Commentary states: When yang qi is excessively abundant (Chong Yang), mania ensues. This refers to the solitary hyperactivity of yang qi. In cold damage (Shanghan), when heat toxins are rampant, causing heat both internally and externally, yang qi becomes agitated and furious, leading to manic behavior that transcends normal limits. The disease causes individuals to run wildly and speak nonsensically. Some may curse without regard for kinship or distance; some may hallucinate, seeing or hearing things that are not there. In severe cases, they may strip naked and run, climb to high places and sing. Or, they may go several days without eating, yet still manage to jump over walls and climb onto roofs. This is because the four limbs are the root of all yang meridians; when heat is excessive, the limbs become full and strong. This is termed "repeated yang" (Chong Yang). Therefore, places they climb to are those they could never reach normally, but due to the illness, they can. If the condition arises from fire pathogenic factors causing fright and mania, or if there is internal blood stasis presenting with external symptoms resembling mania, although these diseases differ, they share the commonality of excess yang qi.
