Re-examination on September 1. Body temperature 37.2°C (rectal thermometer). Two bowel movements occurred, formed but yellow and loose; no pus or blood was observed. According to the mother, after the fever subsided, the child no longer experienced convulsions, slept peacefully, and could take in some milk and food. Although the fever had subsided and diarrhea stopped, the complexion became pale, lethargy and lack of spirit were evident, the fingerprint color was faint and barely visible, lips regained moisture, thirst disappeared, and the tongue coating turned thin, white, and moist. This indicates that the pathogenic factor has receded, but the Middle Jiao Spleen and Stomach are weak. It is necessary to regulate and tonify the Middle Qi, strengthen and transport the Spleen and Stomach to conclude treatment. The Gui Zhi Tang (Cinnamon Twig Decoction) combined with Dang Gui (Angelica Sinensis), Bai Shao (White Peony Root), and Li Zhong Wan (Regulate the Middle Pill) modified was prescribed.
Lu Dang Shen (Codonopsis Pilosula) 10g, Pao Jiang (Prepared Ginger) 6g, Rou Gui (Cinnamon Bark) 5g, Dang Gui (Angelica Sinensis) 6g, Hang Shao (White Peony Root) 5g.
Bai Zhu (Atractylodes Macrocephala) 6g, Sha Ren (Amomum Villosum) 3g, Gong Ding (Clove) 7 pieces, Gan Cao (Licorice Root) 3g.
After taking the above prescription for three doses, re-examination on September 4 showed that the child had no bowel movements on the second and third days of medication, with overall condition good. On this day, two bowel movements occurred, yellow and slightly loose. Body temperature normalized, spirit improved, and milk/food intake returned to normal. Stool cultures taken on September 1 and 5 did not detect dysentery bacilli. After a few days of regulation, the child recovered and was discharged.
Pediatric Damp-Heat Dysentery (Dysentery due to Combined Pathogen Invasion of Three Yang Channels)
Li XX, female, 1.5 years old. Consulted on April 8, 1964 (Outpatient No. 136651, Affiliated Hospital of College of Traditional Chinese Medicine). After contracting measles, the child suffered from bloody and white stools for over ten days, fever of 39.8°C, alternating chills and heat, red face, rapid breathing, frequent crying, insomnia at night, thirst with desire to drink. Bowel movements occurred 17-18 times daily, small volume, greenish with mixed red-white mucus. Crying incessantly when needing to defecate (abdominal pain), only stopping after intense crying; difficult defecation (tenesmus, unsatisfactory evacuation). Urine was short and red. Pulse was fine and rapid; fingerprint was cyan-purple and floating; tongue coating was white and greasy. Western anti-dysentery drugs taken for several days without effect. This is dysentery due to combined pathogen invasion of the Three Yang channels. Prescription:
Gui Zhi (Cinnamon Twig) 15g, Ge Gen (Pueraria Lobata) 10g, Chai Hu (Bupleurum Root) 6g, Huang Qin (Scutellaria Baicalensis) 3g, Huang Lian (Coptis Chinensis) 3g, Lang Pian (Areca Seed Slice) 3g, Hang Shao (White Peony Root) 10g, Xiao Zao (Jujube) 5 pieces, Sheng Jiang (Fresh Ginger) 2 slices, Gan Cao (Licorice Root) 7g.
One dose decocted and taken continuously six times yesterday, resulting in slight sweating. Today, fever subsided, body cool. Three bowel movements, yellow and semi-formed, slightly mucous; redness gone. Urine cleared, thirst resolved, able to eat soft rice. Pulse slowed; fingerprint cyan-purple receded, becoming pale and fine; tongue coating thin white, substance red and moist. The dysentery has stopped, requiring further regulation. One dose of Gui Zhi Tang (Cinnamon Twig Decoction) modified concluded the treatment.
