Ba Wei Shun Qi San (Eight-Ingredient Qi-Regulating Powder) [Commentary: According to this prescription, for stroke caused by Qi obstruction, first administer this Qi-regulating agent.] For all strokes, first take this medicine to regulate Qi, then treat the wind.
Ren Shen (Ginseng Radix et Radix, de-gorged) Pian Bai Zhu (Stir-baked Atractylodis Macrocephalae Rhizoma) Bai Fu Ling (Poria) Bai Zhi (Angelicae Dahuricae Radix) Qing Pi (Citri Reticulatae Pericarpium Viride) Chen Pi (Citri Reticulatae Pericarpium, de-albedo) Wu Yao (Linderae Radix) (each 2 qian) Gan Cao (Licorice Radix et Rhizoma, 1 qian)
The above herbs are chopped, divided into two doses. Each dose is decocted with 2 zhong of water until 8 fen remains, filtered to remove dregs, and taken after meals. Alternatively, add Nan Xing (Arisaematis Rhizoma) and Mu Xiang (Aucklandiae Radix) to awaken phlegm and regulate Qi. If phlegm is abundant, add Ban Xia (Pinelliae Rhizoma, 2 qian) and Sheng Jiang (Zingiberis Rhizoma Recens, 3 slices).
Xiao Xu Ming Tang (Minor Continuation of Life Decoction) [Commentary: According to this prescription, for wind striking the fu-organs, it is a surface-releasing agent.]
Treats sudden severe stroke, loss of consciousness, hemiplegia, deviation of the mouth and eye, trembling of hands and feet, dysarthria, limb numbness, mental confusion and dizziness, blurred vision, copious sputum and saliva, tendon spasm, and weakness of the legs and feet making flexion and extension impossible. It treats exterior manifestations of the six meridians; thus, this formula is modified to release the exterior.
Fang Feng (Saposhnikoviae Radix, 2 qian) Ma Huang (Ephedrae Herba, de-jointed) Xing Ren (Armeniacae Semen Amarum, soaked, de-skin and de-pit) Bai Shao Yao (Paeoniae Radix Alba) Rou Gui (Cinnamomi Cortex) Chuan Xiong (Ligustici Chuanxiong Rhizoma)

The above herbs are chopped, with 5 slices of Sheng Jiang (Zingiberis Rhizoma Recens) and 2 zhong of water, decocted to 1 zhong, and taken warm.
For all strokes, if one does not examine the six meridians for modifications, although treatment is attempted, the pathogenic factor cannot be eliminated. The *Neijing* states: "When pores open, there is shivering cold; when closed, there is fever and oppression." Knowing it is acute wind-stroke, one should first use Xu Ming Tang modified according to symptoms.
Taiyang Wind Stroke, without sweat and aversion to cold, is primarily treated with Ma Huang Xu Ming Tang. Based on this formula, double the dosage of Ma Huang (Ephedrae Herba), Fang Feng (Saposhnikoviae Radix), and Xing Ren (Armeniacae Semen Amarum). Taiyang Wind Stroke, with sweat and aversion to wind, is primarily treated with Gui Zhi Xu Ming Tang. Based on this formula, double the dosage of Gui Zhi (Cinnamomi Ramulus), Shao Yao (Paeoniae Radix), and Xing Ren (Armeniacae Semen Amarum). Yangming Wind Stroke, with sweat, body heat, and no aversion to cold, is primarily treated with Bai Hu Xu Ming Tang. Based on this formula, double the dosage of Gan Cao (Licorice Radix et Rhizoma), and add Shi Gao (Gypsum Fibrosum) and Zhi Mu (Anemarrhenae Rhizoma), each 1 qian. Yangming Wind Stroke, with sweat, body heat, and no aversion to wind, is primarily treated with Ge Gen Xu Ming Tang. Based on this formula, double the dosage of Gui