1. When the smallpox pustules are scabbing and falling off, if the color turns pale and emaciated, presenting a purple grape-like hue, within one or two days, the scabs will begin to form around the mouth and nose, receding from the abdomen down to both legs. However, if the forehead and feet scab simultaneously, healing after the skin peels off, this is an auspicious sign; there is no need for alarm or suspicion, nor should purgative medicines be administered.
2. When the smallpox pustules are scabbing and falling off, if the entire body is putrid and rotting, appearing as if stitched together and unapproachable due to foul odor, and the eyes lack spirit, death is certain.
3. When the smallpox pustules are scabbing and falling off, if the entire body itches, scratching yields no pus, and the skin curls up like dry bean pods, death is certain.
4. When the smallpox pustules are scabbing and falling off, if chills occur, limbs tremble, teeth are clenched, and the mouth is locked shut, death ensues immediately.
5. When the smallpox pustules are scabbing and falling off, if the eyes are closed without spirit, the abdomen is distended, and the feet are cold extending beyond the knees, death is certain.
6. When the smallpox pustules are scabbing and falling off, if the voice is hoarse with rapid breathing, phlegm rattles in the throat, urine output is scant, and bowel movements are frequent, death is certain.
7. When the smallpox pustules are scabbing and falling off, if the scar marks are snow-white with no blood color, death will follow later. Urgently administer two doses of Xiaodu San (Detoxification Powder), followed by herbs that tonify Qi and Blood to nourish the Spleen and Stomach; recovery may be possible. Treatment should be initiated in advance.
Formulas:
Baicao San (Rotten Grass Powder)
Indications: Smallpox sores becoming suppurative due to scratching, with dripping blood.
Usage: Use rotten grass from roofs used for many years, or rotten grass from walls. These grasses, having been exposed to frost and dew, absorb the Yin and Yang Qi of Heaven and Earth, and possess excellent properties for resolving sore toxins, though their full efficacy cannot be fully described. Take an unspecified amount of grass, dry it in the sun or bake it until dry, and grind it into powder. Apply the dry powder directly onto the sores. If the entire body is broken out with incessant pus, sticking to clothes and making sitting or lying difficult, spread two or three sheng [a traditional unit of volume] on the mat, allowing the child to sit or lie upon it. The effect is miraculous. Continue to administer Muxiang San (Costus Root Powder), decocted with Dingxiang (Clove) and Guigui (Cinnamon Twig).
Xiaodan Gao (Niter and Gallbladder Ointment)
Indications: Inability of the mouth/lips to close, foul-smelling scars with flowing bloody pus.
Usage: Grind Zhu Dan (Pig Gallbladder) and Mangxiao (Mirabilite) into fine powder. Apply to the affected area; the disease will heal spontaneously.
Preparation: Grind Pig Gallbladder and Mirabilite evenly to form an ointment, then apply.
Tuojia San (Falling Nail Powder)
Indications: Sores where the crusts/nails do not fall off, preventing proper scabbing.
Ingredients: Xionghuang (Realgar), Chantuitui (Cicada Slough, cleaned of soil), Rendingonggu (Human Vertex Bone, burnt to ash), each 1 qian.
Usage: Grind all ingredients into a fine powder. Take 3 fen per dose, washed down with rice water.
Xionghuang San (Realgar Powder)
Indications: Children with broken teeth, developing gan ulcers (nutritional deficiency sores) and erosive sores.
Ingredients: Xionghuang (Realgar) 1 qian, Tonglv (Malachite) 2 qian.
Usage: Grind both ingredients into an extremely fine powder. Apply dry powder appropriately according to the child's size.
Mianjian San (Silkworm Cocoon Powder)
Indications: Smallpox with gan erosive sores on the body and joints, with incessant pus.
Ingredients: Kong Canjian (Empty Silkworm Cocoon, specifically those from which moths have emerged).
Usage: Take an unspecified amount of empty cocoons. Fill them finely ground Sheng Baifan (Raw Alum). Burn over charcoal until the alum juice is completely dried. Remove, grind into an extremely fine powder, and apply dry powder to the sore openings.
Zhuwei Gao (Pig Tail Ointment)
Ingredients: Nao (Borneol, approx. half zi/grain, finely ground), mixed with fresh Pig Heart Blood to form pills, coated with Zhensha (Cinnabar).
Usage: Dissolve and administer with Zicao Tang (Arnebia Decoction).