Su Tiao Shen (Codonopsis Pilosula) 6g, Hang Shao (White Peony Root) 6g, Fa Xia (Pinellia Ternata Processed) 6g, Sha Ren (Amomum Villosum) 3g, Gui Zhi (Cinnamon Twig) 10g, Xiao Zao (Jujube) 5 pieces, Sheng Jiang (Fresh Ginger) 2 slices, Gan Cao (Licorice Root) 5g.
Autumn Dryness
Shang XX’s daughter, 14 years old. Contracted autumn dryness pathogen in August 1924. Fever, headache, dry and painful throat, inability to eat, preference for cool drinks. Pulse string-like and rapid; tongue tip red; tongue coating white and dry. This resulted from external pathogens combining with Yangming dryness qi. Ma Huang Tang (Ephedra Decoction) combined with Xing Ren (Apricot Seed), Gan Cao (Licorice Root), Shi Gao (Gypsum) modified cured it in one dose.
Ma Huang (Ephedra Herba) 10g, Xing Ren (Armeniacae Semen Amarum) 10g, Sheng Shi Gao (Raw Gypsum Fibrosum) 12g (crushed, wrapped in cloth), Jie Geng (Platycodon Root) 10g, Lian Qiao (Forsythiae Fructus) 6g, Cun Dong (Ophiopogon Japonicus) 10g.
Dry Cough
Wu XX, male, 24 years old, student at Kunming Medical College. Contracted autumn dryness pathogen in October 1963, suffering from frequent coughing, dry cough with sound but no phlegm or nasal discharge, dry cheeks. Tongue coating thin white with little fluid; pulse floating and soft. Western antitussives ineffective. This is Yin deficiency with Lung dryness. Nourishing Yin, moistening the Lung, and stopping cough was indicated. Er Dong Er Mu Tang (Two Winter and Two Mother Decoction) modified cured it in one dose, restoring fluids and stopping the cough immediately.
Mai Dong (Ophiopogonis Radix) 15g, Tian Dong (Asparagi Radix) 12g, Chuan Bei Mu (Fritillariae Cirrhosae Bulbus) 10g, Zhi Mu (Anemarrhenae Rhizoma) 10g, Bai He (Lilii Bulbus) 12g, Huang Qin (Scutellariae Radix) 10g, Sheng Hang Shao (Raw White Peony Root) 12g, Bai Bu (Stemonae Radix) 10g, Sang Bai Pi (Mori Cortex) 12g, Xuan Shen (Scrophulariae Radix) 10g, Wu Wei Zi (Schisandrae Fructus) 3g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g.
Cold Malaria
Yan XX’s wife, suffered from malaria for over twenty days. Attacks every afternoon, starting with chills then heat. Severe chills caused trembling and jaw clattering; sweating profusely after heat subsided. Severe splitting headache during attacks, inability to eat, vomiting sour/bitter saliva, loose stools, distending pain in both hypochondria, preferring hot drinks. Pulse fine, slow, and powerless. Tongue coating white and slippery, substance containing blue hue. Over ten doses of digestion-promoting, heat-clearing, and liver-calming herbs failed. This is internal Yang deficiency, Yang unable to overcome Yin. Liver pathogen combined with cold-water qi rising counterflow. In the afternoon, Yin is abundant and Yang deficient; True Yang is rejected by Yin pathogens and floats outward. Si Ni Tang (Frigid Extremities Decoction) modified was prescribed.
Tian Xiong Pian (Aconiti Lateralis Radix Praeparata) 60g, Gan Jiang (Zingiberis Rhizoma) 30g, Gong Ding (Caryophylli Flos) 5g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added), Fa Xia (Pinelliae Rhizoma Preparatum) 12g, Fu Ling (Poria) 30g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g. Also instructed to swallow two pills of Wu Mei Wan (Mume Pill) first. Took one dose that evening, another the next morning; attacks ceased in the afternoon. Later, adding Sha Ren (Amomi Fructus) 10g to the original formula, cured in two doses.
