For cases presenting neither exterior nor interior syndromes, provided they do not fit the indications for sweating (diaphoresis) or purging (catharsis), Xiao Chai Hu Tang (Minor Bupleurum Decoction) may be used. After administration, wait for other symptoms to manifest, then treat with modified Xiao Chai Hu Tang based on the specific presentation.
In cold damage (Shanghan), from the fourth or fifth day up to the thirteenth day, if the pathogen has passed beyond its original meridian stage (Guojing), and there are no exterior or interior syndromes, and purging is not yet indicated, if illness persists after taking Xiao Chai Hu Tang (Minor Bupleurum Decoction), and the stool becomes hard, (Da Chai Hu Tang (Major Bupleurum Decoction)) is indicated. If after six or seven days, the eyes appear unclear and the pupils are unresponsive, without exterior or interior syndromes, accompanied by fever and difficulty in defecation, even if the pulse is floating and rapid, (Da Chai Hu Tang (Major Bupleurum Decoction)) is indicated. In severe cases, (Da Cheng Qi Tang (Major Order the Qi Decoction)) is indicated.
After purging, if a rapid pulse persists and after six or seven days there is no defecation, this indicates blood stasis (Yuxue), and (Di Dang Tang (Resist the Decoction)) is indicated. A rapid pulse, polyphagia (excessive hunger), and no defecation for six or seven days also constitute a blood stasis syndrome.
When Shaoyin disease (Shaoyin Bing) first occurs, if there is contrary fever and the pulse is deep despite this, (Ma Huang Xi Xin Fu Zi Tang (Ephedra, Asarum, and Aconite Decoction)) should be used to induce slight sweating. If after two or three days, typical Shaoyin presentations occur without Yang heat signs, (Ma Huang Gan Cao Fu Zi Tang (Ephedra, Licorice, and Aconite Decoction)) should be used to induce slight sweating. These two conditions are not confined by exterior or interior classifications; this must be understood.
If a patient has high body heat but desires to wear clothes, it indicates heat at the superficial level (skin) and cold at the deep level (marrow/internal). Zhang Zhongjing did not provide a treatment method for this specific presentation; Yin Dan Tang (Yin Dawn Decoction) is suitable. Once the cold resolves, follow with Xiao Chai Hu Tang Jia Gui Zhi (Xiao Chai Hu Tang plus Cinnamon Twig) to warm the interior. The pulse will likely be deep and slow, with slightly cold extremities and diarrhea containing undigested food, indicating interior cold. Thus, fever can occur in Yin syndromes due to exterior heat.
If a patient feels intense cold but does not desire to wear clothes, it indicates cold at the superficial level and heat at the deep level. Zhang Zhongjing also did not provide a treatment method for this; first administer Bai Hu Jia Ren Shen Tang (White Tiger Decoction plus Ginseng). After the heat subsides, use Gui Zhi Ma Huang Ge Ban Tang (Half-Size Combination of Cinnamon Twig and Ephedra Decoction) to resolve the exterior. The pulse will likely be slippery, with cold extremities and a dry tongue. Thus, aversion to cold and fatigue in Shaoyin disease indicate exterior cold. Frequent inner restlessness without desire for thick clothing indicates interior heat, which may be treated with Da Chai Hu Tang (Major Bupleurum Decoction) via purging.
In general, Yin syndromes present with stillness, speech without voice, shortness of breath, unclear vision, abnormal nasal respiration, coarse breathing, incontinence of urine and feces after swallowing fluids, facial aversion to cold feeling like knife-cutting, cyanotic-black complexion, dull eyes, and floating deficient Yang causing malar flushing with non-vibrant blood color as distinguishing features. The body is heavy and difficult to turn, preferring to lie facing the wall, sleeping deeply, keeping eyes closed avoiding people, being reluctant to speak, speaking with weak breath and difficulty responding. Nasal breath may feel cold, voice may be indistinct. Symptoms may start mild and become severe, or alternate between dry heat/thirst and inability to drink. Lips may be cyanotic, purple, or white, lacking redness. Extremities are naturally cold, nails cyanotic, blood non-vibrant. Urine is white or pale yellow; stool is loose. Palpation of muscles reveals no significant heat. In severe Yin cold, the cold penetrates the hand. Although fever may be present, it differs greatly from Yang syndromes. One must not diagnose based solely on flushed face and thirst, mistaking it for a Yang syndrome; careful differentiation is essential. [Warming Methods] Si Ni Tang (Four Reversals Decoction), Zhen Wu Tang (True Warrior Decoction).