Zhi (Cinnamomi Ramulus) and Huang Qin (Scutellariae Radix), and add Ge Gen (Puerariae Lobatae Radix), 1 qian. Taiyin Wind Stroke, without sweat and cool body, is primarily treated with Fu Zi Xu Ming Tang. Based on this formula, double the dosage of Fu Zi (Aconiti Lateralis Radix Preparata). When six types of wind stroke are confused, attributed to Shaoyang and Jueyin, with joint spasms or numbness and insensitivity, Qiang Huo Lian Qiao Xu Ming Tang (Notopterygium and Forsythia Continuation of Life Decoction) is suitable. Take 8 qian of Xiao Xu Ming Tang, plus Qiang Huo (Notopterygii Rhizoma et Radix, 2 qian) and Lian Qiao (Forsythiae Fructus, 3 qian). General rule: Add Shi Gao (Gypsum Fibrosum), Zhi Mu (Anemarrhenae Rhizoma), and Huang Qin (Scutellariae Radix) in spring and summer; add Rou Gui (Cinnamomi Cortex), Fu Zi (Aconiti Lateralis Radix Preparata), and Shao Yao (Paeoniae Radix) in autumn and winter. If there is heat, remove Hei Fu Zi (Black Prepared Aconite) and add Bai Fu Zi (White Prepared Aconite).
For tendon spasms, slow speech, and wiry pulse, add Yi Yi Ren (Coicis Semen). If tendons are tight, add Ren Shen (Ginseng Radix et Radix), remove Huang Qin (Scutellariae Radix) and Shao Yao (Paeoniae Radix) to avoid middle-cold; after slight relief from medication, add Dang Gui (Angelicae Sinensis Radix). For irritability and constipation, remove Fu Zi (Aconiti Lateralis Radix Preparata) and Rou Gui (Cinnamomi Cortex), double Shao Yao (Paeoniae Radix), and add Zhu Li (Bamboo Juice). For body pain, add Qiang Huo (Notopterygii Rhizoma et Radix); also add for convulsions. For irritability and frequent fright, add Xi Jiao (Rhino CeratUs) and Ling Yang Jiao (Saiga Tataricae Cornu). For anger, add Ling Yang Jiao (Saiga Tataricae Cornu). For confusion and erroneous speech, add Fu Shen (Poria cum Pinodi) and Yuan Zhi (Polygalae Radix). For inability to speak, add Zhu Li (Bamboo Juice). For aphonia, add Shi Chang Pu (Acori Tatarinowii Rhizoma). For headache as if breaking, add Qiang Huo (Notopterygii Rhizoma et Radix). For bone and joint pain, indicating cold-dampness, double Fu Zi (Aconiti Lateralis Radix Preparata) and Rou Gui (Cinnamomi Cortex). For vomiting and abdominal distension, add Ren Shen (Ginseng Radix et Radix) and Ban Xia (Pinelliae Rhizoma). For weak knees and ankles, add Niu Xi (Achyranthis Bidentatae Radix) and Shi Hu (Dendrobii Caulis). For lower back pain, add Tao Ren (Persicami Fructus, de-skin and de-pit), Du Zhong (Eucommiae Cortex), and Ginger-fried preparation. For insomnia, add Suan Zao Ren (Ziziphi Spinosae Semen). For excessive phlegm, add Tian Nan Xing (Arisaematis Rhizoma). For obese individuals with dampness, add Wu Tou (Aconiti Radix) and Fu Zi (Aconiti Lateralis Radix Preparata) to unblock meridians, boiled in Tong Bian (Child's Urine) to reduce toxicity and assist downward movement; adding salt is excellent. For cold-viscera diarrhea, remove Fang Ji (Stephaniae Tetrandrae Radix) and Huang Qin (Scutellariae Radix), double Fu Zi (Aconiti Lateralis Radix Preparata), and add Bai Zhu (Atractylodis Macrocephalae Rhizoma). For singing, laughing, crying, delirious speech, add Bai Zhu (Atractylodis Macrocephalae Rhizoma), double Ma Huang (Ephedrae Herba), Ren Shen (Ginseng Radix et Radix), and Gui Zhi (Cinnamomi Ramulus).