Case of Saving Reversal from Incorrect Purging in Yin Syndrome
Previously treated a male, approx. 20+ years old, only son of a widow, constitutionally weak. Initially presented with abdominal pain, constipation, and fever. A physician diagnosed stagnant stasis-heat and incorrectly used Tao Ren Cheng Qi Tang (Persicae Semen and Magnolia Bark Decoction) to purge. Bowels remained unopened, condition worsened, leading to manic running, wandering, and confused speech. I examined him: pulse deep, slow, and powerless; tongue red, fluids exhausted but no thirst, slight preference for hot drinks in small amounts, panting and short breath, signs of collapse. Diagnosed as Yin syndrome incorrectly purged, forcing Yang to sudden collapse. Prescribed large dose of Hui Yang Yin (Return Yang Drink, i.e., Si Ni Tang plus Rou Gui).
Fu Pian (Aconiti Lateralis Radix Praeparata) 130g, Gan Jiang (Zingiberis Rhizoma) 50g, Shang Rou Gui (Cinnamomi Cortex) 13g (powdered, soaked in water and added), Gan Cao (Glycyrrhizae Radix et Rhizoma) 10g.
That night, epistaxis and black stool occurred. Next day re-examination: pulse faint, spirit depleted, lethargic, reluctant to speak, consciousness cleared. The epistaxis and black stool were not caused by warm herbs, but by blood loss and stasis formation from the incorrect purging of Tao Ren Cheng Qi Tang. The warm herbs moved Qi and Blood; already detached and damaged blood could not return to vessels, so it flowed upward and downward. Instructed to take one more dose of the original formula. Afterward, bleeding stopped, mouth slightly dry. Yang had returned, Ying-Yin still deficient. Continued with Si Ni Tang (Frigid Extremities Decoction) plus Ren Shen (Ginseng Radix) for four doses, curing the case. Ren Shen was added to benefit Qi, generate fluids, nourish Yin, and coordinate with Yang.
Shaoyin Cold Transformation Syndrome of Cold Damage Disease
Zeng XX, male, 17 years old, living on Huancheng East Road, Kunming. Illness started after eating and catching cold: fever, aversion to cold, head and body pain. A TCM physician gave two doses of cooling exterior-resolving herbs with no effect. Admitted to a hospital at the foot of Xishan Mountain. After 19 days of acupuncture and medicine, fever subsided but condition worsened. Multiple physicians diagnosed "Typhoid Fever" with risk of intestinal hemorrhage or perforation, deciding on blood transfusion. Condition did not improve post-transfusion, became critical, declared hopeless. Consulted me on Oct 25, 1943. Arrived at 9 PM. Patient ill for 19 days, no fever now, but abdominal distension, lower abdominal pain, occasional moaning, short red urine, no bowel movement for 7-8 days, no food intake, sleepless day and night, bedridden unable to turn. Nurses fed mineral water. Tongue coating white, slippery, thick, greasy; no thirst. Pulse string-tight, powerless and empty on deep pressure. Advised family: critical condition, complicated disease of Cold Damage, pathogen deeply entered Shaoyin organ-cold syndrome. Internal Yang deficient, Yin-cold excessive. Cold-water Yin qi condensed like ice/frost inside abdomen. Though fever subsided, interior cold is extreme. Inability to pass urine/stool is due to Yin-cold condensation and severe True Yang deficiency, unable to move; not comparable to heat accumulation. A thread of life Yang is about to collapse. Critical. Only method: Support Yang, suppress Yin, warm transformation. Expel upper cold-water pathogenic Yin via vomiting, middle/lower via urination/defecation. When Yang returns and Yin retreats, danger turns to safety. Used Zhong Jing's Tong Mai Si Ni Tang (Pulse-Reviving Frigid Extremities Decoction) plus Yu Zhu (Evodiae Fructus) and Shang Gui (Cinnamon Bark). Warned family: vomiting saliva/phlegm or diarrhea after medication is sign of disease expelling, hope for recovery.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 160g, Gan Jiang (Zingiberis Rhizoma) 30g, Shang Rou Gui (Cinnamomi Cortex) 16g (powdered, soaked in water and added), Fu Ling (Poria) 26g, Yu Zhu (Evodiae Fructus) 6g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g.
