Discussion states: Sanjiao distension (Sanjiao Distension) is what the classics refer to as "Qi filling the skin, appearing hard and taut but without pain." Generally, distension may present with pain or a sensation of blockage, which serves to distinguish between deficiency (Xu) and excess (Shi). For instance, in cases like drum distension (Guzhang), where old food stagnation is retained internally, pressing on the area reveals hardness and pain; this is termed pathogenic excess. However, in the current case of Sanjiao skin being hard and taut without pain, it is merely a deficiency-type distension caused by Qi fullness. The treatment should focus on regulating the ascending and descending of Qi, after which the condition will resolve.
Formula for Regulating Qi, Baizhu Juxiang Tang, to treat Sanjiao Qi fullness and hard, distended skin:
Baizhu (Atractylodis Rhizoma, four liang), Chenju pi (Citri Reticulatae Pericarpium Vetustum, peeled of white pith and dried), Chifuling (Poria Coccos with Red Skin, black skin removed), Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried and chopped, two liang each), Fuzi (Aconiti Lateralis Radix Praeparata, roasted until cracked, peel and navel removed, one liang), Ganjiang (Zingiberis Rhizoma, roasted, half liang).

Each dose consists of three qianbi (approx. 9-10g), boiled with one zhan (approx. 200ml) of water, two slices of fresh ginger, and one jujube, until reduced to seven-tenths. Strain and take warm. If cold invasion is suspected, add Jingjie (Schizonepetae Herba) during decoction. This formula is effective for any slight disharmony of the Sanjiao.
