左右上下，入锅悬起，重汤煮香一炷半，冷定取出，每服一小盏，随疾之上下，以定食前食
后，刻日取效，不可轻忽。

臣谨沭圣恩，叨居相职，节宣弗谨，遂染风疾，半身偏枯，手足拘挛，不堪行步，宣医延医，良剂屡投，
今越十载，并无寸效，乞归故里，广访名医，途至奉先驿，获遇异人，臣陈病状，蒙授一
方，臣根据方浸酒，未服之先，非人扶不能起，及饮一升，使手能梳头；服二升，手足屈伸有力；服三升，语
言行动如故；服四升，肢体通缓，百节康和，步履如飞，效难尽述，乞赐颁行天下，使黎庶
咸登寿域，谨录是方。随表拜进以闻。
防风（去芦，三两，治四肢骨节疼痛，浑身拘急）

出埋于土内，以出火毒；然后取用，每开坛，不可以面对坛口，恐药力冲伤眼目。每日饮二、
三次，尽量为度，毋令药力断续，其效如神。
神仙延寿药酒丹
治久近风邪，左瘫右痪，语言謇涩，手足拘挛，紫白癜风，风寒暑湿，四气交攻，身体虚羸，腰疼膝痛，
耳聋眼 ，下部诸虚，及女子经血不调，脐腹绞痛，胸膨胁胀，呕吐恶心，子宫虚冷，赤
白带下，一切诸疾，皆有神验。此酒互相等制，其性和缓，其味甘香，能追万病，善补诸虚，和胃养丹田，益
精壮筋骨，安和五脏，定魄宁魂，返老还童，延年绵算，病可尽驱，效难罄笔。

晕耳鸣，口苦无味，憎寒毛疏， 癖瘕块，男子阳衰肾冷，女子绝产阴虚，俱能奏效。
锦纹大黄（五两，酒浸，九蒸，九晒） 麻仁（微炒，去壳，二两） 郁李仁（去皮、
壳，泡，二两） 枳壳（麸炒，二两） 干山药（酒蒸，二两） 独活（一两） 山茱萸（酒
蒸，去核，二两） 槟榔（二两） 菟丝子（一两） 车前子（炒，二两半） 牛膝（酒浸，晒干，二两）
上为极细末，炼蜜为丸如桐子大。每服八十丸，白汤送下，早晚各一服。久患肠风脏毒，服之除根。瘫痪
语涩，服之平复。酒后能进一服，宿酒尽消。中年以后之人，过用浓味、酒肉，多有痰火，且不能远房事，
往往致阴虚火动，火助则生风，所谓一水不能胜五火也，故以此方疏风降火为主，不问年高气弱，并宜服之，惟
孕妇忌服。
天麻丸 治风因热而生，热胜则风动，宜以静胜其躁，此药能滋阴抑火，行荣卫，壮筋骨。

料，此圣人不治已病治未病也，或先服竹沥枳术丸及搜风顺气丸，何中风之有？

祛风药酒〔批注〕（按：此方治疗各种风症导致的瘫痪、肿痛、顽固性麻木者，属于祛风败毒之剂）

将上述药物共同研成细末，每取药末三钱，用小黄米烧酒一斤，大枣三枚，蜂蜜五钱，一同放入瓶内，上面盖住小盏，用面糊封口固紧，用麻绳在左右上下扎紧，放入锅中隔水加热蒸煮香一炷半的时间，冷却后取出，每次服用一小盏，根据病位的上下，决定是饭前还是饭后服用，按时日见效，不可轻视。

臣恭敬地沐浴皇恩，担任宰相职务，因起居调摄不慎，于是染上风疾，半身偏枯，手足拘挛，无法行走，请医延医，多次投用良药，至今已过十年，并无丝毫疗效，请求回归故里，广泛寻访名医，途中经过奉先驿，遇到异人，臣陈述病情，蒙受传授一方，臣依照此方浸泡药酒，未服用之前，非人搀扶不能起身，等到饮下一升，使手能梳头；服用二升，手足屈伸有力；服用三升，言语行动如常；服用四升，肢体通畅舒缓，全身关节康泰调和，步履如飞，疗效难以尽述，乞求赐颁天下，使百姓都能长寿安康，谨记录此方。随奏表拜进以闻知。