For spontaneous sweating, remove Ma Huang (Ephedrae Herba) and Xing Ren (Armeniacae Semen Amarum), add Bai Zhu (Atractylodis Macrocephalae Rhizoma).
San Hua Tang (Three Transformations Decoction) [Commentary: According to this prescription, for wind striking the zang-organs, it is a purgative agent.]
Treats stroke with exterior manifestations of the six meridians; first treat with modified Xu Ming Tang according to symptoms. If there is internal obstruction of bowel and bladder movements, use this medicine to guide discharge.
Hou Po (Magnoliae Officinalis Cortex, ginger-juice fried) Qiang Huo (Notopterygii Rhizoma et Radix) Da Huang (Rhei Radix et Rhizoma) Zhi Shi (Immature Bitter Orange, equal parts)
The above herbs are chopped, with 3 slices of ginger and 2 zhong of water, decocted to 1 zhong, taken warm. Stop when bowels move freely; if not, administer again. If internal pathogens are removed and external pathogens exhausted, proceed with Yu Feng Tang (Wind-Dispelling Decoction) to follow the middle way, modifying as needed. Over time, clear and turbid will separate naturally, Ying and Wei will harmonize, and major diseases will disappear.
Da Qin Jiao Tang (Major Gentian Macrophylla Decoction)
Treats stroke without exterior manifestations of the six meridians and without internal obstruction of bowel and bladder. Knowing that blood deficiency fails to nourish the tendons, causing immobility of hands and feet and stiff tongue preventing speech, one should nourish blood so that tendons become naturally prosperous.
Qin Jiao (Gentianae Macrophyllae Radix) Gan Cao (Licorice Radix et Rhizoma) Dang Gui (Angelicae Sinensis Radix) Chuan Xiong (Ligustici Chuanxiong Rhizoma) Bai Shao Yao (Paeoniae Radix Alba) Shi Gao (Gypsum Fibrosum) Du Huo (Angelicae Pubescentis Radix) Bai Fu Ling (Poria, each 1 qian) Qiang Huo (Notopterygii Rhizoma et Radix) Fang Feng (Saposhnikoviae Radix) Huang Qin (Scutellariae Radix) Bai Zhi (Angelicae Dahuricae Radix) Bai Zhu (Atractylodis Macrocephalae Rhizoma) Shu Di Huang (Rehmanniae Radix Preparata) Sheng Di Huang (Rehmanniae Radix, each 0.5 qian) Xi Xin (Asari Radix, 0.3 qian) For subcardiac fullness, add Zhi Shi (Immature Bitter Orange, 1 qian).
The above herbs are chopped, with Zhu Li (Bamboo Juice), Jiang Zhi (Ginger Juice), 3 slices of ginger, and 2 zhong of water, taken at any time.
Qiang Huo Yu Feng Tang (Notopterygium Wind-Dispelling Decoction) [Commentary: According to this prescription, for wind striking the meridians, it is a blood-regulating and nourishing agent.]
Treats Liver and Kidney deficiency, weak sinews and bones, dysarthria, mental confusion, wind-damp invading deficiency entering meridians. In thin people, unilateral hemiplegia; in fat people, half-body paralysis. Generally, heart labor generates hundred diseases; heart stillness extinguishes ten thousand evils. This medicine calms the mind and nourishes spirit, regulates Yin and Yang, prevents excess, treats stroke without interior or exterior pathogens, and follows the middle way.
Qiang Huo (Notopterygii Rhizoma et Radix) Gan Cao (Licorice Radix et Rhizoma) Man Jing Zi (Vittariae Fructus) Fang Feng (Saposhnikoviae Radix, de-gorged) Chuan Xiong (Ligustici Chuanxiong Rhizoma) Xi Xin (Asari Radix, de-radicle) Zhi Ke (Aurantii Fructus Immaturus, bran-fried)

The above herbs are chopped, with 2 zhan of water, decocted to 1 zhan, taken warm. On rainy days, add 3 slices of ginger.
