Stool becomes comfortable and smooth. Currently, there is a person suffering from dysentery (Li Ji, 痢疾), with a pulse that is minute and faint (Xiao Mai, 微小). Upon repeated palpation, it is found to be deep (Chen Mai, 沉) and choppy (Se Mai, 涩). In this condition, if the discharge is white mucus, although the severity is high, it may still be treatable; however, if blood is discharged, fever must be strictly avoided. Generalized fever indicates death; fever persisting for seven days also leads to death. The above discussion, while insufficient to fully elucidate the profound origins of dysentery, is sufficient to clarify its general manifestations.
Critics argue: As you stated, from the Jiazi year to the Guihai year, within every sixty-year cycle, there is not a single year without dysentery. Yet nowadays, among people suffering from dysentery, it suddenly occurs in only one year out of several years. What is the reason? The answer is: Within a sixty-year cycle, there is never a year without the conflict between Water and Earth, nor a year without the stagnation of Earth and Fire, nor a year without the violation between Water and Fire. However, the governance of the Five Movements (Wu Yun, 五运) is like the balance of a scale. Throughout the year, the Qi numbers of the Five Elements mutually restrain each other; when they achieve equilibrium, diseases naturally do not arise. If suddenly one Qi becomes excessive and one Zang organ has surplus, the onset of dysentery will occur instantaneously. Therefore, I have detailed these general principles to enlighten those who are yet to understand, hoping that in diagnosis and treatment, there will be no errors or misunderstandings.
However, the injury caused by toxic dysentery varies; the most severe case is when Pathogenic Water invades the Heart. When people first contract dysentery, they initially present with alternating chills and fever (Han Re, 寒热). After medication, the condition does not subside; the fever becomes extreme, eating causes vomiting and reflux, and dysentery continues incessantly. The heart feels hot as fire, the patient desires cool places, craves cold water, runs wildly, suffers from generalized muscle pain, and cannot tolerate touch. In this condition, three or four out of ten cases are difficult to treat.
For the prescription treating epidemic dysentery (Yi Li, 疫痢), one must carefully read the entire treatise from beginning to end to understand it clearly, identify the root cause of the disease, and then take medicine. When toxic dysentery is first contracted, it initially presents with alternating chills and fever, sudden headache, sudden high fever, or frequent bowel movements with mixed red and white discharge. Sometimes only white mucus is discharged; sometimes white mucus is discharged first and later turns into bloody dysentery; sometimes bloody dysentery is discharged first and later turns into white mucus. This prescription is suitable. However, when dysentery first appears with alternating chills, fever, and headache, it indicates Cold Pathogen invading the Heart. Cold Qi invading the Heart causes a battle between Water and Fire, hence the initial alternating chills and fever. The violation of Water and Fire alters the blood internally, leading to frequent bloody discharge resembling purple grass water or amaranth water. Lack of luster indicates severe invasion of the Heart by Cold Pathogen. If alternating chills and fever appear first, but the discharge is purely white, it indicates mild invasion of the Heart by Cold Pathogen. If white mucus is discharged first, followed by bloody changes, the invasion of the Heart by Cold Pathogen is gradually intensifying. If bloody dysentery is discharged first, followed by a change to white mucus, the invasion of the Heart by Cold Pathogen is gradually diminishing. Equal amounts of red and white indicate that the Qi of Water and Fire are equal, formed by the struggle of Cold-Dampness Qi. Sudden occurrence of less red and less white indicates varying degrees of Cold Pathogen and varying severity of toxic dysentery. White predominance is mild; red predominance is severe. The treatment method involves first overcoming the Cold, so the prescribed medicine remains the same.