Crush the above herbs, add five slices of Shengjiang (Fresh Ginger Rhizome), two zhong of water, decoct until eight parts remain, and take warm before meals.
Beimu Gualou Tang [Note] (According to this prescription for treating hemifacial paralysis with mouth deviation in stroke, it is a conditioning agent.) It treats stroke in obese persons, regardless of left or right side, all treated as phlegm syndrome.
Beimu (remove heart), Gualou Ren (remove oil), Tiannanxing (processed), Jingjiesui, Fangfeng (remove rhizome head), Huangbai (remove rough bark), Qianghuo, Huangqin, Huanglian, Baizhu, Jupi (remove white pith), Bo Gui, Banxia (soaked seven times), Weilingxian, Tianhuafen, Gancao (equal parts each).
Crush the above herbs, add three slices of Shengjiang (Fresh Ginger Rhizome), decoct with water, add Yizhuli (Bamboo Juice) one small zhong, and take.
Michuan Shunqi San (Transmitted by Sun Jiantang) Treats various wind syndromes causing mouth deviation and hemiplegia, left flaccid paralysis and right hemiparesis. Take three to five doses first, then proceed to taking wind-dispelling medicinal wine.
Qingpi, Chenpi, Zhike, Jiegeng, Wuyao, Renshen (remove rhizome head), Baizhu, Fuling, Banxia (processed), Chuanxiong, Baizhi, Xixin, Mahuang (remove joints), Fangfeng (remove rhizome head), Ganjiang, Jiangcan (stir-fried), Gancao, Qinjiao (remove rhizome head), Qianghuo, Duhuo (equal parts each).
Crush the above herbs, add three slices of Shengjiang (Fresh Ginger Rhizome), two zhong of water, decoct until eight parts remain, and take warm on an empty stomach.
Qufeng Yaojiu [Note] (According to this prescription for treating various wind syndromes causing paralysis, swelling, pain, and stubborn numbness, it is a wind-dispelling and toxin-resolving agent.)
Fangfeng (remove rhizome head), Jingjiesui, Cangzhu (soaked in rice wash water), Mahuang (do not remove joints), Xixin, Tianma, Baizhi, Chuanxiong, Danggui, Banxia (processed), Fuling, Jiangcan, Chuanwu (soaked in child's urine), Caowu, Luoyanghua, Baihuashe (equal parts each).
Grind the above medicines into fine powder. For every three qian of medicine, use one jin of Shaojiu (Shaoxing Rice Wine) made from yellow millet, three dates, and five qian of honey. Put them together into a bottle, cover with a small cup, seal tightly with flour paste, tie securely with hemp rope around top, bottom, and sides. Suspend in a pot and steam over boiling water for the time it takes to burn one incense stick. After cooling, remove. Take one small cup per dose. Depending on whether the disease is upper or lower body, decide whether to take before or after meals. Expect efficacy according to days; do not neglect this.

This subject respectfully bathes in the Holy Grace, holding the position of Prime Minister. Due to lack of care in daily regulation, I contracted wind disease, resulting in hemiplegia, contracture of limbs, inability to walk. Physicians were summoned, good remedies repeatedly administered, yet ten years have passed without the slightest effect. I petitioned to return to my hometown to widely seek famous doctors. On the way, at Fengxian Post Station, I met a strange person. I stated my condition and was graciously taught one prescription. Following the prescription to soak medicinal wine, before taking it, I could not rise without support. After drinking one sheng, my hands could comb hair; after two shengs, limbs could bend and extend with strength; after three shengs, speech and movement returned to normal; after four shengs, limbs became relaxed and harmonious, joints healthy, walking swift. Efficacy is hard to fully describe. I beg to grant this to the world so that common people may attain longevity. Respectfully recording this prescription, I submit it with the memorial.