Re-examination Oct 26. Vomited about a bowl of saliva immediately after taking herb, sign of disease expelling. Pulse string-tight receded, became harmonious. Loose stool once, urinated three times. Lower abdominal pain persists, intermittent. Long course, severe Yang depletion, heavy interior cold. Effective but disease heavy, medicine light, strength insufficient. Fortunately, no panting, no rattling phlegm, hands/feet warm, pulse harmonious with spirit. Continue large dose supporting Yang and warming transformation to ensure Yang returns and Yin retreats, turning danger to safety.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 260g, Gan Jiang (Zingiberis Rhizoma) 60g, Yu Zhu (Evodiae Fructus) 20g, Shang Rou Gui (Cinnamomi Cortex) 16g (powdered, soaked in water and added), Gong Ding (Caryophylli Flos) 6g, Fu Ling (Poria) 30g, Xi Sha Ren (Amomi Fructus) 6g.
Third visit Oct 27. Vomited ~2 bowls of saliva early morning, once again in afternoon. Diarrhea several times. All "ice/frost melting," pathogenic Yin retreating through upper/lower orifices. Tongue coating still thick/greasy, substance red/vibrant. Face/lips red/rosy. Temperature normal. Pulse harmonious, rooted, spirited. Abdominal distension/pain mild, distension reduced 60-70%. Major hurdle passed. Long illness, severe Yang depletion, pathogenic Yin not fully cleared. Continue large dose supporting Yang and assisting healthy Qi.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 300g, Gan Jiang (Zingiberis Rhizoma) 60g, Shang Rou Gui (Cinnamomi Cortex) 16g (powdered, soaked in water and added), Tian Ma Bing Lang (Arecae Semen Cum Mesocarpo) 6g (shelled, pounded), Yu Zhu (Evodiae Fructus) 6g, Tai Wu (Cyperi Rhizoma) 4g, Xi Sha Ren (Amomi Fructus) 6g, Fu Ling (Poria) 30g.
Fourth visit Oct 28. Excreted stool 16 times last night, varying amounts. Two times this morning, watery/thin feces. Initially dark like sauce, now gradually yellow. Sign of Stomach generating Yang recovering. Temp 37°C, pulse 80/min. Urinated 6 times today, pale yellow/clear. Stinging pain in urethra during each void, due to pathogenic toxin excretion irritation, not heat excess. Thirst for hot drinks last night due to widespread interior Yin-cold/water evil. Now sun shines bright, Yin-cold scatters, cold water transforms. Yang spirit too deficient to transform Qi/generate fluids, hence preference for hot drinks to nourish. Occasional craving for cold items due to Jueyin Qi not connecting smoothly, Yin/Yang disharmony. Frequent flatulence, Fu pathways open, turbid Qi descending. Symptoms greatly reduced, danger passed. Liver/Kidney Yin not yet cleared, Yuan Yang not fully restored, hence left abdominal mass/pain. Gladly, ate slightly this morning, Stomach Qi harmonizing, generating Yang returning, cure expected. Post-major illness, care is crucial: avoid overeating harming Stomach, overworking harming Spirit, prevent wind-cold. Still support Yang and assist healthy Qi.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 300g, Gan Jiang (Zingiberis Rhizoma) 50g, Fu Ling (Poria) 30g, Yi Yi Ren (Coicis Semen) 16g, Shang Rou Gui (Cinnamomi Cortex) 18g (powdered, soaked in water and added), Bo Kou Ren (Cardamomi Fructus Rotundus) 3g (pounded), Xi Sha Ren (Amomi Fructus) 6g (pounded), Gan Cao (Glycyrrhizae Radix et Rhizoma) 10g, Bai Hu Jiao (Piperis Longi Fructus) 2.6g (pounded).
