The *Mai Jue* (Pulse Classic) states that the Kou pulse (Kou, a hollow pulse resembling a scallion stalk) indicates blood loss. The Kou pulse feels like pressing on a ring: there is sensation at both ends, but it curves and is empty in the middle; this does not mean the pulse has ceased. If the Heart and Liver pulses present with a Kou quality, while the Lung pulse is Floating and Rapid, or if the Lung pulse is Slightly Wiry and Tight, symptoms such as bone-steaming fever and hemoptysis (coughing up blood) will occur.
If the Heart pulse is Kou and the Lung pulse is Soft and Deep, with the Chi (Cun Cun) pulse Deep and Hidden, slightly Tight, this indicates intestinal wind bleeding (hematochezia).
If the Heart and Lung pulses are Kou, the Bladder pulse is Slippery, and the Small Intestine pulse is abnormal, this indicates hematuria (blood in urine).
If the Liver pulse is Kou and the Lung pulse is Large, Floating, and Hasty, with the pulses at the Chize (LU-5) acupoint on both hands being faint, accompanied by panting and hemoptysis.
If the Liver, Heart, and Lung pulses are all Surging and Large, yet lacking "bone strength" (firmness), appearing to open and close intermittently like flowing water, this represents Metal generating Fire (Lung pathology affecting the Heart), indicating impending hematemesis (vomiting blood). If the Heart pulse is Slight and Tight, Hasty, extending upward to the Yuji (LU-10) area, coming quickly upon pressure and departing slowly, this is intestinal wind syndrome, characterized by scant lower bleeding, frequent palpitations/tremors, aversion to tasks, and lack of mental energy. If the Liver pulse is Long and Moving, extending downward through the Chize (LU-5) area, and the Heart pulse is Large and Excess, indicating an excess of blood in the Sanjiao (Triple Burner), lower bleeding should also occur.
If the Liver pulse is Fine, Minute, and Choppy, the Chize (LU-5) pulse is faint, and the Stomach pulse is Soft and Weak, this indicates large intestine bleeding.
If the Heart and Liver pulses are discontinuous, resembling the start of a dripping drop, this is epistaxis (nosebleed). If the Lung pulse is Large, Floating, and Scattered, and the Heart pulse is Large, this indicates sudden epistaxis.
The Pulse Classic states: A Tight pulse indicates parasites or hemorrhoids. It must be that the Liver pulse is also Tight and Fine to diagnose hemorrhoids; otherwise, it indicates parasitic infestation.
If the six pulses are Floating and Large, lacking bone strength, arriving rapidly and departing slowly, feeling Surging with light pressure but Soft (Ru) with heavy pressure, this results from prolonged sores/boils leading to blood deficiency. If the six pulses are Floating and Large but possess bone strength, the Liver pulse is Wiry and Long, the Lung pulse is Large, Excess, and Deep, interfacing with the Stomach pulse which is Slightly Wiry and Soft with light pressure, this indicates flooding of blood in the Sanjiao due to Stomach deficiency, causing lower bleeding.
If the six pulses are normal, but the Heart pulse is Agitated, and the Kidney pulse is Pulsating, Deep, Fine, and Rapid, this indicates bloody diarrhea, or hematuria, or melena.
In pregnant women, if the Guan and Cun pulses on the hand are Slippery and Rapid, while the Lung pulse is Deficient and fine like hair, and the Chize (LU-5) area is depressed and does not interface with the Guan pulse, this indicates pregnancy. If the Liver pulse is Long and overflows into the Cun position, this is fetal leakage with blood loss. There are two types of fetal lower bleeding: one where Liver Qi is weak, the Kidney pulse is soft and limp, the fetal pulse is depressed, and fetal movement causes bleeding, which requires tonification; the other where the Liver pulse is Excess and bleeding occurs, known as fetal overflow, which requires cooling methods.
In women, if the Liver pulse is Surging and Large but paradoxically Knotted and Choppy, the Pulse Classic states: A Choppy pulse implies generation; for women, this signifies retained blood (stasis). If the pulse is Surging, Large, and Hidden, it indicates mass accumulation and stagnant blood. Over time, this leads to swelling of the yin gate (vulva), as the Foot-Jueyin Liver meridian passes through this region.