Biejia (Turtle Shell) (1 liang, nine-ribbed or seven-ribbed are best, treats paralysis) Danggui (2 liang, treats blood deficiency and nourishes blood)
Cangerzi (4 liang, crushed, expels wind-damp and stubborn numbness in joints)
Wancansha (Late Silkworm Sand) (2 liang, stir-fried until yellow, treats paralysis with joint immobility and stubborn limb numbness)
Gouqizi (5 liang, baked, treats wind pathogens in five zang organs) Yousongjie (Oil Pine Joint) (2 liang, crushed, strengthens bones and tendons)
Ganqiegen (Dry Eggplant Root) (8 liang, steamed over rice until cooked, treats inability to flex and extend joints)
Baizhu (2 liang, remove rhizome head, stir-fried in earth, tonifies spleen and stomach) Duzhong (2 liang, stir-fried with ginger juice, remove silk fibers, tonifies waist and knees)
Finely chop the above, use 35 jin of good wine. Place herbs in a raw silk bag, suspend inside, seal tightly. After fourteen days, place the jar in a pot, boil over water several times. Remove and bury in soil to eliminate fire toxicity. Then use. When opening the jar, do not face the mouth, lest the medicinal power injure the eyes. Drink two or three times daily, to the point of intoxication. Do not let the medicinal power be interrupted. Its efficacy is miraculous.
Shenxian Yanshou Yaojiudan
Treats long-standing or recent wind pathogens, left flaccid paralysis and right hemiparesis, slurred speech, limb contracture, purple and white wind urticaria, wind-cold-wind-heat dampness, attack of four qi, emaciation, lower back and knee pain, deafness and blurred vision, deficiency in lower jiao, irregular menstruation in women, colic in umbilical and abdominal regions, chest distension and hypochondriac fullness, vomiting and nausea, uterine coldness, red and white leukorrhea. All these diseases respond miraculously. This wine is mutually prepared, its nature is mild, its taste sweet and fragrant. It can chase away myriad diseases, well-tonify various deficiencies, harmonize stomach and nourish dantian, benefit essence and strengthen bones and tendons, calm and harmonize five zang organs, stabilize soul and spirit, reverse aging, prolong life, drive away illness completely. Efficacy is hard to write down.
Renshen (remove rhizome head), Baizhu (earth-fried), Gancao (roasted), Baifuling (each 3 liang), Danggui, Chuanxiong, Baishao Yao (stir-fried), Shengdihuang (stir-fried with ginger juice), Shudihuang, Gouqizi, Rourongrong (washed in wine), Heshouwu (soaked in rice wash water), Niuxi (remove rhizome head), Tianmendong (remove heart), Maimendong (remove heart), Sharen (stir-fried, each 2.5 liang), Chuanjiao (remove stems and eyes), Chuanwu (remove skin and navel), Caowu (round ones, soaked), Wuyao (each 1 liang), Wujiapi, Hujinggu (Tiger Tibia Bone) (roasted with butter), Zhike (stir-fried), Ganjiang (soaked), Houpo (stir-fried with ginger juice), Chenpi (remove white pith), Chenxiang, Huixiang (stir-fried with salt wine), Xiangfu (soaked in child's urine, stir-fried), Qianghuo, Duhuo, Fangfeng (remove rhizome head), Baizhi, Mahuang (do not remove joints), Xixin (washed in wine), Banxia (processed), Cangzhu (soaked in rice wash water, stir-fried), Wuweizi, Poguzhi (stir-fried), Gui (each 2 liang), Hongzao (remove pits), Suyou (butter oil), Fengmi (honey, each half liang), Hutao Rou (walnut meat, soaked in hot water, remove skin).
Chop together, place in silk bag. Use one jar of Shaojiu (Shaoxing Rice Wine), soak for three days and nights. Place in pot, steam over water for about three hours. Remove and bury in soil to vent fire toxicity. Drink one or two cups daily. Depending on upper or lower disease, decide on empty stomach or after meals. When wine is nearly finished, dry the medicinal residue, grind to powder, make pills with wine like wutong seeds. Take thirty pills on empty stomach with wine.
Lujiaoshuang Wan [Note] (According to this prescription for treating various wind syndromes causing paralysis belonging to deficiency-cold, it is a warming and tonifying agent.) Treats hemibody weakness or those unable to walk for two or three years.