Also take two Wu Mei Wan (Mume Pills).
Fifth visit Oct 29. Pulse harmonious, 72/min. Temp 37.6°C. Six bowel movements. Urine clearer/longer, pale yellow. Mild urethral stinging. Abdominal mass gone. Complexion reddening, nose shiny. Tongue coating receded 60-70%. Appetite open, food intake increased, abdominal pain healed. Now full-body rash (Hong Dan). Major illness retreated, Yuan Yang recovering. Gradually shift to post-illness regulation. Reduce medicine as illness retreats, still support Yang/assist healthy Qi. Advise avoiding raw/cold fruits, sour/cold foods, tender eggs, sweet wine, sticky foods. Guard against wind-cold, regulate diet. Health can be restored.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 160g, Gan Jiang (Zingiberis Rhizoma) 30g, Fu Ling (Poria) 16g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added), Bo Kou Ren (Cardamomi Fructus Rotundus) 5g (pounded), Yi Yi Ren (Coicis Semen) 16g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 6g, Yuan Rou (Longan Arillus) 5g, Da Zao (Jujubae Fructus) 3 pieces.
Sixth visit Oct 30. Temp normal, pulse harmonious. Tongue root still white/greasy. Two bowel movements, scant loose stool. Urine pale yellow/clear. Mild abdominal distension/discomfort. Food intake increasing. Consider Spleen/Stomach still weak, digestion poor, giving measured porridge. Post-major illness, residual cold/Yin not cleared, Yuan Yang not fully restored. Stick to supporting Yang/assisting healthy Qi principle. Few doses will cure, no fear of change. Wrote a four-line poem:
Dark clouds gather, sunlight dims;
In a blink, the true dragon flies.
Acrid-sweet transforms to Yang, Li Fire appears;
Why worry that earth won't see spring return?
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 300g, Gan Jiang (Zingiberis Rhizoma) 50g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 10g, Shang Rou Gui (Cinnamomi Cortex) 16g (powdered, soaked in water and added), Yu Zhu (Evodiae Fructus) 6g, Bo Kou Ren (Cardamomi Fructus Rotundus) 6g (pounded), Fu Ling (Poria) 30g, Bai Hu Jiao (Piperis Longi Fructus) 3g (pounded).
Seventh visit Oct 31. Temp/pulse normal. Diarrhea stopped. Pathogenic toxins/dregs fully expelled. Urine clear/long. Abdominal distension/pain gone. Good appetite. Tongue root still white/greasy. Residual cold/warm pathogens not fully cleared. Yuan Yang needs continued warming/support.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 300g, Gan Jiang (Zingiberis Rhizoma) 30g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 10g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added), Xi Sha Ren (Amomi Fructus) 10g (pounded), Yi Yi Ren (Coicis Semen) 10g.
Eighth visit Nov 1. Greasy coating receded, slight thin white coating. Pulse/temp normal. Urine clear/long. Abdomen comfortable, no other pain. Appetite improving, eating 60-70% per meal to avoid harming Spleen/Stomach. Sleep better, but dreaming due to initial Yang recovery instability. Qi not full, dizziness/weak legs upon sitting/standing. Still support Yang/assist healthy Qi, making True Yang vigorous and pathogenic Yin extinct. Warming True Yang does not harm True Yin; True Yang return helps generate True Yin. This is "Yang generates Yin," "Heaven One generates Water." Balancing Yin/Yang cures all symptoms.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 300g, Gan Jiang (Zingiberis Rhizoma) 36g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 10g, Xi Sha Ren (Amomi Fructus) 10g, Zhu Yi Fu Shen (Poria with Red-Dyed Heart) 30g, Zhi Yuan Zhi (Polygalae Radix Preparata) 10g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added).
