Discussion states: In all cases of childbirth, when the fetus and mother have separated, but the placenta (Baoyi) has not yet been expelled. This is called "Xi Bao" (Stagnant Placenta), likely due to excessive exertion during labor, followed by invasion of wind-cold pathogenic factors after delivery. This causes blood and Qi to become stagnant and obstructed, leading to weakness and inability to promote circulation, resulting in prolonged retention. This not only hinders the handling of the infant's umbilical cord but also endangers the mother's life. It is urgent to support the parturient woman, ensuring her body posture is upright, allowing her to rest briefly until her breathing stabilizes and she can move. Additionally, administer Zhuang Qi Yi Xue Tang San (Vigor-Qi and Benefit-Blood Powder/Decoction) to assist. This will make blood and Qi smooth and unobstructed, allowing the placenta to be expelled. If it is a primipara who lacks experience, seeing that the placenta has not come down, fearing it may hinder the infant's body, she might rashly cut the umbilical cord, or accidentally pull it off, or hold it insecurely, causing the placenta to rise directly and cover the heart, leading many to die prematurely. The ancients' method for cutting the umbilical cord required skilled attendants. They would prepare heavy objects such as gold, silver, copper, iron, or weights beforehand. Using a soft, well-worn cloth belt, they would knot one end securely, leaving the other end free. Immediately after cutting the cord, they would tie the umbilical cord to the cloth belt, securing it tightly. This ensured the placenta would not rise up to cover the heart.