According to the Tai Sui (Grand Duke Jupiter) cycles, specifically in Bing Zi, Bing Wu, Jia Zi, Jia Wu, Geng Zi, Geng Wu, Bing Yin, Bing Shen, Jia Yin, Jia Shen, Geng Yin, Geng Shen, Chen Xu years, where the movement passes through Bing Shen and Geng positions, the harm is particularly severe. Also in Chou Wei years, this condition is likely. Furthermore, one need not strictly rely on the Tai Sui, as heavenly and earthly changes manifest in diverse ways, which are hard to exhaust. Now, this prescription is established, but people need not rigidly adhere to the Tai Sui. One should observe if, during spring and summer of a given year, there is much cold and austere transformation with little sunshine, and if cold and heat Qi suddenly contend. Or if, in mid-summer, there is much Cold-Damp transformation, and Cold Pathogen invades the Heart causing dysentery with initial alternating chills and fever, sudden headache, frequent bowel movements followed by bloody dysentery, mixed red and white discharge, or pure white discharge. In all these cases, Tong Shen San (Spirit-Passing Powder) is suitable. After taking it, if high fever subsides, good. But if after two or three doses, high fever persists, do not take this prescription further; see the separate treatise below.
Ephedrae Herba (Mahuang, 麻黄) [roots and joints removed], Cinnamon Twig (Guizhi, 官桂) [bark removed, 3 fen each], Licorice Root (Gancao, 甘草) [1 fen each, roasted], Ligusticum Rhizome (Daxiong, 大芎) [2 qian], Asarum Root (Xixin, 细辛) [8 zhu], Angelica Pubescens Root (Duhuo, 独活), Platycodon Root (Jiegeng, 桔梗) [4 zhu, stir-fried].
Grind all ingredients into fine powder. Take 2 qian each time, at any time, mixed with hot water. Consume the dregs while hot. If after two or three doses, chills and fever persist, stop taking it; see the separate treatise below. If after taking this medicine, chills and fever subside and bloody dysentery decreases, proceed to prepare and take the second or third prescriptions to achieve recovery. If chills and fever subside but bloody dysentery does not decrease, take two or three more doses, but do not overconsume; limit to two doses per day. Wait until bloody dysentery decreases and suddenly turns into white mucus, then gradually prepare and take the second prescription. As symptoms diminish, prepare and take the third prescription to ensure safety.
In Jia years and Geng years, during spring and summer, seasonal Qi is often cold. If people contract dysentery, this Tong Shen prescription is effective. In Jia years and Chou/Wei years, Damp transformation is predominant. If people contract dysentery with initial alternating chills and fever, add 1 liang of Cardamom Seed (Caodoukou, 草豆蔻) to the prescription. Regardless of the Tai Sui, if during spring and summer of a year, Cold is prevalent, and people contract dysentery with initial alternating chills and fever, this prescription is suitable. For toxic dysentery initially presenting with alternating chills and fever, take the previous prescription. Once chills and fever subside and bloody dysentery decreases, advance to Zhen Huan San (True Return Powder). If after the previous prescription, chills and fever persist and bloody dysentery does not decrease, do not advance to this medicine. Heavenly and earthly changes are extraordinary, and dysentery symptoms are manifold; thus, careful attention is necessary.
Terminalia Chebula Fruit (Hezi, 诃子) [5 fruits, wrapped in flour, fire-roasted until cooked, not raw, not burnt. Remove flour. While warm, bite open, remove core, use only the peel, bake dry].
Grind finely and sieve. Take 2 qian each time, with 1.5 cups of rice soup, boiled down to 1 cup. If one or two mouthfuls of saliva are vomited, stop. After consuming several doses, bowel function improves, and frequency decreases. Then prepare and take the third prescription to strengthen the large intestine. If after the previous prescription, high fever persists and bloody dysentery does not decrease, do not take this medicine.
For epidemic toxic dysentery, after taking the first two prescriptions, if the condition has reduced and only small amounts remain, appearing suddenly as green stool, duck-like stool, tea-colored liquid, turbid oil, or slight shallow/deep red color, take this prescription to strengthen the large intestine and restore true Qi: Bo Shang Liu Huang Wan (Sulfur Pill from the Ships).
Bo Shang Liu Huang (Ship-sourced Sulfur, 舶上硫黄) [1 liang, remove sand and stones, grind finely like flying dust].
