The rotting of the gum roots is not caused by Stomach Fire, but due to insufficiency of Kidney Water, leading to rampant Fire of the Taiyang Bladder channel, which combines with Heart Fire to become excessive. Treatment requires modification of Xiexin Tang (Drain the Epigastrium Decoction).
For upper toothache caused by Yin deficiency and rising deficient fire, Siwu Tang (Four Substances Decoction) combined with Shengma Gegen Tang (Kudzu and Rhizoma Cimicifugae Decoction) should be used.
If the tongue protrudes about one cun, applying Bingpian (Borneolum Syntheticum) will cause it to retract immediately.
For adults and children with swelling under the tongue, known as Chongshe (Double Tongue), accompanied by surging phlegm and difficulty speaking, mix Pengsha (Borax), Zhusha (Cinnabar), and Puxiao (Mirabilite) each at 5 fen, and Bingpian (Borneolum Syntheticum) at 1 fen into a powder. Mix with honey and apply externally.
For swollen gum roots and inability to open the mouth, press Badou (Fructus Crotonis Tiglii) to extract oil, soak it in grass paper, roll the paper into a strip, ignite it, blow out the flame, and use the smoke to fumigate the nose to restore mouth opening; alternatively, use Puxiao (Mirabilite) 1 qian 8 fen, Puhuang (Pollen Typhae) 4 fen, Jiangcan (Bombyx Batryticatus) 2 fen, Yazao (Gleditsia Sinensis Fruit) 1 fen, and Bingpian (Borneolum Syntheticum) 1 fen, grind into a fine powder, and insufflate into the nose.
After hematemesis, if there is rebellious Qi and sore throat, Fuling Buxin Tang (Poria Cocos Tonifying the Heart Decoction) is the primary treatment.
For Hou'e (Throat Moth/Epiglottitis-like lesions) in the throat, with surging phlegm and sore throat, use Danfan (Alumen) 3 fen and Pengsha (Borax) 2 fen, mixed with boiling water for oral administration, and use external insufflation.
Formula: Renzhongbai (Calcinata Humanis Ossis or similar calcined substance) 1 qian, Pengsha (Borax) 5 fen, Danfan (Alumen) 3 fen, Bingpian (Borneolum Syntheticum) 1 fen. Grind into powder and insufflate.
For mouth ulcers caused by Excess Heat, accompanied by irritability, thirst, and cheek pain, use Huoxiang (Patchouli Herba) 7 qian, Shigao (Gypsum Fibrosum), Zhizi (Gardeniae Fructus), and Zhi Gancao (Prepared Licorice Root) each 5 qian, and Fangfeng (Saposhnikoviae Radix) 4 qian. Grind into powder.
For mouth ulcers associated with an Excess pulse in the Large Intestine channel, use Shengjiang (Zingiberis Rhizoma Recens), Chenpi (Citri Reticulatae Pericarpium), Zhuru (Bambusae Caulis in Taenia), Huangqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), and Baizhu (Atractylodis Macrocephalae Rhizoma) each 5 qian, Guixin (Cinnamomi Cortex) 1 qian, Fuling (Poria), Mangxiao (Natrii Sulfas), Sheng Toumo (Head Powder of Raw Polygonum Multiflorum) 2 qian, and Sharen Toumo (Head Powder of Amomi Fructus) 1 qian 5 fen. Form into honey pills and hold in the mouth to dissolve.
Medical Case
A patient presented with sore throat and hemorrhoid pain, with deep and slow pulses in all six positions. This indicates insufficient Stomach Qi and Kidney Water failing to control Fire. Just as fire not being lit at the bottom of the pot leads to spleen dysfunction and weak water source, weakness leads to cold and rising deficient fire, hence symptoms like sore throat with a slow pulse. It is necessary to warm the "bottom of the pot" (Spleen/Stomach), so that water warms and rises, fluids are moistened, and floating deficient fire naturally calms, resolving the symptoms. Therefore, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) is inferior to Shenqi Dihuang Wan (Eight-Ingredient Pill with Rehmannia), especially when combined with herbs that strengthen the Spleen. Generally, when using tonics, regardless of whether the pathology is in the Upper, Middle, or Lower Jiao, restoring Spleen function must be prioritized. The Spleen is the root of postnatal generation; once the root is established, diseases recede naturally. Moreover, since the disease is located in the Kidney, if no attention is paid to the Spleen and Stomach first, even if there is efficacy in generating Water, there is no method to prevent water overflow, resulting in delayed therapeutic effects.