Fang Feng Tong Sheng San (Saposhnikovia Unblocking Hero Powder)
Treats all wind-heat from stroke, dry constipation, red and scanty urination, sores on head and face, red and painful eyes; or extreme heat generating wind, stiff tongue and locked jaw; or purple-red wind-thorns and urticaria on nose, constituting Lung Wind; or becoming Feng Li (commonly called Great Wind); or Intestinal Wind causing hemorrhoids and fistulas; or intestinal depression causing various heats, delirium, and mania. All can be regulated.
Fang Feng (Saposhnikoviae Radix) Chuan Xiong (Ligustici Chuanxiong Rhizoma) Dang Gui (Angelicae Sinensis Radix) Bai Shao Yao (Paeoniae Radix Alba) Da Huang (Rhei Radix et Rhizoma) Mang Xiao (Natrii Sulfas) Lian Qiao (Forsythiae Fructus) Ma Huang (Ephedrae Herba, not de-jointed)

Hua Shi (Talcum, 2.4 qian) Gan Cao (Licorice Radix et Rhizoma, honey-fried, 1 qian)
The above herbs are chopped, with 3 slices of ginger and 2 zhan of water, decocted to 1 zhan, taken warm.
Sweating during labor while exposed to wind causes wind accumulation due to sitting fatigue, emerging from Xuan Fu (pores), congealing lipid fluid. Remove Mang Xiao (Natrii Sulfas), double Shao Yao (Paeoniae Radix) and Dang Gui (Angelicae Sinensis Radix) to disperse wind from Xuan Fu and regulate Ying and Wei. Commonly called Wind Thorns. Urticaria, red or white, uses Ma Huang (Ephedrae Herba), Yan Chi (Fermented Soybean), and Cong Bai (Allii Fistulosi Bulbus) to induce sweat. De-joint Ma Huang and remove Mang Xiao (Natrii Sulfas); salty flavors enter blood and coagulate internally, thus not inducing sweat. Also add Si Wu Tang (Four Substances Decoction) and Huang Lian Jie Du Tang (Coptis Detoxification Decoction) to the previous formula, taking all three together twice daily. Thus, the *Neijing* says: "Use bitter to disperse," meaning heat is in the muscle layer connecting to the interior. For dribbling and difficult urination, remove Ma Huang (Ephedrae Herba), add Hua Shi (Talcum) and Lian Qiao (Forsythiae Fructus), decoct and mix with Mu Xiang (Aucklandiae Radix) powder, 2 qian.
Ma Huang (Ephedrae Herba) governs the exterior and is unsuitable for interior conditions, hence removed. For migrating pain in waist and hypochondrium, add Mang Xiao (Natrii Sulfas), Shi Gao (Gypsum Fibrosum), Dang Gui (Angelicae Sinensis Radix), and Gan Cao (Licorice Radix et Rhizoma), each 2 qian, mixed with Che Qian Zi (Plantaginis Semen) powder and Hai Jin Sha (Lygodii Spora) powder, each 1 qian. The *Neijing* says: "The waist is the mansion of the Kidney." For tetanus, if in the exterior, scatter with pungent flavors; if in the interior, purge with bitter flavors, combined with dispersing. After sweating and purging, to unblock Qi and Blood and dispel wind pathogens, add Jing Jie Sui (Schizonepetae Spica) and Da Huang (Rhei Radix et Rhizoma), each 2 qian per liang of herb, mixed with Quan Xie (Scorpio) powder and Qiang Huo (Notopterygii Rhizoma et Radix) powder, each 1 qian.