Mix with 2 liang of Coix Seed (Yiyiren, 薏苡仁), stir-fried until cooked and ground into powder. Combine evenly with the sulfur. Add drops of water to form pills the size of wu zi (Chinese quince seeds). Take 50 pills on an empty stomach with rice soup.
For toxic dysentery, initially presenting with alternating chills and fever, take three doses of Tong Shen San over one day. If overnight chills and fever persist, dysentery frequency does not decrease, generalized fever occurs, and discharge resembles thick nasal mucus, peach blossom color, or red rice glue, with each episode yielding about one and a half cups. Take two or three doses of this medicine. If after one day or overnight, chills and fever persist and dysentery does not decrease, stop this medicine. If taken according to method for two doses, high fever subsides and dysentery slightly decreases, then gradually continue, limiting to two doses daily. Wait until bloody dysentery slightly decreases and suddenly turns white, then prepare and take Zhen Huan San above to ensure safety. Regarding dysentery with chills and fever, the syndrome is one, but the former prescription uses warming medicines to resolve it, while this one uses cooling medicines. Why? Answer: Pathogenic Water invading the Heart causes body fever; Dampness of Earth stagnating Fire causes body fever; Liver Heat punishing Spleen causes body fever. Only the most refined practitioners can recognize these subtle changes. Fearing that common people cannot grasp these profound principles, I have set up these two prescriptions to remedy deficiencies. However, among injuries from toxic dysentery, Water Pathogen invading the Heart is the most severe. Patients with dysentery experiencing increased chills and high fever should first take two or three doses of Tong Shen San above. If chills and fever persist and dysentery continues, then advance to this medicine. According to Tai Sui, this condition is suitable in Ren years, Wu years, Yin/Shen years, and Si/Hai years. In Wu years, there may be variations, as in Fire-dominant years, Cold-Dampness inevitably violates, hence this. Due to the shifting Six Qi, one cannot rigidly fix the year. Simply, if during spring and summer of a year, Heat Qi is predominant, Damp transformation fails to rise, and people contract dysentery with wiry Liver pulse and flooding Heart pulse, then take this medicine. If after two doses, no effect is seen, do not take further. Since dysentery arises, one must fear Water invading the Heart; do not recklessly administer cooling medicines. Thus, if this prescription shows no effect after two doses, do not take it further.
Schizonepeta Spike (Jingjie Sui, 荆芥穗), Moutan Bark (Mudan Pi, 牡丹皮) [remove heart], Aucklandia Root (Muxiang, 木香), Pulsatilla Root (Baitouweng, 白头翁), Coptis Rhizome (Huanglian, 黄连) [honey-soaked overnight, roasted on fire until dry, use 3 parts], Morning Glory Seed (Qianniu, 牵牛) [stir-fried, 1 fen].
Grind into fine powder. Take 2 qian each time, with 1 cup of water, boil for one or two boils, strain immediately. Take warm after meals. If this medicine shows slight effect, it can be continued until peace is achieved. Do not overconsume; limit to two doses per day. Stop once dysentery ceases.
For toxic dysentery, initially presenting with chills and fever, slight cold, sudden cessation of heat, abdominal pain. During defecation, small amount of gelatinous saliva follows stool, mixed with bloody dysentery. Mistakenly taking cooling medicines causes gradual extreme dysentery from morning to night. Patient only desires sleep. Early morning complexion is withered, body slightly cool. Late evening to night, face is red and lustrous. Extreme dysentery consists of clear saliva mixed with bloody dysentery. Generalized fever, loss of appetite, generalized muscle pain upon touching the bed. Based on this condition, this medicine is suitable. If body is extremely hot like fire, radiating heat to others, pulse is slight and faint. Light palpation reveals dispersion filling the fingers, lacking structure, no bone strength, feeling like wind or Qi. Heavy palpation reveals nothing. Urgently use 5 Terminalia Chebula fruits (Hezi, 诃子) prepared as follows: roast, grind, and sieve. For one dose, mix with hot rice soup, consume dregs hot. Vomiting saliva allows pulse Qi to regenerate, then advance to the following prescription.
