Due to horse riding, archery, galloping, and leaping, combined with exposure to cold and heat and physical fatigue, all these factors can injure the Yang Qi. For the past three years, the right chest and hypochondrium have shown a slightly protruding elevation. Initially, there was distending pain without any visible form; over time, it became hard like a lump. This is because initially, it was Qi stagnation in the meridians, but after a long duration, blood injury has entered the collaterals. The meridians are connected to the outer framework of the Zang-Fu organs and can still barely support themselves. However, as Qi becomes sluggish and Blood stagnates, blood stasis and bi-syndrome gradually develop, leading to the formation of Jia masses (accumulations). Anger, labor, or strenuous effort cause chaotic Qi and Blood, causing the disease to flare up immediately. Therefore, methods such as using cold/warm remedies, digestion-promoting purgatives, regulating Qi, and expelling Blood ultimately fail to address the intricacies of treating collateral diseases. Examining Zhang Zhongjing's methods for Lao Shang (labor injury) and Xue Bi (blood obstruction), his methods for unblocking collaterals often utilize insect and ant species, as well as agile creatures that fly or walk swiftly. Those that fly ascend, those that walk descend, preventing Blood from coagulating and adhering, allowing Qi to circulate freely. This differs from methods that merely attack accumulations and remove hardness by directly entering the Zang-Fu organs. These methods are recorded for reference.

After illness, improper diet leads to dysfunction in intestinal transformation and transportation. Qi stagnation breeds Dampness, which stagnates and transforms into Heat. Stagnant turbid Qi in the Six Fu organs gathers. In the past, physicians like Jie Gu and Dong Yuan often used pill formulations for slow attack when treating gastrointestinal diseases; one should emulate this approach.

Anger combined with forced eating causes Liver Wood to invade Spleen Earth, resulting in abdominal pain. A palpable mass suddenly appears, vanishing when relaxed. Audible gurgling upon Qi descent indicates residual effects of Liver Fire and Heart Fire, belonging to the category of Jia masses and Hernias. Attack and digestion-promoting methods will inevitably lead to abdominal fullness, as this is deficiency mixed with stagnation, which is difficult to cure quickly. Recent painful diarrhea suggests a risk of extending into autumnal dysentery.
Prescription: *Flos Caryophylli* (Ding Xiang), *Cortex Magnoliiae Officinalis* (Nong Po), *Poria* (Fu Ling), *Radix Paeoniae Alba* (Chao Bai Shao), *Pericarpium Citri Reticulatae* (Guang Pi), *Rhizoma Alpiniae Oxyphyllae* (Wei Yi Zhi Ren).
It is stated that bloody diarrhea lasted for five days; once bleeding stopped, coughing and wheezing began. There is a lump-like sensation under the left hypochondrium. Physical movement and walking cause dizziness and a feeling of impending collapse. Food intake decreases in spring and summer, increasing slightly in autumn and winter. Upon entering winter, the Ren Ying pulse shows knotting and goiter-like changes. The pulse is deficient, while the left Guan and Chi positions are rapid. This indicates injury to Liver-Kidney Essence and Blood due to fright, fear, worry, and labor. Yang loses Yin attachment; in the empty spaces of the collaterals, Yang transforms into Internal Wind, beating incessantly, causing daily consumption without recovery. This is a syndrome of Depressive Consumption. Beyond forty, the generating Qi is shallow; mere treatment of disease cannot reverse it. It is evident that during spring and summer, the body's True Qi cannot withstand dispersal. One must shun household affairs, care for nurturing, hoping for symptom relief.
Prescription: *Concha Haliotidis* (Shi Jue Ming), *Fructus Ligustri Lucidi* (Nv Zhen Shi), *Fructus Lycii Chinensis* (Qi Zi), *Semen Sesami Nigrum* (Hei Zhi Ma), *Folium Mori* (Sang Ye), *Corium Corii Asini Colla* (E Jiao), *Ramulus Taxilli* (Ji Sheng), *Semen Platycladi* (Bai Zi Ren), *Poria* (Fu Ling), *Radix Angelicae Sinensis* (Chao Dang Gui).
Old Jia masses under the hypochondrium are similar to Fei Qi (Liver Accumulation). Self-reported due to anger, this is an accumulation of the Liver. Long-term stagnation prevents Qi and Blood flow. The Liver houses Ministerial Fire. During summer, amidst humid rains, the Spleen-Stomach suffers from Dampness, naturally leading to fatigue. Liver Wood overacts on Spleen Earth, worsening distension daily. The acquired Damp Evil has no outlet, steaming in the intestines and stomach, causing sticky pus and stagnation, with incomplete evacuation. Middle Jiao disharmony prevents rest even when awake. The condition is difficult to treat. Tentatively propose separating and eliminating pathogens.
Prescription: *Radix Atractylodis Macrocephalae* (Bai Zhu), *Cortex Magnoliiae Officinalis* (Nong Po), *Poria* (Fu Ling), *Polyporus* (Zhu Ling), *Herba Artemisiae Scopariae* (Yin Chen), *Herba Lycopi Scandentis* (Tong Cao).
The disease started in the lower legs/shins, originating from the Lower Jiao Liver and Kidneys. Since there is no swelling, it is not caused by the Six Evils' Dampness. It should be investigated under the category of Wei Syndrome (flaccidity) from Internal Damage. Cornus and Rehmannia are cloying and obstructive; long-term use damages the Stomach. Reduced appetite and vomiting are due to heavy flavors and stagnant Qi. After a year without recovery, the damaged become more damaged. Visceral True Qi cannot flourish. The Eight Extraordinary Vessels fail to function. The Classics state: "When the Chong Mai is diseased, men develop internal seven hernias, women develop leukorrhea and Jia Ju." The Chong Mai is the Sea of Blood; men store Essence, women connect to the Uterus. Now, Essence depletion with internal knotting forms a tangible mass; this is Essence obstruction and Qi stagnation, similar to female Jia Ju. Regarding treatments for seven hernias, later generations mostly followed Zhang Hezi, who exclusively used acrid-hot herbs, which is completely different from current Essence obstruction and Qi stagnation. Chronic illness leads to emaciation and muscle wasting. Propose using sentient substances of Essence and Blood to nourish Generating Qi.
Usage: Fresh *Placenta Humana* (He Che) boiled, mashed, mixed with *Rhizoma Dioscoreae* (Shan Yao) and *Semen Nelumbinis* (Jian Lian), formed into pills the size of paulownia seeds. Take in morning Ginseng decoction.
It is stated that left hypochondriac pain radiates to the chest and the Xu Li acupoint. Palpation reveals a tangible mass. Food intake does not descend smoothly. Frequent anger, labor, and vexation cause Qi rebellion and Blood depression. Beyond fifty, vitality declines. Prolonged duration raises concern for Ye Ge (dysphagia). Propose promoting circulation of Qi and Blood. Use acrid-moistening herbs; avoid aromatic-dry ones. Even if turbid stasis remains, it can move downward.
Prescription: *Radix Angelicae Sinensis* (Dang Gui Wei), *Beijing Ink Juice* (Jing Mo Zhi), *Paste of Semen Persicae* (Tao Ren Ni), *Rhizoma Corydalis Yanhusuo* (Yan Hu Suo), *Faeces Trogopterori* (Wu Ling Zhi), *Rhizoma Allii Macrostemonis* (Lao Xie Bai).