For various wind tremors, pediatric acute and chronic convulsive disorders, constipation, violent pathogenic heat, dry intestines and stomach, upward staring and teeth grinding, night sweats and sleep-talking, muscle twitching and palpitations, add Da Huang (Rhei Radix et Rhizoma), 1 qian, and Zhi Zi (Gardeniae Fructus), 2 qian per liang, mixed with Fu Ling (Poria) powder, 2 qian. For muscle twitching, mix with Qiang Huo (Notopterygii Rhizoma et Radix) powder, 1 qian. The Classic says: "Muscle twitching is named Mild Wind." Wind injuring the Lung, cough and asthma, add Ban Xia (Pinelliae Rhizoma) and Fu Ling (Poria), each 2 qian per liang. For trauma and injury, joint pain, and stagnant evil blood in abdomen not descending, add Dang Gui (Angelicae Sinensis Radix) and Da Huang (Rhei Radix et Rhizoma), each 3.5 qian per liang, mixed with Ru Xiang (Frankincense) and Mo Yao (Myrrh), each 2 qian. To resolve seasonal cold damage and internal/external injuries, add Yi Yuan San (Beneficial Prime Powder), 1 liang, Cong Bai (Allii Fistulosi Bulbus), 10 stems, Dou Chi (Fermented Soybean), 1 he, Sheng Jiang (Zingiberis Rhizoma Recens), 5 qian, with 1 large bowl of water, decoct to 8 fen. First take half warm, probe throat with chopsticks to induce vomiting. After vomiting, take remaining half hot; sweat releases immediately. For alcohol-induced stroke with body heat and headache, add Huang Lian Xu (Coptidis Rhizoma Stamen), 2 qian, and Jin Bai (likely a typo for Cong Bai or similar), 10 stems. Do not use Gui Zhi Tang (Cinnamon Twig Decoction) or Ma Huang Tang (Ephedra Decoction) to resolve. For vertigo and nasal congestion with turbid discharge, add Bo He (Menthae Haplocalycis Herba) and Huang Lian (Coptidis Rhizoma), each 2.5 qian per liang. The *Neijing* says: "When Gallbladder heat transfers to the brain, there is frontal pain and Bi Yuan (nasal sinusitis), with continuous turbid discharge." Wang Bing says: "Gallbladder fluid descends clearly, causing continuous turbid discharge like a spring, hence called Bi Yuan." This refers to flourishing of Foot-Taiyang and Foot-Yangming meridians. For rebellious Qi, take Mu Xiang (Aucklandiae Radix) powder, 1 qian.
For carbuncles, sores, swelling, toxins, and all malignant ulcers, take 1 liang of this formula, double Lian Qiao (Forsythiae Fructus) and Dang Gui (Angelicae Sinensis Radix), add Huang Lian (Coptidis Rhizoma), Fu Ling (Poria), Mu Xiang (Aucklandiae Radix), Ren Shen (Ginseng Radix et Radix), Bai Zhi (Angelicae Dahuricae Radix), Jin Yin Hua (Lonicerae Japonicae Flos), Mu Li (Ostrea Shell), and Huang Qi (Astragali Radix), each 0.5 fen. Named Sheng Huang Yin Zi (Superior Yellow Decoction). If sores are upper, add Dang Gui (Angelicae Sinensis Radix) soaked in wine. For macular heat, add Huang Lian (Coptidis Rhizoma), 5 qian.

Ma Huang (Ephedrae Herba) Da Huang (Rhei Radix et Rhizoma) Mang Xiao (Natrii Sulfas) Xi Xin (Asari Radix, each 5 qian) Dang Gui (Angelicae Sinensis Radix) Chuan Xiong (Ligustici Chuanxiong Rhizoma, each 2.5 liang) Gan Cao (Licorice Radix et Rhizoma) Zhu Sha (Cinnabar, each 2 liang, as coating) Huang Lian (Coptidis Rhizoma) Huang Bo (Phellodendri Cortex, each 6 qian) Hua Shi (Talcum, 3 liang)