Atractylodes Macrocephala Rhizome (Baizhu, 白术) [0.5 liang], Angelica Dahurica Root (Baizhi, 白芷), Peony Root (Shaoyao, 芍药), Ligusticum Rhizome (Daxiong, 大芎) [1 fen each], Licorice Root (Gancao, 甘草) [4 zhu, roasted], Terminalia Chebula Fruit (Hezi, 诃子) [5 fruits, flour-wrapped, fire-roasted].
Grind finely and sieve. Take 3 qian each time, mixed with hot aged rice soup, consume dregs.
For toxic dysentery initially contracting without chills or fever, discharge is entirely thick blood or dark blood. Pulse Qi in left and right three positions is minute and fine. Take this prescription.
Mulberry Mistletoe (Sangjisheng, 桑寄生) [1 liang], Saposhnikovia Root (Fangfeng, 防风), Ligusticum Rhizome (Xiong, 芎) [1 fen each], Licorice Root (Gancao, 甘草) [4 zhu, roasted].
Grind into fine powder. Take 2 qian each time, with 1 cup of water, boil to 0.8 cup. Take on empty stomach with dregs.
For red and white dysentery, initially contracting without chills or fever, suddenly developing red and white dysentery, enduring long without worsening, no additional chills or high fever. Take this prescription.
Fermented Soybean (Danshi, 淡豉) [10 pieces], Coptis Rhizome (Huanglian, 黄连) [49 pieces, each the length of a grain of rice].
Place herbs in a new bottle, cover mouth with new tile, fire until red, smoke disappears. Take out, grind while hot like flying dust. Take 2 qian each time, mixed with hot salted rice soup, consume dregs. For all red and white dysentery, regardless of red or white predominance, if body has no added chills or high fever, this prescription is suitable.
For red dysentery, or mixed red and white, initially no added chills or high fever. After several days, body is not hot, only internal restlessness and heat in lower abdomen. Unceasing blood discharge. Warming medicines for hemostasis show no effect. Take this prescription. Suitable in Yin/Shen, Ji/Hai years, and Ren years. If after two doses no effect, do not take further. If lips are red, suddenly swollen, dark colored, urine yellow-white, body cool, fever at night, this indicates Liver Heat punishing Spleen.
Schizonepeta Spike (Jingjie Sui, 荆芥穗), Moutan Bark (Mudan Pi, 牡丹皮) [remove heart], Pulsatilla Root (Baitouweng, 白头翁), Sanguisorba Root (Diyu, 地榆), Morning Glory Seed (Qianniu, 牵牛) [roasted, 3 zhu], Aucklandia Root (Muxiang, 木香) [3 zhu].
Grind into fine powder. Take 2 qian each time, with 1 cup of water, boil two boils. Take warm with dregs after meals. Must be during spring and summer when Wind-Heat is excessive. Clearly identify dysentery as latent Heat before taking this medicine.
For red dysentery, body has no added chills or high fever, named Liver-Heart Transformation.
Coptis Rhizome (Huanglian, 黄连) [1 liang, honey-soaked overnight, roasted until fragrant], Aucklandia Root (Muxiang, 木香) [3 zhu].
Grind into fine powder. Decoct strongly, mix 2 qian with aged rice soup, take with dregs after meals.
For unceasing red dysentery, pulse is minute, faint, deep, and choppy. Body fever indicates death; fever for seven days indicates death. This condition is extremely critical. Take one dose of Tong Shen San above; if fever subsides, it may be treatable. Then prepare and take Zhen Huan San and Bo Shang Huang Wan above to ensure safety. If after Tong Shen San, fever persists, it is difficult to treat.
For patients with red or mixed dysentery, suddenly due to prior cold malaria turning into dysentery, or due to dysentery turning into cold malaria. During acute dysentery, regardless of red or white, if there is added chills and high fever, take Tong Shen San above. After taking, if heat and severity subside, prepare and take Zhen Huan San and Bo Shang Huang Wan to ensure safety. This syndrome is Water Pathogen invading the Heart; treatment must not be light.
