogenic toxin contracted at the onset is severe. If sweating should have been induced but wasn't, or purging should have been performed but wasn't, or if emesis or purgation was misused; subsequently, pathogenic heat enters due to deficiency, or hot-natured medicines are mistakenly taken, causing toxic heat to scatter. This leads to all six pulses being surging, the tongue curled and scorched black, soot-like deposits in the nostrils, brocade-patterned spots on the body and face, delirious speech, running around aimlessly, climbing walls or roofs, singing while standing on high places, and walking about stripping off clothes. These are all its manifestations. It can be treated within five days, but after six or seven days, it becomes incurable. (Treated with Yang Du Shengma Tang.) For sore throat, (use Xuanshen Shengma Tang.) For severe heat, characterized by alternating mania and coma, locked jaw and clenched teeth, hallucinations of ghosts and spirits, vomiting of pus and blood, or vomiting up ingested medicine, purging is contraindicated; instead, use the Water Drenching Method (Shui Zi Fa).
[Dissipating Formula] Yang Du Shengma Tang.
[Emetic Formulas] Shengma Zhizi Tang and Xuanshen Shengma Tang.
[Water Drenching Method] If heat is severe, with alternating mania and coma, locked jaw and clenched teeth, purging is contraindicated. Use the Water Drenching Method. Wait until the jaw relaxes slightly and mental state stabilizes, then administer medication.
Yin Du syndrome (Yin Toxin Syndrome) occurs when severe cold pathogen is contracted initially, leading to an excess of Yin qi. After sweating, emesis, or purging, it transforms into Yin Du. The six pulses are deep and minute. There is colicky abdominal pain or spontaneous diarrhea, heavy limbs, throat discomfort, spontaneous sweating with nausea and vomiting, cyanotic lips and dark complexion, cold hands and feet, body pain as if beaten by a stick, shortness of breath and difficulty breathing. This is the presentation of Yin Du. It can be treated within three days, but after four or five days, it becomes difficult to treat. First, administer Gancao Tang (Licorice Decoction), Zhenwu Tang (True Warrior Decoction), and Fuzi Tang (Aconite Decoction). Then, moxa Qihai (CV 6) and Guanyuan (CV 4) acupoints for two to three hundred moxibustion units (zhuang), or apply scallion compression below the navel. Effectiveness is indicated by warming of the hands and feet and gradual return of the pulse. For cases of extreme cold reversal with testicular retraction, fumigation can be used, which has proven effective in restoring life.
Yin Du originates from Kidney Qi deficiency and cold, or injury from cold foods compounded by wind-cold invasion. Internally, there is latent Yin; externally, cold is contracted. Or, cold is contracted first, followed by latent Yin internally. With both internal and external conditions being Yin, Yang qi fails to maintain control, resulting in headache, heavy sensation in the lower back, abdominal pain, eye pain, bodily fatigue without significant fever, cold extremities, paroxysmal pain in the umbilical and abdominal region, deep and fine six pulses, sometimes rapid, with the Chi position being small, short, and minute, and the Cun position appearing large when strong. These are all its signs. Alternatively, there may be irritability with incessant spontaneous sweating, repetitive speech (Zhengsheng), and cyanosis of the fingernails and complexion.
[Yin-Rectifying and Yang-Restoring Formulas] Pili San, Zhengyuan San, Tianxiong San, and Fuzi San.
[Meridian-Warming Formulas] Zhenwu Tang, Sini Tang (Frigid Extremities Decoction), Baizhu Tang, and Rougui Tang.
[Phlegm-Seizing Formula] Jin Ye Dan.
[Compression Method] For acute abdominal pain, compress with salt stir-fried hot and wrapped in a scallion cake.
[Fumigation Method] For cold reversal and testicular retraction, throw stir-fried beans into hot vinegar and fumigate according to the method.
[Moxibustion Acupoints] Guanyuan (CV 4), Qihai (CV 6), Shenque (CV 8).
