Commentary states: After the umbilical cord is severed in a newborn, if the umbilical wound has not yet healed, one must prevent external invasion of Wind pathogenic factors and avoid allowing bathwater to enter the wound, thereby preventing dampness and cold from retaining within the body. These conditions can all lead to Umbilical Tetanus (Qifeng). The manifestations include swelling of the umbilicus and excessive crying. In severe cases, Wind pathogenic factors may disseminate through the meridians, resulting in trismus (lockjaw), inability to suckle milk, and opisthotonos (backward arching of the body), which may ultimately prove fatal and incurable.
For treating Umbilical Tetanus occurring within seven days after birth, characterized by umbilical swelling and impending separation of the cord, use Jin Gao San (Brocade Ash Powder):
Jinbo (Burned Ash of Silk Brocade, 1 qian) Xiongshufen (Excrement of Male Mouse, slightly stir-fried, 7 pieces) Dazao (Jujube, pitted, 3 fen) Shexiang (Musk, ground, a small amount)
Grind these four ingredients into a powder. Observe whether the umbilical cord is about to fall off but has not yet done so, and apply the powder to seal the umbilicus. Strictly avoid exposure to external Wind.
To prevent Umbilical Tetanus within the first month after birth, apply this powder immediately after severing the umbilical cord. Use Tianjiang San (Sky Pulp Powder):
Tianjiangzi (Sky Pulp Seeds, 3 pieces) Luangfa (Tangled Hair, burned to ash preserving its nature, 0.5 qian) Wugong (Centipede, 2 cun, burned to ash) Lingyangjiao (Antelope Horn, burned to ash, 1 qian) Shexiang (Musk, a small amount)
Grind these five ingredients into a fine, uniform powder. Immediately after cutting the umbilical cord, apply a small amount topically.
For treating Umbilical Tetanus in infants, use Bailong San (White Dragon Powder):
Tianjiangzi (with larva inside, 1 piece) Baijiangcan (White Silkworm, straight ones, stir-fried, 1 piece)
Grind these two ingredients into a powder, add a small amount of Nifen (Calomel/Calamine), and mix with natural juice of Bohe (Peppermint) for administration. This effectively expels toxic substances. Divide the dose according to the infant's size into two administrations.
For treating Umbilical Tetanus with swelling, use Shen Hui San (Divine Ash Powder):
Xiongshufen (Excrement of Male Mouse, 7 pieces, those with pointed ends are authentic) Ganjiang (Dried Ginger, a piece the size of a jujube) Zengdai (Steamer Cloth, a ball) Feimian (Red Silk Floss) Hufen (Lead Carbonate, stir-fried until golden) [Text incomplete here]
Burn these six ingredients to ash while preserving their medicinal properties, mix and grind finely, add a small amount of Musk, and prepare as a powder. Apply to seal the umbilicus when the cord is about to separate; this will cure the condition. If applied prophylactically before onset, the disease will not occur. During the burning process, ensure no other impure ashes are mixed in.
For treating chronic Umbilical Tetanus that does not heal, with oozing fluid, use Muli San (Oyster Shell Powder):
Muli (Oyster Shell, 1 piece) Xama (Toad, 1 piece)
Burn both ingredients to ash, grind finely into a powder. Apply a small amount to the umbilicus each time; this is highly effective.
For treating Umbilical Tetanus with oozing fluid, use Danggui San (Angelica Sinensis Powder):
Danggui (Radix Angelicae Sinensis, sliced and dried, 0.5 liang) Gancao (Radix Glycyrrhizae, roasted and cut, 1 fen) Qiandan (Minium, ground, 0.5 fen)
Except for Minium, grind the other three ingredients into a powder. Add Minium and mix thoroughly. Apply to the umbilicus to achieve cure.
For treating Umbilical Tetanus with continuous oozing, use Huangbai San (Phellodendron Bark Powder):
Huangbai (Cortex Phellodendri, bark removed, 1.5 liang) Fudiheimei (Soot from the bottom of the cauldron, ground, 3 fen) Luangfa (Tangled Hair, burned to ash and ground, 1 fen)
First grind Huangbai into powder, then mix with the other two ingredients uniformly. Apply to the umbilicus to achieve cure.
Another prescription:
Danggui (Radix Angelicae Sinensis, sliced and dried)
Grind this single ingredient into a powder and apply frequently.
Method for treating Umbilical Tetanus with mouth puckering (Cuokou) in swaddled infants:
Inspect the infant's upper and lower gums. If there is a white spot, the size of a bean, at the center of the mouth, this indicates the onset of the disease. Immediately pinch this spot firmly with fingernails, moving from outside to inside, completing a full circle. Slight bleeding is acceptable. Also pinch the two extremities of the white spot using the same method to cut off the internal and external connections. The patient should recover promptly.