Ninth visit Nov 2. Pulse/temp normal. Tongue root thin white, substance red/vibrant. Sleep/diet improving, Stomach Qi opening. Still limit diet to 70-80%. Dizziness gone upon rising, legs still soft. Support Yang/assist healthy Qi.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 160g, Gan Jiang (Zingiberis Rhizoma) 30g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added), Xiao Hui (Foeniculi Fructus) 3g (lightly fried), Fu Ling (Poria) 16g.
Tenth visit Nov 3. Water/Qi transforming, gurgling in abdomen. Sleep/diet good. Legs weak when walking, indicating Yang spirit not full. Maintain previous method.
Bai Fu Pian (Aconiti Lateralis Radix Praeparata) 160g, Gan Jiang (Zingiberis Rhizoma) 30g, Gan Cao (Glycyrrhizae Radix et Rhizoma) 10g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added), Xi Sha Ren (Amomi Fructus) 6g, Bai Hu Jiao (Piperis Longi Fructus) 3g (pounded).
Eleventh visit Nov 4. Illness cured. Spirit/diet good, form/spirit still weak. One dose of Si Ni Tang (Frigid Extremities Decoction) modified. Followed by Huang Qi Jian Zhong Tang (Astragalus Regulate the Middle Decoction), Gui Fu Li Zhong Tang (Cinnamon-Atractylodes Regulate Middle Pill), Gui Pi Yang Xin Tang (Restore Spleen Nourish Heart Decoction) etc. for over ten days post-illness regulation. Spirit gradually recovered. Discharged home to rest. Health/constitution returned to normal thereafter.
Shaoyin Extreme Yin Appearing Like Yang Syndrome (I)
Yang XX, male, 31 years old, from Yao'an County, Yunnan Province. Ill for 20 days in March 1923. Initially slight wind-cold, fever, headache. Given over ten doses by a physician, each heavily using bitter-cold purgatives plus Xi Jiao (Rhinoceros Horn), Ling Yang Jiao (Antelope Horn), Huang Lian (Coptidis Rhizoma), etc. Condition worsened with each dose. Physician did not reflect, nearly fatal before consulting me. At that time: red eyes, swollen/scorched lips, barefoot/exposed body, irritability, insomnia, confusion, delirium, body hot as fire, thirst preferring boiling hot water, short red urine, no bowel movement for days, no food intake. Pulse floating, deficient, about to scatter. This is a changed syndrome from incorrect treatment of wind-cold. Overuse of bitter-cold purgatives forced True Yang outward, creating Extreme Yin appearing like Yang. Exterior shows heat image (false heat), interior is extremely cold (true cold). If truly Yang syndrome with internal heat steaming, should have great thirst for cold drinks, not preference for boiling hot? Moreover, pulse floating/deficient/about to scatter indicates Yuan Yang about to collapse. Past cold/purgative use must stop. Only large dose returning Yang and gathering [Qi] might save life. Condition critical. Urgently prescribed Bai Tong Tang (White Onion Decoction) plus Shang Rou Gui (Cinnamon Bark) one dose.
Fu Pian (Aconiti Lateralis Radix Praeparata) 60g, Gan Jiang (Zingiberis Rhizoma) 26g, Shang Rou Gui (Cinnamomi Cortex) 10g (powdered, soaked in water and added), Cong Bai (Allii Fistulosi Bulbus) 4 stems.
After prescribing, family said no one was present that evening to supervise decoction, dared not take. Next morning, urgently consulted again. Insisted on same formula, advised trying Shang Rou Gui (Cinnamon Bark) soaked in water first. If tolerated, decoct and take whole formula. No other good method. Family gave cinnamon water. Immediately vomited ~bowl of saliva/phlegm, consciousness slightly cleared, felt internally refreshed, took main formula. After one dose, condition reduced, showing aversion to cold and cold limbs. Afternoon re-examination: fever subsided 1-2 degrees, no longer irritable/delirious, slept briefly. Prescribed Si Ni Tang (Frigid Extremities Decoction) plus Shang Rou Gui (Cinnamon Bark) as main treatment.