Grind into extremely fine powder. Make pills with wine like wutong seeds. Take one hundred pills on empty stomach with rice soup.
Jianbu Huqian Wan (Secret Prescription) [Note] (According to this prescription for treating various wind syndromes causing paralysis, tongue stiffness belonging to deficiency-heat, it is a nourishing and tonifying agent.) Treats stroke with left flaccid paralysis and right hemiparesis, slurred speech.

With Liuqu (Medicated Leaven) 6 liang, Shengjiangzhi (Ginger Juice) 1 zhong, Yizhuli (Bamboo Juice) 1 bowl, Zhu Jingsui (Pig Spinal Cord) 5 strips, refine honey and mix to form pills like wutong seeds. Take one hundred pills per dose, on empty stomach with light salt water.
Zhuli Zhizhu Wan Resolves phlegm and clears fire, regulates stomach and harmonizes spleen. Suitable for obese, pale, qi-deficient persons to take this medicine to prevent falling and fainting. Extremely divine and wonderful.

Grind into extremely fine powder. With Liuqu (Medicated Leaven) 6 liang, Shengjiangzhi (Ginger Juice) 1 zhong, Yizhuli (Bamboo Juice) 1 bowl, cook paste to form pills like wutong seeds. Take one hundred pills daily on empty stomach with ginger water.
Sofeng Shunqi Wan
Treats various wind syndromes causing flaccidity and bi-syndrome, hemiplegia, mouth deviation, difficulty speaking, muscle numbness, vertigo and tinnitus, bitter taste and loss of flavor, aversion to cold and sparse hair, masses and accumulations. In men, yang deficiency and cold kidneys; in women, infertility and yin deficiency. All can achieve efficacy.
Jinwen Dahuang (Brocade-patterned Rhubarb) (5 liang, soaked in wine, nine steams, nine sun-dryings), Maren (lightly stir-fried, remove shell, 2 liang), Yuliren (remove skin and shell, soaked, 2 liang), Zhike (bran-stir-fried, 2 liang), Ganshanyao (wine-steamed, 2 liang), Duhuo (1 liang), Shanzhuyu (wine-steamed, remove pit, 2 liang), Binglang (2 liang), Tiansizi (1 liang), Cheqianzi (stir-fried, 2.5 liang), Niuxi (wine-soaked, sun-dried, 2 liang).
Grind into extremely fine powder. Refine honey to form pills like wutong seeds. Take eighty pills per dose with plain boiled water, morning and evening. For long-term intestinal wind and visceral toxin, it removes the root. For paralysis and slurred speech, it restores normalcy. One dose after drinking alcohol eliminates hangover. Persons past middle age who consume rich flavors, alcohol, and meat often have phlegm-fire and cannot abstain from sexual activity, often leading to yin deficiency and fire hyperactivity. Fire aids wind generation, described as "one water cannot overcome five fires." Thus, this prescription focuses on dispersing wind and descending fire. Suitable for all regardless of age and weak qi, except pregnant women.
Tianma Wan Treats wind generated by heat; when heat prevails, wind moves. Stillness should overcome restlessness. This medicine can nourish yin and suppress fire, circulate Ying and Wei, and strengthen bones and tendons.

When a person initially feels numbness and insensitivity in the thumb and index finger, or slight weakness in hands and feet, or slight muscle twitching, this is a precursor to stroke. One should take Yufeng Tang and Tianma Wan, one dose each. This is what sages mean by treating disease before it arises, rather than after it has occurred. Or first take Zhuli Zhizhu Wan and Sofeng Shunqi Wan. How could stroke occur?
